【摘要】：目的：通過探討針刺曲泉穴對腦梗死患者心率變異性的影響,以及針刺時(shí)不同時(shí)間點(diǎn)的相關(guān)性,為臨床使用曲泉穴治療腦梗死提供理論依據(jù),為選擇針刺曲泉穴的安全性奠定理論基礎(chǔ)。方法：隨機(jī)選取來自黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院針灸科門診的腦梗死患者30例,男女各15人,身高、體重不限,進(jìn)行針刺曲泉穴的心率變異性監(jiān)測,記錄針刺前5分鐘,進(jìn)針并得氣時(shí),針刺后5min,針刺后10 min(行針時(shí)),針刺后15min,出針時(shí)(針刺后20 min),出針后5分鐘,七個(gè)時(shí)間點(diǎn)的參數(shù)值。將所取的RR間期標(biāo)準(zhǔn)差(SDNN).相鄰RR間期之差的均方根值(RMSSD).愛丁堡指數(shù)(PNN50),高頻(HF)成分和低頻(LF)和低頻成分和高頻成分的比值(LF/HF),進(jìn)行分析和測定HRV的變化,從而判斷針刺曲泉穴對自主神經(jīng)系統(tǒng)功能的影響,為臨床提供治療依據(jù)。結(jié)果：針刺曲泉穴在針刺時(shí)對LF的影響與針刺前5分鐘相比有明顯變化(P0.05)；針刺曲泉穴HF在針刺時(shí),針刺后5分鐘,針刺后10分鐘(行針),行針后5分鐘,出針時(shí),與針刺前5分鐘相比具有明顯變化(P＜0.05)：LF/HF在針刺后5分鐘,行針后5分鐘與針刺前5分鐘相比有明顯變化(P0.05)；SDNN在針刺時(shí),針刺后5分鐘,針刺后10分鐘(行針),行針后5分鐘,行針后10分鐘,出針后5分鐘與針刺前5分鐘相比均沒有明顯變化(P0.05)；RMSSD在針刺時(shí),針刺后5分鐘,針刺后10分鐘(行針),行針后5分鐘,行針后10分鐘,出針后5分鐘與針刺前5分鐘相比均沒有明顯變化(P)0.05):VLF在針刺時(shí),針刺后5分鐘,針刺后10分鐘(行針),行針后5分鐘,行針后10分鐘,出針后5分鐘與針刺前5分鐘相比均沒有明顯變化(P0.05)。結(jié)論：1、針刺曲泉穴進(jìn)針時(shí)LF減小,提示針刺時(shí)交感神經(jīng)張力降低。2、針刺曲泉穴HF減小,提示針刺曲泉穴可以降低迷走神經(jīng)張力。3、針刺曲泉穴對LF/HF值有影響,提示針刺曲泉穴能調(diào)節(jié)自主神經(jīng)系統(tǒng)的平衡狀態(tài)。
[Abstract]:Objective: to explore the effect of acupuncture on heart rate variability of patients with cerebral infarction and the correlation of different time points during acupuncture, so as to provide a theoretical basis for clinical use of Ququan point in the treatment of cerebral infarction. For the selection of acupuncture Ququan point safety lay a theoretical foundation. Methods: thirty patients with cerebral infarction, 15 male and 15 male, were randomly selected from the Department of Acupuncture and moxibustion Department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The heart rate variability was monitored by acupuncture at Ququan point and recorded 5 minutes before acupuncture. 5 minutes after acupuncture, 10 min after acupuncture, 15 minutes after acupuncture, and 5 minutes after acupuncture (20 min), after acupuncture, 7 time points). The RR interval standard deviation (SDNN) is obtained. The root mean square value (RMSSD) of the difference between adjacent RR intervals. Edinburgh index (PNN50), high frequency (HF) and low frequency (LF) and low frequency (LF / HF) ratio (LF / HF) were analyzed and determined to determine the effect of acupuncture on the function of autonomic nervous system. Results: the effect of acupuncture at Ququan point on LF was significantly different from that at 5 minutes before acupuncture (P0.05), while at acupuncture point HF, 5 minutes after acupuncture, 10 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes after acupuncture. Compared with the first 5 minutes of acupuncture, there were significant changes (P < 0.05) in 5 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes before acupuncture (P0.05) SDNN in acupuncture, 5 minutes after acupuncture, and 5 minutes after acupuncture. 10 minutes after acupuncture, 5 minutes after acupuncture, 10 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes before acupuncture (P0.05), 5 minutes after acupuncture, 5 minutes after acupuncture. 10 minutes after acupuncture, 5 minutes after acupuncture, 10 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes after acupuncture (P) 0.05): at acupuncture, 5 minutes after acupuncture, 10 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes after acupuncture. 10 minutes after acupuncture, 5 minutes after acupuncture and 5 minutes before acupuncture had no significant change (P0.05). Conclusion the LF decreased when acupuncture at Ququan point, indicating that the tension of sympathetic nerve was decreased by 0.2 and HF was decreased by acupuncture at Ququan point, indicating that acupuncture at Ququan point could decrease the tension of vagus nerve, and the acupuncture of Ququan point had an effect on LFP-HF value. The results suggest that acupuncture at Ququan can regulate the balance of autonomic nervous system.
