【摘要】：深化醫(yī)藥衛(wèi)生體制改革是世界性的難題，是人民群眾最關(guān)心的社會(huì)熱點(diǎn)問(wèn)題之一，各國(guó)都經(jīng)歷了不同的改革階段。公立醫(yī)院在我國(guó)醫(yī)療服務(wù)體系中處于主導(dǎo)地位，是人民群眾看病就醫(yī)的主要場(chǎng)所，擔(dān)當(dāng)著為人民群眾提供基本醫(yī)療衛(wèi)生服務(wù)的重任，是體現(xiàn)政府公益性的主要載體。公立醫(yī)院改革的成敗直接關(guān)乎本輪深化醫(yī)藥衛(wèi)生體制改革的成敗。2010年，鄂爾多斯市被列為內(nèi)蒙古自治區(qū)公立醫(yī)院改革兩個(gè)試點(diǎn)地區(qū)之一，，最根本目的就是解決群眾看病難、看病貴的問(wèn)題，實(shí)現(xiàn)人人享有基本醫(yī)療衛(wèi)生服務(wù)的目標(biāo)要求。但是在政府財(cái)政投入有限、舊的體制機(jī)制難以打破的現(xiàn)實(shí)下，如何扭轉(zhuǎn)公立醫(yī)院的逐利行為，構(gòu)建新的醫(yī)療秩序，回歸公益性成了急需解決的難題。 本文以公共產(chǎn)品、新公共管理、新公共服務(wù)理論為基礎(chǔ)，從鄂爾多斯市公立醫(yī)院改革現(xiàn)狀入手，借鑒國(guó)內(nèi)外公立醫(yī)院改革理論研究和實(shí)踐經(jīng)驗(yàn)，在鄂爾多斯市公立醫(yī)院改革所取得成績(jī)基礎(chǔ)上，進(jìn)一步從公立醫(yī)院公益性淡化、醫(yī)療資源配置不合理、基層醫(yī)療衛(wèi)生機(jī)構(gòu)服務(wù)能力差、基本醫(yī)療保障體系不健全、公立醫(yī)院績(jī)效考核缺失、公立醫(yī)院的有效監(jiān)管未建立等方面，分析了鄂爾多斯市公立醫(yī)院改革中存在的問(wèn)題及其原因。并針對(duì)上述問(wèn)題從強(qiáng)化責(zé)任合理投入、優(yōu)化布局合理配置、深化公立醫(yī)院體制機(jī)制改革、建立完善公立醫(yī)院績(jī)效考評(píng)體系、建立有效的醫(yī)保控費(fèi)機(jī)制這五個(gè)方面為鄂爾多斯市公立醫(yī)院未來(lái)改革提出了建議。
[Abstract]:Deepening the reform of the medical and health system is a worldwide problem and one of the hot social issues that the people are most concerned about. Public hospitals are in the leading position in the medical service system of our country, which is the main place for the people to see a doctor. Bear the responsibility of providing basic medical and health services for the people. The success or failure of the public hospital reform is directly related to the success or failure of deepening the reform of the medical and health system. 2010. Ordos City is listed as one of the two pilot areas in the reform of public hospitals in Inner Mongolia Autonomous region. The most basic purpose is to solve the problem of difficult and expensive medical treatment for the masses. But in the reality of limited government financial investment and difficult to break the old institutional mechanism, how to reverse the profit-driven behavior of public hospitals and build a new medical order. Returning to public welfare has become a difficult problem that needs to be solved urgently. Based on the theory of public goods, new public management and new public service, this paper starts with the current situation of public hospital reform in Ordos, and draws lessons from the theoretical research and practical experience of public hospital reform at home and abroad. On the basis of the achievements made in the reform of public hospitals in Ordos, the public welfare of public hospitals is further desalinated, the allocation of medical resources is unreasonable, and the service capacity of primary medical and health institutions is poor. The basic medical security system is not perfect, the performance evaluation of public hospitals is missing, and the effective supervision of public hospitals has not been established. This paper analyzes the problems and causes in the reform of public hospitals in Ordos, and aims at the above problems from strengthening the reasonable input of responsibility, optimizing the distribution and rational allocation, and deepening the reform of the system and mechanism of public hospitals. The establishment and improvement of public hospital performance evaluation system and the establishment of an effective medical insurance fee control mechanism provide suggestions for the future reform of public hospitals in Ordos.
