藍(lán)苓降脂顆粒對(duì)高脂血癥脾虛痰瘀阻滯證臨床療效的影響

【摘要】：目的:初步評(píng)價(jià)藍(lán)苓降脂顆粒治療高脂血癥(脾虛痰瘀阻滯證)的臨床療效。方法:以湖南中醫(yī)藥大學(xué)第二附屬醫(yī)院心血管科篩選的60例高脂血癥患者為研究對(duì)象,中醫(yī)辨證為脾虛痰瘀阻滯證,隨機(jī)分為治療組(藍(lán)苓降脂顆粒組)30例及對(duì)照組(血脂康組)30例,以陽(yáng)性對(duì)照雙盲雙模擬的試驗(yàn)設(shè)計(jì)方式,治療組予以藍(lán)苓降脂顆粒一次1包,一日2次+模擬血脂康膠囊一次2粒,一日2次,對(duì)照組予以血脂康膠囊一次2粒,一日2次+模擬藍(lán)苓降脂顆粒,一次1包,一日2次,治療周期均為4周。分別觀(guān)察治療前后兩組患者中醫(yī)癥候積分、血脂水平改善情況、C反應(yīng)蛋白水平、患者總有效率等變化,通過(guò)統(tǒng)計(jì)得出研究結(jié)果,以評(píng)價(jià)分析藍(lán)苓降脂顆粒的臨床療效及安全性。結(jié)果:在中醫(yī)癥候方面,治療組總有效率93.3%優(yōu)于對(duì)照組總有效66.6%,說(shuō)明治療組治療高脂血癥在改善中醫(yī)癥候方面有明顯療效(P0.05)。血脂水平前后比較,兩組患者組內(nèi)比較降低TC、TG、LDL-C和升高HDL-C效果均顯著(P0.01)。兩組患者組間比較,治療前TC、TG、LDL-C、HDL-C無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);治療后兩組降低TC、TG水平療效相當(dāng),無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),治療組降低LDL-C和升高HDL-C療效優(yōu)于對(duì)照組(P0.05)。結(jié)果說(shuō)明治療組治療高脂血癥在降低低密度脂蛋白和升高高密度脂蛋白方面療效顯著。CRP水平前后比較,兩組組內(nèi)及組間比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),說(shuō)明治療組對(duì)CRP水平改善程度優(yōu)于對(duì)照組。臨床觀(guān)察期間,兩組均未發(fā)現(xiàn)嚴(yán)重不良反應(yīng)事件。結(jié)論:藍(lán)苓降脂顆粒治療高脂血癥脾虛痰瘀阻滯證患者,具有良好的調(diào)脂、改善中醫(yī)癥候積分及有效降低C-反應(yīng)蛋白水平,副作用少,安全可靠。
[Abstract]:Objective: to evaluate the clinical efficacy of Lanling Jiangzhi granule in the treatment of hyperlipidemia (syndrome of spleen deficiency and phlegm stasis). Methods: 60 cases of hyperlipidemia were selected from the Department of Cardiovascular Medicine of the second affiliated Hospital of Hunan University of traditional Chinese Medicine, and the syndrome differentiation of TCM was syndrome of spleen deficiency and phlegm stasis. They were randomly divided into treatment group (Luling Jiangzhi granule group, n = 30) and control group (Xuezhikang group, n = 30). Xuezhikang capsule was simulated twice a day and twice a day. The control group was given Xuezhikang capsule 2 tablets once a day, twice a day to simulate Luling Jiangzhi granule, 1 packet once, twice a day, and the treatment period was 4 weeks. Before and after treatment, the TCM symptom integral, the improvement of blood lipid level, the level of C-reactive protein and the total effective rate of the two groups were observed, and the results of the study were obtained by statistics. To evaluate the clinical efficacy and safety of Lanling Jiangzhi granules. Results: in terms of TCM symptoms, the total effective rate of the treatment group was 93.3% higher than that of the control group, which indicated that the treatment group had obvious curative effect in improving the TCM syndrome of hyperlipidemia (P0.05). Compared with before and after blood lipid level, the effect of decreasing TC,TG,LDL-C and increasing HDL-C in the two groups was significant (P0.01). There was no significant difference in TC,TG,LDL-C,HDL-C before treatment between the two groups (P0.05). There was no significant difference between the two groups in reducing TC,TG level after treatment (P0.05). The therapeutic effect of treatment group was better than that of control group in reducing LDL-C and increasing HDL-C (P0.05). The results showed that the treatment group had significant effect on lowering low density lipoprotein and increasing high density lipoprotein. Compared with before and after CRP level, the difference was statistically significant between the two groups (P0.05). The results showed that the improvement of CRP level in the treatment group was better than that in the control group. During the clinical observation, no serious adverse reaction events were found in both groups. Conclusion: Lanling Jiangzhi granule in treating hyperlipidemia patients with spleen deficiency, phlegm and blood stasis syndrome has good lipid regulation, improve TCM syndrome integral and effectively reduce the level of C-reactive protein, with less side effects, safe and reliable.
