【摘要】：改革開(kāi)放三十多年來(lái),經(jīng)濟(jì)的騰飛、社會(huì)的發(fā)展,我國(guó)基本醫(yī)療衛(wèi)生服務(wù)領(lǐng)域取得了顯著的成績(jī)。然而在縣級(jí)政府之間人民群眾享有基本醫(yī)療衛(wèi)生服務(wù)是不均等的,這不僅不利于人民群眾健康的維護(hù),而且影響社會(huì)安定。如何實(shí)現(xiàn)縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化,推進(jìn)“人人享有基本醫(yī)療衛(wèi)生服務(wù)”目標(biāo),對(duì)濟(jì)南市這個(gè)東部省會(huì)城市來(lái)說(shuō)意義重大。論文以濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化為研究對(duì)象,采用實(shí)證研究分析濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)現(xiàn)狀,通過(guò)“理論基礎(chǔ)→現(xiàn)狀分析→經(jīng)驗(yàn)借鑒→解決對(duì)策”的思路展開(kāi)進(jìn)行論述。首先,論文詳細(xì)的闡釋了基本醫(yī)療衛(wèi)生服務(wù)均等化的相關(guān)概念和理論基礎(chǔ)。公平正義理論、福利經(jīng)濟(jì)理論和新公共管理理論為探討濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化問(wèn)題提供了有力的理論支撐。其次,論文構(gòu)建濟(jì)南的基本醫(yī)療衛(wèi)生服務(wù)評(píng)價(jià)指標(biāo)體系,并通過(guò)詳實(shí)的數(shù)據(jù)分析濟(jì)南縣級(jí)政府的基本醫(yī)療衛(wèi)生服務(wù)情況,得到兩個(gè)結(jié)論,即濟(jì)南市縣級(jí)政府確實(shí)存在基本醫(yī)療衛(wèi)生服務(wù)的不均等、濟(jì)南市市區(qū)與周邊縣級(jí)單位基本醫(yī)療衛(wèi)生服務(wù)差距較大。通過(guò)進(jìn)一步的數(shù)據(jù)分析論證,發(fā)現(xiàn)濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化問(wèn)題,具體表現(xiàn)為基本醫(yī)療衛(wèi)生資源配置差異較大、基本醫(yī)療衛(wèi)生服務(wù)質(zhì)量有待提高、基本醫(yī)療衛(wèi)生服務(wù)管理存在漏洞。緊接著分析造成不均等化的原因,具體是醫(yī)療衛(wèi)生資源投入不均、各縣級(jí)政府經(jīng)濟(jì)發(fā)展不均衡、基本醫(yī)療衛(wèi)生服務(wù)體系不健全、醫(yī)療衛(wèi)生機(jī)構(gòu)結(jié)構(gòu)問(wèn)題突出。再次,論文借鑒了國(guó)外推進(jìn)基本醫(yī)療衛(wèi)生服務(wù),促進(jìn)均等化的先進(jìn)實(shí)踐經(jīng)驗(yàn),為濟(jì)南市各縣級(jí)政府提供有益參考。最后,筆者針對(duì)濟(jì)南市基本醫(yī)療衛(wèi)生服務(wù)不均等化問(wèn)題,并結(jié)合國(guó)外經(jīng)驗(yàn),提出了一系列對(duì)策,以實(shí)現(xiàn)濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化。論文系統(tǒng)而全面的分析了,濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)情況,發(fā)現(xiàn)不均等化問(wèn)題,并找出造成不均等問(wèn)題的原因,最后為實(shí)現(xiàn)均等化提出對(duì)策。這樣不僅能夠?qū)崿F(xiàn)濟(jì)南市縣級(jí)政府基本醫(yī)療衛(wèi)生服務(wù)均等化,而且有利于為其他地市解決相類似問(wèn)題提供了有益借鑒。
[Abstract]:In the past thirty years, with the rapid development of economy and society, remarkable achievements have been made in the field of basic medical and health service. However, it is not equal for the people to enjoy basic medical and health services between county governments, which is not conducive to the maintenance of people's health, and affects social stability. How to realize the equalization of basic medical and health services for county governments and promote the goal of "basic medical and health services for all" is of great significance to Jinan, the capital city of eastern China. This paper takes the equalization of the basic medical and health services of the county government in Jinan as the research object, and analyzes the present situation of the basic medical and health service of the county government in Jinan by using the empirical research. Through the theoretical basis of the current situation analysis and experience to learn from the solutions to the idea of the development of the discussion. First of all, the paper explains in detail the basic medical and health services equalization related concepts and theoretical basis. The theory of fairness and justice, the theory of welfare economy and the theory of new public management provide powerful theoretical support for discussing the equalization of the basic medical and health services of the county government in Jinan. Secondly, the paper constructs the evaluation index system of the basic medical and health service in Jinan, and analyzes the basic medical and health service of the county government in Jinan through the detailed data, and obtains two conclusions. That is, the county government of Jinan does not have equal basic medical and health services, and the gap between the basic medical and health services between the urban areas of Jinan and the surrounding county-level units is large. Through further data analysis and demonstration, it is found that the problem of equalization of basic medical and health services of the county government in Jinan shows that the distribution of basic medical and health resources is quite different, and the quality of basic medical and health services needs to be improved. There are loopholes in the management of basic medical and health services. Then it analyzes the causes of inequality, such as uneven input of medical and health resources, unbalanced economic development of county governments, imperfect basic medical and health service system, and prominent problems in the structure of medical and health institutions. Thirdly, the paper draws lessons from the advanced practical experience of foreign countries to promote basic medical and health services and promote equalization, which provides a useful reference for the county governments in Jinan. Finally, the author puts forward a series of countermeasures to realize the equalization of the basic medical and health services of the county government in Jinan. The paper systematically and comprehensively analyzes the situation of the basic medical and health service of the county government in Jinan, finds out the problem of inequality, finds out the causes of the problem, and finally puts forward the countermeasures to realize the equalization. This can not only realize the equalization of the basic medical and health services of the county government in Jinan, but also provide useful reference for other cities to solve similar problems.
