【摘要】：目的:通過調(diào)查濕疹不同分期和不同中醫(yī)證型合并西北燥證的情況,探索濕疹與西北燥證之間的相關(guān)性,為濕疹的辨證論治提供臨床指導(dǎo)意義。方法:收集新疆醫(yī)科大學(xué)附屬中醫(yī)醫(yī)院皮膚科門診2015年1月至2015年12月的濕疹典型病例,通過調(diào)查量表調(diào)查濕疹患者的不同分期、不同中醫(yī)證型,以及其合并西北燥證的情況。結(jié)果:(1)共調(diào)查符合納入標準的濕疹患者160例,其中,急性濕疹25例,所占百分比為15.60%,亞急性濕疹83例,所占百分比為51.90%,慢性濕疹52例,所占百分比為32.50%。其中,濕熱蘊膚型52例,所占比例為32.50%,脾虛濕蘊型72例,所占比例45.00%,血虛風燥型29例,所占比例18.10%,其他證型7例,所占比例為4.40%。(2)160例濕疹中,其中83例合并西北燥證,所占百分比為51.88%。有6例急性濕疹合并西北燥證;有48例亞急性濕疹合并西北燥證;有29例慢性濕疹合并西北燥證。濕疹合并西北燥證在其不同分期之間進行比較,P=0.010.05,具有統(tǒng)計學(xué)意義。(3)濕疹的中醫(yī)證型中,有20例濕熱蘊膚型合并西北燥證;有37例脾虛濕蘊型合并西北燥證;有22例血虛風燥型合并西北燥證;有4例其他證型合并西北燥證。濕疹合并西北燥證在其不同中醫(yī)證型之間進行比較,P=0.0120.05,具有統(tǒng)計學(xué)意義。(4)160例濕疹患者中,其中先發(fā)西北燥證的有63例,后發(fā)西北燥證的有19例,同發(fā)西北燥證的有1例,不合并西北燥證的有77例。急性濕疹、亞急性濕疹、慢性濕疹三組之間西北燥證發(fā)生的先后情況進行比較,P=0.0280.05,差異有統(tǒng)計學(xué)意義,表明濕疹不同分期之間,西北燥證發(fā)生先后的情況有差異。結(jié)論:本次調(diào)查的160例濕疹患者中,有83例合并西北燥證,所占百分比為51.88%;在所收集的濕疹患者中亞急性濕疹合并西北燥證最多,48例,占百分比57.83%;所收集的濕疹患者中血虛風燥型合并西北燥證最多,22例,占百分比75.86%。所收集的濕疹患者中先發(fā)西北燥證的有63例,占百分比39.37%。說明濕疹與西北燥證有相關(guān)性,西北燥證可能是濕疹的誘發(fā)因素,也有可能是濕疹的病機基礎(chǔ)。
[Abstract]:Objective: to explore the correlation between eczema and northwest dryness syndrome by investigating different stages and different TCM syndromes of eczema combined with northwest dryness syndrome, and to provide clinical guidance for syndrome differentiation and treatment of eczema. Methods: the typical eczema cases were collected from January 2015 to December 2015 in dermatology department of affiliated traditional Chinese Medicine Hospital of Xinjiang Medical University. Results among 160 eczema patients, 25 were acute eczema, 83 were subacute eczema, 52 were chronic eczema and 52 were chronic eczema, among which 25 were acute eczema with a percentage of 15.60, 83 subacute eczema, and 52 chronic eczema. Among them, 52 cases of damp-heat accumulation skin type, 72 cases of spleen deficiency and dampness accumulation type, 45.00% of them, 29 cases of blood deficiency wind dryness type, 18.10%, 7 cases of other syndrome type, the proportion is 4.40% of 160 cases of eczema, of which 83 cases are complicated with northwest dryness syndrome. The percentage was 51.88. There were 6 cases of acute eczema with northwest dryness syndrome, 48 cases of subacute eczema with northwest dryness syndrome, 29 cases of chronic eczema with northwest dryness syndrome. Comparing eczema with Northwest dryness Syndrome in different stages, there are 20 cases of eczema syndrome type combined with Northwest dryness syndrome, 37 cases of spleen deficiency and dampness accumulation type combined with Northwest dryness Syndrome, among the TCM syndromes of eczema with statistical significance. There were 22 cases of blood deficiency and wind dryness combined with northwest dryness syndrome, 4 cases of other syndromes combined with northwest dryness syndrome. Eczema combined with northwest dryness syndrome was compared among different TCM syndromes. Among 160 cases of eczema with statistical significance, 63 cases had northwest dryness syndrome, 19 cases had northwest dryness syndrome, and 1 case had northwest dryness syndrome. There were 77 cases without northwest dryness syndrome. Acute eczema, subacute eczema and chronic eczema were compared among the three groups. There were significant differences in the occurrence of northwest dryness syndrome between the three groups. The difference was statistically significant, which indicated that there were differences in the occurrence of northwest dryness syndrome among different stages of eczema. Conclusion: of the 160 eczema patients surveyed, 83 cases were associated with northwest dryness syndrome, the percentage was 51.88, and the most 48 cases were subacute eczema combined with northwest dryness syndrome in the collected eczema patients. Among the patients with eczema, 22 cases (75.86%) had blood deficiency and wind dryness syndrome combined with northwest dryness syndrome. Among the patients with eczema, 63 cases (39.37%) developed northwest dryness syndrome. It is suggested that eczema is related to northwest dryness syndrome, which may be the inducing factor of eczema, and may also be the pathogenesis of eczema.
