【摘要】：目的:通過(guò)觀察細(xì)蝎膠囊對(duì)偏頭痛(瘀血阻絡(luò)證)患者的臨床療效、中醫(yī)證候療效、頭痛影響測(cè)定評(píng)分以及偏頭痛發(fā)作次數(shù)、天數(shù)、疼痛程度、持續(xù)時(shí)間及不良反應(yīng)等指標(biāo)的影響統(tǒng)計(jì),以期能對(duì)細(xì)蝎膠囊在治療偏頭痛(瘀血阻絡(luò)型)有效性以及安全性得出一個(gè)客觀的結(jié)論,從而對(duì)其臨床應(yīng)用提出科學(xué)、合理依據(jù)。方法:本試驗(yàn)共選取了42例來(lái)自于2014年11月至2015年06月襄陽(yáng)市中醫(yī)院腦病科門(mén)診就診偏頭痛(瘀血阻絡(luò)型)患者,運(yùn)用隨機(jī)分組方法,組成各由21例患者組成的治療組及對(duì)照組。治療組給予單藥口服細(xì)蝎囊(藥物組成:細(xì)辛10g、全蝎10g、防風(fēng)10g、刺蒺藜10g、川芎30g、白芷10g、桃仁10g、紅花10g、白僵蠶10g。由本院藥劑科制備),6粒/次,3次/日;對(duì)照組采用鹽酸氟桂利嗪膠囊(由西安楊森制藥公司生產(chǎn)),5mg/次,每晚睡前服用,所有患者的病程均在6個(gè)月以上,平均(2.2±1.4)年。所有患者均排除頸椎病、顱內(nèi)占位性病變、腦卒中等疾病導(dǎo)致的頭痛可能,同時(shí)還需要排除妊娠期女性以及有對(duì)治療藥物過(guò)敏史的患者。2組患者在基線期之前4周統(tǒng)計(jì)的關(guān)于性別、年齡、病程等一般資料上不存在顯著差異(P0.05),兩組比較具有統(tǒng)計(jì)學(xué)意義。在14天的療程之后繼續(xù)隨訪,每四周當(dāng)面隨訪一次,每周電話隨訪,直至12周末結(jié)束隨訪,對(duì)其臨床療效以及安全性等數(shù)據(jù)進(jìn)行綜合統(tǒng)計(jì),同時(shí)應(yīng)用SPSS17.0軟件對(duì)統(tǒng)計(jì)的相關(guān)數(shù)據(jù)進(jìn)行分析,以此來(lái)觀察兩組患者治療后在療效、中醫(yī)證候以及安全性上有無(wú)明顯差異。結(jié)果:通過(guò)對(duì)細(xì)蝎膠囊療效、中醫(yī)證候及安全性等方面相關(guān)數(shù)據(jù)統(tǒng)計(jì)學(xué)分析,最后的結(jié)果表明在比較兩組藥物的臨床療效這一方面,治療組5人臨床痊愈,7人顯效,6人有效,3人無(wú)效,總有效率為85.72%,對(duì)照組6人臨床痊愈,6人顯效,5人有效,4人無(wú)效,總有效率為80.96%,兩組療效無(wú)顯著差異(P0.05),說(shuō)明細(xì)蝎膠囊與鹽酸氟桂利嗪膠囊在針對(duì)偏頭痛的治療方面上均有確切的療效。但另一方面,在改善中醫(yī)證候療效上,治療組與對(duì)照組兩組經(jīng)統(tǒng)計(jì)學(xué)分析有顯著性差異(P0.05),這些結(jié)果充分表現(xiàn)出單用細(xì)蝎膠囊組對(duì)比于單用鹽酸氟桂利嗪膠囊組在改善中醫(yī)癥候方面有獨(dú)特的優(yōu)勢(shì)。而在安全性方面,治療組相較于對(duì)照組沒(méi)有相關(guān)不良事件報(bào)告提出,安全性良好。結(jié)論:細(xì)蝎膠囊在治療瘀血阻絡(luò)型偏頭痛的治療上面有效,同時(shí)在針對(duì)中醫(yī)癥候的改善上更是具有一些自身的優(yōu)勢(shì),安全性良好。
[Abstract]:Objective: to observe the clinical effect of Scorpio capsule on patients with migraine (blood stasis blocking collateral syndrome), the curative effect of TCM syndromes, the score of headache influence, the number of migraine attacks, the days of migraine attack, and the degree of pain. In order to obtain an objective conclusion on the efficacy and safety of Scorpion capsule in the treatment of migraine (blood stasis and collateral obstruction type), the effect of duration and adverse reactions was analyzed, and the scientific and reasonable basis for its clinical application was put forward. Methods: from November 2014 to June 2015, 42 patients with migraine (blood stasis blocking collateral type) were selected from the Department of Encephalopathy, Xiangyang traditional Chinese Medicine Hospital, and were randomly divided into two groups. Each group consisted of 21 patients and the control group. The treatment group was given single drug oral Scorpion sac (drug composition: Asarum 10g, Scorpion 10g, Fangfeng 10g, Tribulus terrestris 10g, Chuanxiong 30g, dahurica 10g, peach kernel 10g, safflower 10g, white standosia 10g. The patients in the control group were treated with flunarizine hydrochloride capsule (5 mg / time produced by Xi'an Yang Sen Pharmaceutical Company) before bedtime every night. The course of disease of all patients was more than 6 months (average 2.2 鹵1.4) years. All patients were excluded from headache caused by cervical spondylopathy, intracranial space-occupying lesions, stroke, etc. At the same time, there was no significant difference in general data of sex, age, course of disease and so on between group 2 and group .2 of pregnancy women and patients with history of drug allergy before the baseline period (P 0.05). The comparison between the two groups was statistically significant. They were followed up after 14 days of treatment, followed up in person every four weeks, followed by telephone every week until the end of 12 weeks. The clinical efficacy and safety data were analyzed. At the same time, the statistical data were analyzed by SPSS17.0 software to observe whether there were significant differences in curative effect, TCM syndromes and safety between the two groups after treatment. Results: according to the statistical analysis of Scorpion capsule's curative effect, TCM syndrome and safety, the final result shows that the clinical curative effect of the two groups is compared. The total effective rate was 85.72in the treatment group, while in the control group, 6 patients were cured and 6 patients were effective and 4 patients were ineffective, and the total effective rate of the treatment group was 85.722.In the control group, 6 patients were clinically cured and 6 patients were significantly effective. The total effective rate was 80.96, and there was no significant difference between the two groups (P 0.05), which indicated that Scorpion capsule and flunarizine Hydrochloride capsule had definite curative effect on migraine. On the other hand, on the other hand, in improving the curative effect of TCM syndromes, There was significant difference between the treatment group and the control group by statistical analysis. These results showed that the single scorpion capsule group had a unique advantage in improving TCM symptoms compared with the single flunarizine hydrochloride capsule group. In terms of safety, the safety of the treatment group was better than that of the control group. Conclusion: Scorpio capsule is effective in the treatment of migraine caused by blood stasis and obstruction of collaterals, and has some advantages and good safety in the improvement of TCM symptoms.
