【摘要】：目的:觀察益氣滋陰通絡(luò)方治療氣陰兩虛、脈絡(luò)瘀阻型2型糖尿病周圍神經(jīng)病變的臨床療效。方法:采用隨機(jī)、平行對(duì)照的臨床試驗(yàn)研究方法,將符合研究要求的2型糖尿病神經(jīng)病變的患者60例采用數(shù)字表法隨機(jī)分為對(duì)照組(30例)與治療組(30例),兩組患者糖尿病基礎(chǔ)治療不變,對(duì)照組給予彌可保常規(guī)治療,治療組在彌可保常規(guī)治療基礎(chǔ)上給予益氣滋陰通絡(luò)方一日一劑治療。治療4周后,觀察血脂,血糖,血流變,密歇根篩查量表評(píng)分,周圍神經(jīng)傳導(dǎo)速度,血清8-異前列腺素F2α(8-iso-PGF-2α)及治療前后中醫(yī)臨床癥候療效的變化,評(píng)價(jià)益氣滋陰通絡(luò)方的臨床療效。結(jié)果:1.中醫(yī)臨床癥候療效:治療組總有效率93.3%,對(duì)照組總有效率60.0%,組間比較具有顯著性差異(P0.05);2.密歇根篩查量表評(píng)分:兩組患者治療前后密歇根篩查量表評(píng)分均有明顯下降(P0.05),兩組治療前后評(píng)分差值比較具有顯著的差異(P0.05),提示治療組對(duì)密歇根篩查量表評(píng)分的改善優(yōu)于對(duì)照組;3.血清8-iso-PGF-2α:兩組患者經(jīng)藥物治療后均可顯著降低血清8-iso-PGF-2α水平(P0.05),但治療前后8-iso-PGF-2α差值治療組較對(duì)照組具有顯著的差異(P0.05),提示治療組對(duì)8-iso-PGF-2α的改善優(yōu)于對(duì)照組;4.周圍神經(jīng)傳導(dǎo)速度:治療前后相比治療組周圍神經(jīng)傳導(dǎo)速度顯著降低(P0.05),對(duì)照組無顯著變化(P0.05);5.治療組在改善血脂、血糖、血流變方面顯著優(yōu)于對(duì)照組(P0.05)。結(jié)論:益氣滋陰通絡(luò)方對(duì)氣陰兩虛、脈絡(luò)瘀阻型糖尿病周圍神經(jīng)病變患者血脂、血糖、血流變、密歇根篩查量表評(píng)分、8-iso-PGF-2α、周圍神經(jīng)傳導(dǎo)速度有改善作用,并可明顯改善DPN患者中醫(yī)臨床癥狀,提高生活質(zhì)量,驗(yàn)證了采用益氣滋陰通絡(luò)方治療氣陰兩虛、脈絡(luò)瘀阻型DPN的可行性,為中醫(yī)藥防治DPN提供了一種的新的思路與方法。
[Abstract]:Objective: to observe the clinical effect of Yiqi Ziyin Tongluo recipe on peripheral neuropathy of type 2 diabetes mellitus with deficiency of qi and yin and choroid stasis. Methods: a randomized, parallel controlled clinical trial was conducted. 60 patients with type 2 diabetic neuropathy were randomly divided into control group (n = 30) and treatment group (n = 30). The treatment group was treated with Yiqi Ziyin Tongluo prescription on the basis of routine treatment. After 4 weeks of treatment, the changes of blood lipids, blood glucose, hemorheology, Michigan screening scale, peripheral nerve conduction velocity, serum 8-iso-PGF-2 偽) and the clinical symptoms before and after treatment were observed. To evaluate the clinical effect of Yiqi Ziyin Tongluo recipe. The result is 1: 1. The curative effect of TCM clinical symptoms: the total effective rate of the treatment group was 93.3m, the total effective rate of the control group was 60.0.The difference between the two groups was significant (P 0.05). Michigan screening scale score: the scores of Michigan screening scale were significantly decreased in both groups before and after treatment, and there was significant difference between the two groups before and after treatment (P 0.05), indicating that the Michigan screening scale score in the treatment group was higher than that in the control group. The improvement was better than that of the control group. Serum 8-iso-PGF-2 偽: after drug treatment, the serum 8-iso-PGF-2 偽 level was significantly decreased in both groups, but the difference of 8-iso-PGF-2 偽 between the treatment group and the control group was significant (P 0.05), indicating that the improvement of 8-iso-PGF-2 偽 in the treatment group was better than that in the control group (P < 0.05). Peripheral nerve conduction velocity: compared with the treatment group, the peripheral nerve conduction velocity of the treatment group was significantly lower than that of the control group, while the control group had no significant change. The treatment group was superior to the control group in improving blood lipids, blood glucose and hemorheology. Conclusion: Yiqi Ziyin Tongluo recipe can improve blood lipid, blood glucose, blood rheology, 8-iso-PGF-2 偽 and peripheral nerve conduction velocity in diabetic patients with deficiency of qi and yin and choroidal stasis. It can obviously improve the clinical symptoms of traditional Chinese medicine (TCM) and improve the quality of life in patients with DPN. It verifies the feasibility of using Yiqi Ziyin Tongluo recipe to treat DPN of deficiency of Qi and Yin and of choroid stasis, and provides a new way of thinking and method for the prevention and treatment of DPN by traditional Chinese medicine.
