【摘要】：目的:探討以清熱解毒、健脾補腎、宣肺調(diào)腸、暢達三焦為組方原則的糖胰生1號方對中重度血糖升高的2型糖尿病的臨床療效,探究其原理。方法:(1)臨床研究:從符合條件的患者中選取70例,隨機抽35例作為試驗組,35例對照。兩組均在基礎治療的基礎上,再分別給予糖胰生1號方聯(lián)合二甲雙胍(格華止)和二甲雙胍(格華止)治療,以12周為一個療程。比較研究前后,患者的臨床癥狀、體征、實驗指標的差異。(2)實驗研究:采用STZ作為造成2型糖尿病大鼠的實驗用藥。從造模成功的Wistar雄性大鼠中選取4組,每組10只,共40只:模型組、二甲雙胍組、糖胰生1號方低劑量組、糖胰生1號方高劑量組。空白組為10只正常大鼠。給藥觀察6周后,分別采用并用酶聯(lián)免疫方法檢測門靜脈中GLP-1、免疫組化法測定小腸與結(jié)腸中GLP-1的表達。結(jié)果:(1)臨床研究:糖胰生1號方不僅對2型糖尿病患者的臨床癥狀及體征具有明顯的改善,在降低血糖、HbA1c、增加C肽及胰島素釋放及調(diào)節(jié)血脂的方面臨床亦有明顯療效,優(yōu)于對照組。(2)實驗研究:糖胰生1號方不僅可明顯改善糖尿病大鼠的空腹血糖、減輕體重、促進引導素分泌,還可明顯增強GLP-1的分泌與表達,高劑量中藥組對大鼠的療效優(yōu)于各對照組。結(jié)論:以清熱解毒、健脾補腎、宣肺調(diào)腸、暢達三焦為組方原則的糖胰生1號方對中重度血糖升高的2型糖尿病患者療效確切,值得深入研究,推測可能與改善糖代謝、激發(fā)胰島功能有關(guān)。
[Abstract]:Objective: to explore the clinical effect of Tangyi Shengsheng 1 prescription on type 2 diabetes mellitus with moderate and severe hyperglycemia based on the principle of clearing heat and detoxification, invigorating spleen and kidney, promoting lung and regulating intestine, and Changda Sanjiao. Methods Clinical study: 70 patients were selected from eligible patients and 35 patients were randomly selected as control group. On the basis of basic treatment, the two groups were treated with Tang-Yisheng No. 1 prescription combined with metformin and metformin for 12 weeks. To compare the clinical symptoms, signs and experimental indexes of the patients before and after the study. Experimental study: STZ was used as the experimental drug to cause type 2 diabetes in rats. A total of 40 male Wistar rats were divided into 4 groups: model group, metformin group, low dose group and high dose group. The blank group consisted of 10 normal rats. After 6 weeks of administration GLP-1 was detected by enzyme-linked immunosorbent assay (Elisa) and the expression of GLP-1 in small intestine and colon was detected by immunohistochemistry. Results: the clinical study showed that Tang Yisheng No. 1 prescription not only improved the clinical symptoms and signs of type 2 diabetes mellitus, but also decreased blood glucose and HbA1c, increased the release of C peptide and insulin, and regulated blood lipids. The experimental study showed that Tang Yisheng No. 1 could not only improve the fasting blood glucose, reduce body weight, promote the secretion of guiding hormone, but also enhance the secretion and expression of GLP-1 in diabetic rats. The therapeutic effect of high dose Chinese medicine group on rats was better than that of other control groups. Conclusion: Tang Yisheng 1 prescription, which is based on the principle of clearing heat and detoxifying toxin, invigorating spleen and kidney, promoting lung and regulating intestine, and Changda Sanjiao, is effective in treating type 2 diabetes with moderate and severe hyperglycemia. It is worthy of further study, and it may be speculated and improved on glucose metabolism. Activation of islet function.
