【摘要】：目的:基于清化承氣湯對(duì)慢性阻塞性肺疾病急性加重期(AECOPD)痰熱腑實(shí)證的臨床研究,客觀評(píng)價(jià)清化承氣湯治療AECOPD痰熱腑實(shí)證的有效性和安全性,為肺腸同治法治療AECOPD提供相應(yīng)的臨床數(shù)據(jù)。方法:將符合納入標(biāo)準(zhǔn)的60例AECOPD痰熱腑實(shí)證的患者隨機(jī)分為治療組和對(duì)照組各30例,兩組均采用符合西醫(yī)診療規(guī)范的治療方案,治療組在此基礎(chǔ)上加用清化承氣湯,臨床觀察9天,以SPSS21為統(tǒng)計(jì)軟件,通過對(duì)比兩組患者治療前后的相關(guān)體征及癥狀評(píng)分等數(shù)據(jù)來評(píng)價(jià)兩組的療效。結(jié)果:入組病例以65-75歲的男性居多。經(jīng)治療,兩組體征和癥狀評(píng)分都得到了明顯的改善,治療組總有效率達(dá)93.3%,對(duì)照組總有效率達(dá)86.7%,兩組間在咳嗽、咳痰、呼吸困難、腹脹、便秘等主要癥狀,聽診Up音、肺功能(FEV_1/FVC,FEV_1%)、有效率、有效所需天數(shù)等方面存在統(tǒng)計(jì)學(xué)差異。結(jié)論:對(duì)于AECOPD痰熱腑實(shí)證的治療,加用清化承氣湯以瀉熱通腑的治療組療效優(yōu)于單純西醫(yī)治療組,對(duì)咳嗽、咳痰、呼吸困難等肺系癥狀,腹脹、便秘等腑氣不通癥狀有更為明顯的改善作用,進(jìn)而提高AECOPD痰熱腑實(shí)證患者的生命質(zhì)量,說明以肺與大腸相表里理論為指導(dǎo)的肺腸同治法可以應(yīng)用于AECOPD痰熱腑實(shí)證,具有臨床意義和社會(huì)效益,值得進(jìn)一步深入研究。
[Abstract]:Objective: to evaluate the efficacy and safety of Qinghua Chengqi decoction in treating (AECOPD) phlegm-heat syndrome in acute exacerbation of chronic obstructive pulmonary disease (COPD). To provide the corresponding clinical data for the treatment of AECOPD. Methods: sixty patients with AECOPD phlegm and heat were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group were treated with Qinghua Chengqi decoction. After 9 days of clinical observation, SPSS21 was used as statistical software to evaluate the efficacy of the two groups by comparing the data of signs and symptom scores before and after treatment. Results: male patients aged 65-75 years were the majority of the patients. After treatment, the physical signs and symptom scores of the two groups were obviously improved. The total effective rate of the treatment group was 93.3 and the total effective rate of the control group was 86.7. The main symptoms of the two groups were cough, expectoration, dyspnea, abdominal distension, constipation, and so on. Auscultation Up sounds. There were significant differences in lung function (FEV_1/FVC,FEV_1%), effective rate and effective days. Conclusion: for the treatment of AECOPD syndrome of phlegm and heat, the treatment group with Qinghua Chengqi decoction for purging heat and clearing the Fu is better than the western medicine group in the treatment of cough, expectoration, dyspnea and other pulmonary system symptoms, abdominal distension, and so on. Constipation and other symptoms can improve the quality of life of patients with phlegm and heat in AECOPD. It shows that the method of lung and intestine syndromes therapy can be applied to the diagnosis of phlegm and heat in AECOPD, which is guided by the theory of lung and large intestine. It has clinical significance and social benefit, and is worthy of further study.
