【摘要】：目的：本課題采用宮血停方治療圍絕經(jīng)期腎陰虧虛型崩漏患者,并與對照組(炔諾酮片)比較,觀察治療后患者陰道出血、中醫(yī)癥狀、血紅蛋白及月經(jīng)的改善情況,客觀評價兩種治療方法的療效差異。方法：選擇60例符合標(biāo)準(zhǔn)的診刮術(shù)后一周仍見陰道不規(guī)則出血的圍絕經(jīng)期腎陰虧虛型崩漏患者,按隨機(jī)數(shù)字表法分為治療組.(宮血停方組)、對照組(炔諾酮片組)各30例。觀察兩組治療后陰道出血情況及治療3個月經(jīng)周期后的中醫(yī)癥狀改善情況、兩組停藥后3個月的月經(jīng)情況、并測定治療前后血紅蛋白含量的變化情況。最后進(jìn)行統(tǒng)計學(xué)處理及分析。結(jié)果：1.治療組總有效率80.00%；對照組總有效率83.33%。兩組患者綜合療效比較,差異無統(tǒng)計學(xué)意義(P0.05),表明兩組綜合療效相當(dāng)。2.兩組患者治療后與各自治療前癥狀總積分比較,均有顯著性差異(P0.05)；兩組患者治療后組間癥狀總積分比較,差異有顯著性意義(P0.05)；表明治療組在改善中醫(yī)癥狀方面優(yōu)于對照組。3.兩組患者治療后陰道止血情況比較,差異無統(tǒng)計學(xué)意義(P0.05),表明治療組與對照組在陰道止血療效上相當(dāng)。4.兩組患者治療后與各自治療前血紅蛋白含量的變化比較,有顯著性差異(P0.05)；兩組患者治療后組間血紅蛋白含量比較,差異有顯著性意義(P0.05)；表明治療組在提高血紅蛋白含量方面優(yōu)于對照組。5.兩組治療后隨訪3個月的組間月經(jīng)情況比較,差異均有顯著性意義(P0.05),表明治療組的遠(yuǎn)期調(diào)經(jīng)療效優(yōu)于對照組。結(jié)論：采用中醫(yī)藥辨證施治“澄源、復(fù)舊”是治崩的延續(xù),運(yùn)用宮血停方治療圍絕經(jīng)期腎陰虧虛型崩漏患者,對于遠(yuǎn)期調(diào)經(jīng)療效較有優(yōu)勢,從而提高治療效果及降低復(fù)發(fā)率。
[Abstract]:Objective: to observe the improvement of vaginal bleeding, symptoms of traditional Chinese medicine, hemoglobin and menstruation in patients with peri-menopausal kidney yin deficiency type insipidus treated with Gong Xue stopping prescription, and compared with control group (norethisterone tablet). Objective to evaluate the difference of therapeutic effect between the two methods. Methods: sixty cases of peri-menopausal kidney yin deficiency type with irregular vaginal bleeding one week after curettage were selected and randomly divided into treatment group. (Gongxue stopping prescription group), control group (norethisterone tablet group) 30 cases each. The vaginal bleeding after treatment and the improvement of TCM symptoms after 3 menstrual cycles were observed in the two groups. The menstruation of the two groups after 3 months' withdrawal was observed and the changes of hemoglobin content before and after treatment were measured. Finally, statistical processing and analysis are carried out. The result is 1: 1. The total effective rate was 80.00in the treatment group and 83.33g in the control group. Two groups of patients comprehensive efficacy comparison, the difference is not statistically significant (P0.05), indicating that the two groups of comprehensive efficacy is equivalent. 2. Two groups of patients after treatment and their symptoms before the total score, there were significant differences (P0.05); two groups of patients after treatment, the total symptom score comparison, significant difference (P0.05); show that the treatment group in improving TCM symptoms than the control group. 3. There was no significant difference between the two groups in vaginal hemostasis after treatment (P0.05), indicating that the treatment group and the control group in the vaginal hemostatic efficacy was equivalent. 4. There was significant difference in hemoglobin content between the two groups after treatment and before treatment (P0.05), and there was significant difference in hemoglobin content between the two groups after treatment (P0.05). The results showed that the treatment group was superior to the control group in increasing hemoglobin content. There were significant differences in menstruation between the two groups after 3 months follow-up (P0.05), which indicated that the long-term menstrual adjustment effect in the treatment group was better than that in the control group. Conclusion: the treatment of "Chengyuan, restoring old" by TCM differentiation is the continuation of the treatment of avalanche. The treatment of peri-menopausal patients with deficiency of kidney yin and deficiency of kidney yin by using Gongxue stopping prescription has an advantage in long-term menstrual regulation, thus improving the therapeutic effect and reducing the recurrence rate.
