【摘要】：目的:觀察益氣祛瘀飲對(duì)慢性心力衰竭氣虛血瘀證治療的臨床療效,并從中、西醫(yī)兩方面初步探究、討論其作用機(jī)理。方法:將符合診斷與納入標(biāo)準(zhǔn)的60例病例隨機(jī)分為治療組和對(duì)照組各30例,對(duì)照組給予西醫(yī)標(biāo)準(zhǔn)治療,治療組在西醫(yī)標(biāo)準(zhǔn)治療基礎(chǔ)上加用中藥益氣祛瘀飲,均治療28天,記錄患者治療前后心功能NYHA分級(jí)、氨基末端腦納素前體(NT-proBNP)、左室射血分?jǐn)?shù)(LVEF)、6分鐘步行距離和中醫(yī)證候積分療效,并采用SPSS17.0進(jìn)行統(tǒng)計(jì)學(xué)分析。成果:治療組心功能改善總有效率為100%,與對(duì)照組總有效率80%相比,有顯著差異(P0.05)。兩組血清NT-proBNP均有非常顯著下降(P0.01),但治療組治療后NT-proBNP的值明顯低于對(duì)照組(P0.01),兩組LVEF和6分鐘步行試驗(yàn)距離均有非常顯著上升(P0.01),且治療組改善幅度明顯高于對(duì)照組(P0.05)。對(duì)中醫(yī)評(píng)價(jià)指標(biāo)的比較中,中醫(yī)證候總積分治療組療效明顯優(yōu)于對(duì)照組(P0.05),單個(gè)癥狀評(píng)分療效治療組也優(yōu)于對(duì)照組,且除口干P0.05外,其余差異均有統(tǒng)計(jì)學(xué)意義。結(jié)論:益氣祛瘀飲聯(lián)合西藥標(biāo)準(zhǔn)治療慢性心力衰竭氣虛血瘀證,對(duì)于改善患者心功能,和改善無論總體還是單個(gè)中醫(yī)癥狀改善均較單用西藥治療有顯著作用。
[Abstract]:Objective: to observe the clinical effect of Yiqi Quyu decoction on qi deficiency and blood stasis syndrome of chronic heart failure and discuss its mechanism. Methods: sixty patients who met the criteria of diagnosis and inclusion were randomly divided into treatment group (n = 30) and control group (n = 30). The control group was treated with western medicine standard therapy. The treatment group was treated with traditional Chinese medicine Yiqi Quyu decoction on the basis of western medicine standard treatment for 28 days. The NYHA classification of cardiac function, amino-terminal nana precursor (NT-proBNP), 6-minute walking distance of left ventricular ejection fraction (LVEF),) and the curative effect of TCM syndromes were recorded before and after treatment. The results were statistically analyzed by SPSS17.0. Results: the total effective rate of heart function improvement in the treatment group was 100%, which was significantly different from that in the control group (80%) (P0.05). Serum NT-proBNP decreased significantly in both groups (P0.01), but the value of NT-proBNP in the treatment group was significantly lower than that in the control group (P0.01). The distance of LVEF and 6-minute walking test was significantly increased in both groups (P0.01). The improvement of the treatment group was significantly higher than that of the control group (P0.05). In the comparison of TCM evaluation indexes, the curative effect of the treatment group was significantly better than that of the control group (P0.05), the treatment group of single symptom score was also superior to the control group, and the other differences were statistically significant except for the dry mouth P0.05. Conclusion: the combination of Yiqi Quyu Yin and western medicine standard in the treatment of chronic heart failure syndrome of qi deficiency and blood stasis has a significant effect on improving the heart function of patients and improving the symptoms of both general and individual Chinese medicine.
