電針經(jīng)筋結(jié)點(diǎn)治療腦卒中痙攣期膝過伸的臨床療效觀察

【摘要】:目的:通過對(duì)比傳統(tǒng)針刺療法結(jié)合電針和針刺大腿后側(cè)經(jīng)筋結(jié)點(diǎn)結(jié)合電針治療患者痙攣期膝過伸的療效,為痙攣期膝過伸提供新的治療思路。方法:隨機(jī)選擇40例腦卒中痙攣期膝過伸患者,患者樣本均來(lái)自2016年5月至2016年12月黑龍江中醫(yī)藥大學(xué)附屬第二醫(yī)院針灸一門診及病房,樣本經(jīng)改良Ashworth分級(jí)為1級(jí)至3級(jí)、肌力分級(jí)在3級(jí)至4級(jí)、Brunnstrom分級(jí)Ⅲ級(jí)至Ⅴ級(jí),均符合腦卒中診斷標(biāo)準(zhǔn)及納入標(biāo)準(zhǔn),且為病情穩(wěn)定的患者。將患者按信封法分成兩組,即治療組和對(duì)照組,治療組20例,對(duì)照組20例。兩組患者均進(jìn)行神經(jīng)內(nèi)科常規(guī)藥物治療,同時(shí),對(duì)照組患者采用頭針體針針刺及電針治療,治療組患者頭針、上肢、小腿選穴均同對(duì)照組,但大腿選擇針刺后部經(jīng)筋結(jié)點(diǎn)。針刺得氣后,連接電療儀,采用疏波,波幅大小應(yīng)以患者能夠耐受為度。治療6天,休息1天,連續(xù)治療6周。根據(jù)Holden行走功能評(píng)定、Loudon膝過伸診斷方法、膝關(guān)節(jié)ROM評(píng)定等進(jìn)行療效評(píng)定。實(shí)驗(yàn)數(shù)據(jù)的錄入及統(tǒng)計(jì)分析均采用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行分析。結(jié)果:1.按照治療后Loudon膝過伸療效評(píng)定,治療組有效18例,無(wú)效2例;對(duì)照組有效12例,無(wú)效8例。治療組和對(duì)照組治療后膝過伸程度均較治療前有所改善,且治療組膝過伸療效的有效率明顯高于對(duì)照組(p0.05)。2.兩組患者治療前與治療后Holden功能分級(jí)療效比較,治療后結(jié)果明顯優(yōu)于治療前(p0.05),對(duì)照組行走功能較治療組有所改善。3.兩組患者治療前后膝關(guān)節(jié)伸展位ROM比較,兩組膝過伸角度均較治療前有所改善,且治療組結(jié)果明顯優(yōu)于對(duì)照組(p0.05)。結(jié)論:1.傳統(tǒng)針刺方法結(jié)合電針和針刺大腿后側(cè)經(jīng)筋結(jié)點(diǎn)法結(jié)合電針,兩種治療方法對(duì)痙攣期膝過伸均有效。2.針刺大腿后側(cè)經(jīng)筋結(jié)點(diǎn)結(jié)合電針治療腦卒中痙攣期膝過伸有效,治療效果優(yōu)于傳統(tǒng)針刺結(jié)合電針。
[Abstract]:Objective: to compare the curative effect of traditional acupuncture therapy combined with electroacupuncture and acupuncture on the posterior side of thigh meridian and electroacupuncture in the treatment of spastic knee hyperextension in order to provide a new treatment idea for spastic knee hyperextension. Methods: forty patients with spastic knee extension in stroke were randomly selected. The samples were collected from the first clinic and ward of the second affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from May 2016 to December 2016.
