【摘要】：鄭重光認(rèn)為三陽證的診治顯明易見,而對(duì)于亢害危殆的三陰證,在他看來,其似是而非的臨床表現(xiàn)決定了診斷是關(guān)鍵的一環(huán)。在各類疾病中,陰證似陽證的陰陽病機(jī)最不易判斷,常常表現(xiàn)出表、里、寒、熱、虛、實(shí)錯(cuò)雜繁復(fù)的特點(diǎn),向來最易造成誤診誤治。鄭重光認(rèn)為的陰證似陽證的脈象普遍特點(diǎn)是脈必沉細(xì)而疾,或沉遲無力,脈沉緊或散大。在表象都是熱癥的情況下,鄭重光注重病人的脈診結(jié)果,在脈證不吻合時(shí),鄭重光進(jìn)一步指出要"憑脈不憑證",以脈象來揭開繁雜的表象,探求疾病的本質(zhì),達(dá)撥亂反正之功,可見這種脈辨病機(jī)的思想是鄭重光辨識(shí)陰證的重要特點(diǎn)。鄭重光獨(dú)尊脈診,強(qiáng)調(diào)憑脈不憑證,但并不是說他忽視了其他診斷方式,脈證合參是其臨證的基礎(chǔ)。鄭重光辨識(shí)錯(cuò)綜復(fù)雜的陰證似陽證疾病有豐富的經(jīng)驗(yàn),他以脈證合參為辨證基礎(chǔ),同時(shí)又以脈診為直接判識(shí)病機(jī)的依據(jù),為后世醫(yī)家提供了可靠詳盡的參考,堪為后世所珍。
[Abstract]:Zheng Zhongguang thinks that the diagnosis and treatment of the three yang syndrome is obvious and easy to see, but for the three yin syndrome of hyperactivity, its specious clinical manifestation determines the diagnosis is a key link. Among all kinds of diseases, the pathogenesis of Yin and Yang syndrome is the most difficult to judge, often showing the characteristics of surface, cold, heat, vacuity and complexity, which is always the most likely to cause misdiagnosis and mistreatment. Zheng Chongguang thinks that the pulse of Yin syndrome like Yang syndrome is generally characterized by the pulse must be fine and disease, or late powerless, the pulse is tight or scattered. Under the condition that the appearance is fever, Zheng Chongguang pays attention to the result of pulse diagnosis of the patient. When the pulse syndrome does not match, Zheng Chongguang further points out that it is necessary to "rely on the pulse to uncover the complicated appearance and explore the essence of the disease by using the pulse sign." It can be seen that the thought of differentiating the pathogenesis of pulse is an important feature of Zheng Zhongguang's identification of Yin syndrome. Zheng Chongguang emphasizes that pulse is not proof, but does not mean that he neglects other diagnostic methods, pulse syndromes and ginseng are the basis of his clinical diagnosis. Zheng Zhongguang has rich experience in identifying the complex diseases of Yin syndrome like Yang syndrome. He takes pulse syndromes and ginseng as the basis of syndrome differentiation, and at the same time, he takes pulse diagnosis as the basis for the direct identification of pathogenesis, which provides reliable and detailed reference for the doctors of later generations.
