【摘要】：目的:運用系統(tǒng)評價方法,構建穩(wěn)定型心絞痛循證病機-證治體系,為提高臨床療效提供科學依據。方法:(1)檢索CBM、CNKI、VIP、Wanfang data各國內中文數據庫中中醫(yī)藥治療穩(wěn)定型心絞痛的臨床研究,篩選合格研究,采用Meta-分析的方法進行有效性及安全性的系統(tǒng)評價。(2)以有效、安全及統(tǒng)計結果穩(wěn)定為標準,篩選中醫(yī)藥治療穩(wěn)定型心絞痛的優(yōu)勢治法、方藥。結果:共檢索到相關臨床對照試驗研究28704篇,最終納入合格研究402篇。Jadad評分,60項研究得分≥3分,其余研究得分均3分。涉及湯劑、丸劑、散劑、注射劑等方藥155種,體現(xiàn)出益氣養(yǎng)陰、活血祛瘀、益氣活血祛瘀等12種治法及其相應治則的臨床指導作用。結論:(1)虛性病機—證治體系為氣虛-氣虛證-補氣法-黃芪注射液;氣陰兩虛-氣陰兩虛證-益氣養(yǎng)陰法-參麥注射液。(2)實性病機—證治體系為心血瘀阻-心血瘀阻證-活血祛瘀法-葛根素注射液、銀丹心腦通膠囊、丹參川芎嗪注射液、血府逐瘀湯、杏丁注射液、心可舒片、疏血通注射液、紅花黃色素注射液、復方丹參滴丸、血塞通膠囊。(3)虛實夾雜病機—證治體系為氣陰兩虛,瘀血阻滯-氣陰兩虛,瘀血阻滯證-益氣養(yǎng)陰,活血祛瘀法-穩(wěn)心顆粒;氣虛痰瘀阻絡-氣虛痰瘀阻絡證-芳香開竅,益氣活血祛瘀法-麝香保心丸;氣虛血瘀-氣虛血瘀證-益氣活血祛瘀法-補陽還五湯、通心絡膠囊。
[Abstract]:Objective: to establish a system of Evidence-Based Pathogenesis and treatment of stable angina Pectoris by systematic evaluation, and to provide scientific basis for improving clinical efficacy.
