【摘要】：研究目的:觀察運(yùn)脾益腎,降濁通絡(luò)法治療早中期糖尿病腎病的臨床療效。研究方法:根據(jù)2型糖尿病、早中期糖尿病腎病的臨床診斷標(biāo)準(zhǔn),采用隨機(jī)對(duì)照方法,納入60例早中期糖尿病腎病患者,按1:1對(duì)照原則,隨機(jī)分為中藥組、西藥組,在常規(guī)治療的基礎(chǔ)上,前者給予運(yùn)脾益腎湯治療,后者給予纈沙坦(80mg)治療。12周為1療程,1個(gè)療程后,進(jìn)行療效評(píng)價(jià)。統(tǒng)計(jì)兩組患者的癥狀積分、MALB、24h尿蛋白定量,血糖血脂,纖維蛋白原,腎功、GFR、UAER等,采用spss17.0進(jìn)行統(tǒng)計(jì)學(xué)分析。研究結(jié)果:1.比較中藥組與西藥組減輕中醫(yī)癥狀方面,前者總有效率是83%,后者總有效率是67%。2.運(yùn)脾益腎湯有較好的降低尿蛋白作用,統(tǒng)計(jì)結(jié)果顯示:P0.05。3.運(yùn)脾益腎湯在控制血糖血脂方面,療效突出,統(tǒng)計(jì)結(jié)果顯示:P0.05。4.運(yùn)脾益腎湯具有改善腎功能作用,統(tǒng)計(jì)結(jié)果顯示:P0.05。5.運(yùn)脾益腎湯在降血漿纖維蛋白原方面,療效突出,統(tǒng)計(jì)結(jié)果顯示:P0.05。結(jié)論:運(yùn)脾益腎湯改善早中期糖尿病腎病患者的臨床癥狀,降低尿蛋白、調(diào)節(jié)血糖血脂、改善腎功能、降低纖維蛋白原等方面,療效突出。
[Abstract]:Objective: to observe the clinical effect of removing spleen and tonifying kidney and reducing turbidity and dredging collaterals in the treatment of early and middle stage diabetic nephropathy. Methods: according to the clinical diagnostic criteria of type 2 diabetes mellitus and early and middle stage diabetic nephropathy, 60 patients with early and middle stage diabetic nephropathy were randomly divided into Chinese medicine group and western medicine group according to the 1:1 control principle. On the basis of routine treatment, the former was treated with Yunpi Yishen decoction, while the latter was treated with valsartan (80mg) for 12 weeks as a course of treatment. After one course of treatment, the curative effect was evaluated. The symptom scores of the two groups were analyzed by spss17.0, including 24 hours urinary protein quantification, blood glucose, blood lipids, fibrinogen, renal function GFR UAER, etc. The result of the study was: 1. Compared with the traditional Chinese medicine group and the western medicine group, the total effective rate of the former is 833 and the latter is 67.2. Yunpi Yishen decoction has a better effect on reducing urine protein, and the statistical results show that: P0.05.3. Yunpi Yishen decoction in the control of blood glucose and lipids, the curative effect is outstanding, the statistical result shows that: P0.05.4. Yunpi Yishen decoction can improve renal function, and the statistical results show that: P0.05.5. The effect of Yunpi Yishen decoction on reducing plasma fibrinogen was remarkable, and the statistical results showed that: P 0. 05. Conclusion: Yunpi Yishen decoction can improve the clinical symptoms, reduce urinary protein, regulate blood glucose and blood lipid, improve renal function and reduce fibrinogen in patients with early and middle stage diabetic nephropathy.
