【摘要】：目的:本研究通過觀察不同時(shí)間間隔電針治療膝骨性關(guān)節(jié)炎的臨床療效,探索電針治療膝骨性關(guān)節(jié)炎引起的慢性膝關(guān)節(jié)痛的最佳有效頻次,為臨床電針治療慢性膝關(guān)節(jié)痛選取最佳時(shí)間間隔提供參考。方法:本研究選取2016.3-2016.9期間,于黑龍江中醫(yī)藥大學(xué)附屬一、二院針灸科門診就診患者,收集符合納入標(biāo)準(zhǔn)的90例膝骨關(guān)節(jié)炎患者進(jìn)行臨床觀察研究。觀察對象隨機(jī)分為3組:等待治療組、隔六日治療1次組、隔日治療組,每組各30例。治療方法:選取內(nèi)膝眼、外膝眼、梁丘、血海四穴,針刺后施平補(bǔ)平瀉法,使上述穴位得氣,取兩組導(dǎo)線分別交叉連接(內(nèi)膝眼—梁丘、外膝眼—血海)四穴,兩正極在膝關(guān)節(jié)上,負(fù)極在下,接通電療儀,調(diào)節(jié)至2/100Hz等幅疏密波,電流強(qiáng)度調(diào)試在患者感覺最大耐受舒適為度,留針時(shí)間為30min。治療時(shí)間為4周。治療結(jié)束后,對總有效率及VAS評分和WOMAC膝關(guān)節(jié)功能量表進(jìn)行測評。結(jié)果:1.疼痛評分比較[VAS、WOMAC(pain)]:組內(nèi)比較:等待治療組治療前后VAS、WOMAC(pain)評分比較,差異無統(tǒng)計(jì)學(xué)意義(p0.05),隔六日治療組、隔日治療組治療組治療前后VAS、WOMAC(pain)評分比較,差異均有顯著統(tǒng)計(jì)學(xué)意義(p0.01)。組間比較:治療后,隔六日治療組、隔日治療組與等待治療組VAS、WOMAC(pain)評分組間比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P0.01),隔日治療組與隔六日治療組VAS評分組間比較,差異無統(tǒng)計(jì)學(xué)意義(p0.05),隔日治療組與隔六日治療組WOMAC(pain)評分組間比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。說明電針治療可以改善膝關(guān)節(jié)疼痛癥狀,隔日組療效優(yōu)于隔六日組。2.功能評分比較[WOMAC總分、WOMAC(function)]:組內(nèi)比較:等待治療組治療前后比較,差異無統(tǒng)計(jì)學(xué)意義(p0.05),隔六日治療組、隔日治療組治療組治療前后比較,差異均有顯著統(tǒng)計(jì)學(xué)意義(P0.01)。組間比較:治療后,隔六日治療組、隔日治療組與等待治療組組間比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P0.01),隔日治療組與隔六日治療組組間比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P0.01)。說明電針治療可以改善膝關(guān)節(jié)功能受限的癥狀,隔日組療效優(yōu)于隔六日組。3.僵硬評分比較[WOMAC(stiffness)]:組內(nèi)比較:等待治療組治療前后比較,差異無統(tǒng)計(jì)學(xué)意義(p0.05),隔六日治療組、隔日治療組治療組治療前后比較,差異均有顯著統(tǒng)計(jì)學(xué)意義(p0.01)。組間比較:治療后,隔六日治療組與等待治療組組間比較,差異無統(tǒng)計(jì)學(xué)意義(p0.05),隔日治療組與等待治療組組間比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05),隔日治療組與隔六日治療組組間比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。說明電針治療可以改善膝關(guān)節(jié)僵硬癥狀,隔日組療效優(yōu)于隔六日組。4.臨床療效比較:治療后,等待治療組總有效率為0,隔六日治療組總有效率為14.28%,隔日治療組總與效率為86.21%,隔日治療組與隔六日治療組比較,差異有顯著統(tǒng)計(jì)學(xué)意義(p0.01),說明隔日治療組具有更好的療效。結(jié)論:1.隔六日治療組與隔日治療組均具有臨床療效,電針治療可以改善膝骨性關(guān)節(jié)炎患者的疼痛、功能受限及僵硬方面的不適癥狀。等待治療組未給予電針干預(yù),無改善膝關(guān)節(jié)癥狀的作用。2.隔日治療組療效優(yōu)于隔六日治療組,隔日電針治療對減輕膝關(guān)節(jié)疼痛,改善關(guān)節(jié)功能受限及僵硬癥狀方面療效更明顯。
[Abstract]:Objective: To investigate the clinical efficacy of electroacupuncture at different time intervals for the treatment of knee osteoarthritis, and to explore the best effective frequency of electroacupuncture in the treatment of chronic knee pain caused by osteoarthritis of the knee, and to provide a reference for selecting the best time interval for the clinical electroacupuncture treatment of chronic knee pain. The clinical observation and study of 90 patients with osteoarthritis of the knee were collected in the Department of acupuncture and moxibustion Department of the second hospital of Heilongjiang University Of Chinese Medicine. The observation subjects were randomly divided into 3 groups: waiting for treatment group, 1 groups after six days of treatment and 30 cases in each group in each group. The treatment method was to select the internal knee, external knee eye, beam hillock, blood sea. The difference of VAS and WOMAC (pain) scores before and after treatment was statistically significant (P0.01). After treatment, the difference was statistically significant (P0.01) between the six day treatment group and the VAS and WOMAC (pain) score group in the daily treatment group and the waiting treatment group (P0.01), and the difference was not statistically significant between the treatment group and the six day treatment group. The difference was statistically significant (P0.05) between the treatment group and the waiting group group, and the comparison between the treatment group and the six day treatment group was compared. The difference was statistically significant (P0.05). It indicated that electroacupuncture could improve the stiffness of knee joint. The curative effect of.4. was better than that in the six day group. After treatment, the total effective rate of the treatment group was 0, the total effective rate of the six day treatment group was 14.28%, the total and efficiency of the treatment group in the other day were 86.21%, and the treatment group in the other day was compared with the six day treatment group. The difference has significant statistical significance (P0.01), indicating that the treatment group has better curative effect in the other day. Conclusion: 1. septum six day treatment group and the other day treatment group have clinical curative effect, electroacupuncture treatment can improve the pain, function limitation and stiffness of the patients with knee osteoarthritis. The effect of knee joint symptoms in.2. group was better than that in the six day treatment group. Electroacupuncture treatment was more effective in reducing knee pain and improving joint function limitation and stiff symptoms every other day.
