利用增齡性定量指標(biāo)判定華中地區(qū)青少年生理年齡的研究

【摘要】: 在口腔醫(yī)學(xué)及法醫(yī)學(xué)研究領(lǐng)域,青少年個體生理年齡的判定方法研究已成為該學(xué)科的前沿課題,尋找一種合適準(zhǔn)確的判定方法對于法醫(yī)學(xué)個體鑒別以及口腔醫(yī)學(xué)的研究有相當(dāng)大的幫助。一直以來,大量的研究集中在如何利用不同的指標(biāo)進(jìn)行個體生理年齡判定系統(tǒng)的研究上。對于青少年人群采用的基本都是牙齡及骨齡的相關(guān)性增齡指標(biāo),并且相關(guān)的判定方法一般均采用圖形判定法,由于選取的指標(biāo)的不同以及判定者本身理論基礎(chǔ)的不同導(dǎo)致判定的結(jié)果準(zhǔn)確度有很大的差異,誤差較大。 本文在對國內(nèi)外具有代表性的個體生理年齡判定方法研究發(fā)展進(jìn)行總結(jié)綜述的基礎(chǔ)上,系統(tǒng)的論述了采用定量化指標(biāo)判定華中地區(qū)青少年人群個體生理年齡的理論和方法。個體的生理性增齡指標(biāo)主要集中在第三磨牙,頸椎,及牙根成熟指數(shù)這幾個指標(biāo),本文試圖通過這幾項指標(biāo)的定量化,探討新的個體年齡的判定方法。 第一部分利用Demirjian法判定第三磨牙發(fā)育程度,通過量表評分使其定量化;通過評分系統(tǒng)按照性別差異將其定量化,并推導(dǎo)其判定公式。 第二部分利用計算機(jī)系統(tǒng)建立第三、四頸椎的增齡性發(fā)育指標(biāo)的定量化方法,主要定量化指標(biāo)集中在頸椎椎體的高度(vertebral body height (AH), vertebral body height (H),posterior vertebral body height (PH))這三個指標(biāo),利用逐步線性回歸方法推導(dǎo)出個體年齡的判定公式。 第三部分利用全口曲面斷層片上發(fā)育牙齒的牙齒發(fā)育成熟指數(shù)建立關(guān)于牙齒發(fā)育增齡性指標(biāo)的定量化方法,同時利用逐步線性回歸方法推導(dǎo)出個體年齡的判定公式。 建立第三磨牙發(fā)育階段定量化指標(biāo)與個體生理年齡之間的判定公式(公式1): 男性:Age=8.33+1.51 Development stage 女性:Age=9.10+1.43 Development stage 建立利用第三、四頸椎的增齡性發(fā)育的定量化指標(biāo)與個體生理年齡之間的判定公式(公式2): 男性:Age=-0.528+4.534PH/AP3+1.463AH/H4+5.364AH/AP3-1.894 AH/PH4 +3.345AH/H3 女性:Age=1.679+5.503AH/AP3+3.432AH/H4+3.528H/P-3.278 PH/AP4+2.502 AH/H3 建立利用牙齒根尖孔的寬度的定量化指標(biāo)與個體生理年齡之間的判定公式(公式3):Age=9.231-0.723 N0-1.588X5+0.592S-0.099 s·N0 第四部分將其上三組判定公式用于判定青少年人群的生理年齡,比較其三種方法的適用范圍。針對不同年齡人群以上三種判定方法準(zhǔn)確度并不相同,青少年(8-10歲)早期公式3的判定效果較好,發(fā)育生長高峰期(11-13歲)公式2比較合適,青春期晚期(15-18歲)公式1效果最佳。 綜上所述,青春期個體的發(fā)育過程中的增齡性改變對于判定個體的生理年齡具有較佳的結(jié)果,定量化這些指標(biāo)后可以大大提高判定結(jié)果的準(zhǔn)確性。同時針對青春期的不同年齡人群也應(yīng)在粗略估計年齡的基礎(chǔ)上選用較為合適的定量化增齡指標(biāo)進(jìn)行個體生理年齡的判定。
[Abstract]:In the field of stomatology and forensic medicine, the study of the method of determining the individual physiological age of teenagers has become the frontier subject of this discipline. Finding a suitable and accurate method is of great help to the individual identification of forensic medicine and the study of stomatology. A large number of studies have been focused on how to use different indicators to determine the individual physiological age system. For young people, the basic is the age of teeth and bone age related to the aging index, and the related methods are generally used to determine the figure of the method, Because of the difference of the selected indexes and the different theoretical basis of the judge itself, the accuracy of the judgment results is very different, and the error is large. On the basis of summing up the research and development of representative individual physiological age determination methods at home and abroad, this paper systematically discusses the theory and method of using quantitative indexes to determine the individual physiological age of teenagers in central China. The physiological aging indexes of individuals are mainly in the third molar, cervical vertebrae and root maturation index. This paper attempts to discuss the new method of judging individual age through the quantification of these indexes. The first part uses Demirjian method to judge the development degree of the third molar, quantifies the third molar by rating scale, quantifies it according to the gender difference through the scoring system, and deduces its judging formula. The second part uses the computer system to establish the quantitative method of aging development index of the third and fourth cervical vertebrae. The main quantitative indexes focus on the three indexes of cervical vertebral body height and vertebral body height. The method of stepwise linear regression is used to deduce the determination formula of individual age. In the third part, the quantitative method of tooth development aging index is established by using the tooth development maturity index of the tooth on the whole surface section, and the determination formula of individual age is deduced by using the stepwise linear regression method. The formula is used to determine the physiological age of teenagers, The scope of application of the three methods is compared. The accuracy of the above three methods is not the same. The effect of early formula 3 is better, the peak growth period of development is 11-13 years old (2) formula 2 is more suitable, and the late puberty period is 15-18 years old) formula 1 is the best. To sum up, the aging changes in the development of puberty individuals have a better result in determining the physiological age of individuals, and the accuracy of the results can be greatly improved by quantifying these indicators. At the same time, according to the different ages of puberty, we should choose a more suitable quantitative aging index to judge the individual physiological age on the basis of rough estimation of age.
