【摘要】：目的臨床觀察參芪益腎湯治療糖尿病腎病前后期間,患者的各項(xiàng)指標(biāo)變化和治療效果。方法此次臨床研究選取60例糖尿病腎病患者,并隨機(jī)劃分為治療組和對(duì)照組各30例患者,符合臨床納入標(biāo)準(zhǔn)要求,及中醫(yī)辨證分型標(biāo)準(zhǔn)。在基礎(chǔ)治療上,治療組服用參芪益腎湯和對(duì)照組服用厄貝沙坦片,療程均為16周。觀察患者血常規(guī)、尿常規(guī)、肝功、腎功、血脂、血糖、糖化血紅蛋白、24小時(shí)尿蛋白定量、尿微量白蛋白、氧化低密度脂蛋白受體的影響及中醫(yī)證候積分進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果受試患者通過(guò)口服參芪益腎湯后提高了治療糖尿病腎病臨床效果。臨床治療組有效率達(dá)到83.32%,而對(duì)照組有效率達(dá)到63.32%,治療組有效率高于對(duì)照組(P0.01);患者證候積分均較治療前明顯降低(P0.01),治療組明顯優(yōu)于對(duì)照組(P0.01);患者服藥后24小時(shí)尿蛋白定量、尿微量白蛋白明顯下降,治療組優(yōu)于對(duì)照組(P0.05);甘油三酯下降(P0.01);總膽固醇下降(P0.05);血糖下降(P0.05);糖化血紅蛋白下降(P0.01);氧化低密度脂蛋白受體降低(P0.05);治療后兩組未出現(xiàn)生化指標(biāo)改變。結(jié)論1.臨床應(yīng)用參芪益腎湯治療氣陰兩虛兼痰瘀型糖尿病腎病的有效率、改善中醫(yī)證候明顯優(yōu)于對(duì)照組。2.受試患者服藥后能使糖尿病腎病患者Ⅲ期的24小時(shí)尿白蛋白定量、尿微量白蛋白、氧化低密度脂蛋白受體、指標(biāo)下降。3.可以調(diào)節(jié)血糖和脂類代謝紊亂。4.減慢糖尿病腎病病情發(fā)展。
[Abstract]:Objective To observe the changes and therapeutic effects of Shenqi Yishen Decoction (Shenqi Yishi Decoction) in the treatment of diabetic nephropathy. Methods 60 patients with diabetic nephropathy were selected and divided into 30 patients in the treatment group and the control group, which were in accordance with the clinical inclusion criteria and the standard of TCM syndrome differentiation. On the basis of basic treatment, The treatment group took Shenqi Yishi Decoction and the control group to take Irbesartan Tablets for 16 weeks. The blood routine, urine routine, liver function, kidney work, blood lipid, blood glucose, glycemic hemoglobin, 24 hour urine protein quantitative, urine microalbuminuria, oxidized low density lipoprotein receptor and TCM syndrome score were statistically analyzed. The effective rate of the treatment group was 83.32%, the effective rate of the control group was 83.32%, the control group was 63.32%, the effective rate of the treatment group was higher than that of the control group (P0.01); the syndrome score of the patients was significantly lower than that before the treatment (P0.01), and the treatment group was obviously superior to the control group (P0.01), and the patient was 24 after taking the medicine. Hourly urine protein quantitative, urine microalbumin decreased significantly, the treatment group was better than the control group (P0.05), triglyceride drop (P0.01), total cholesterol drop (P0.05), blood glucose drop (P0.05), glycated hemoglobin (P0.01) and low density lipoprotein receptor reduction (P0.05); the two groups did not appear biochemical changes after treatment. Conclusion 1. clinical application of the conclusion The effective rate of Qi Yishen Decoction in the treatment of diabetic nephropathy with Qi Yin deficiency and phlegm stasis type diabetic nephropathy is better than that of the control group. The 24 hours urine albumin quantitation, urine microalbumin, oxidized low density lipoprotein receptor, and the decrease of.3. can regulate blood glucose and lipid metabolism turbulence in the patients with diabetic nephropathy after the treatment of.2. patients. The disorder of.4. slowed the development of diabetic nephropathy.
