【摘要】：目的:觀察毫火針結(jié)合康復(fù)訓(xùn)練治療腦卒中后上肢痙攣的治療效果,并證實(shí)使用毫火針治療腦卒中上肢痙攣療效的確切性。方法:選取自治區(qū)中醫(yī)醫(yī)院針灸科、神經(jīng)外科、康復(fù)門(mén)診以及病房符合納入標(biāo)準(zhǔn)的60例患者,隨機(jī)分成兩組。治療組采用毫火針結(jié)合康復(fù)訓(xùn)練,對(duì)照組采用毫針結(jié)合康復(fù)訓(xùn)練。治療7天為一療程,共治療2個(gè)療程。治療后采用FMA評(píng)分,改良Ashworth評(píng)分進(jìn)行療效評(píng)價(jià)。結(jié)果:(1)、總體療效:治療組治愈8例、顯效、有效以及無(wú)效分別為7例、13例、2例,有效率93.33%;對(duì)照組治愈5例、顯效、有效以及無(wú)效分別為8例、12例、5例,有效率83.33%。兩組比較有差異(P0.05)。(2)、兩組治療前后FMA評(píng)分比較,兩組均有效果,但治療組較對(duì)照組效果更加明顯。(3)、兩組治療前后改良Ashworth評(píng)分比較,兩組均有明顯效果,但治療組優(yōu)于對(duì)照組。(4)、在治療過(guò)程中兩組患者均未出現(xiàn)不良反應(yīng)。結(jié)論:毫火針結(jié)合康復(fù)訓(xùn)練可以顯著改善腦卒中后上肢痙攣,并降低肌張力。毫火針結(jié)合康復(fù)訓(xùn)練和毫針結(jié)合康復(fù)訓(xùn)練都能緩解肌痙攣,降低肌張力,經(jīng)兩組對(duì)照,治療后兩組患者的上肢FMA評(píng)分和MAS評(píng)分相比有統(tǒng)計(jì)學(xué)意義(P0.05),說(shuō)明毫火針結(jié)合康復(fù)訓(xùn)練比毫針結(jié)合康復(fù)訓(xùn)練的療效更好。
[Abstract]:Objective: to observe the therapeutic effect of acupuncture combined with rehabilitation training on spasm of upper limb after stroke, and to confirm the accuracy of the treatment of spasm of upper limb after stroke. Methods: 60 patients in acupuncture department, neurosurgery department, rehabilitation clinic and ward were randomly divided into two groups. The treatment group was treated with acupuncture combined with rehabilitation training, while the control group was treated with acupuncture combined with rehabilitation training. Treatment for 7 days as a course of treatment, a total of 2 courses of treatment. After treatment, FMA score and modified Ashworth score were used to evaluate the curative effect. Results: in the treatment group, 8 cases were cured, 13 cases were effective and 2 cases were effective, and the effective rate was 93.33%. In the control group, 5 cases were cured, 5 cases were effective, 12 cases were effective, and 5 cases were ineffective, the effective rate was 83.33%. There was significant difference between the two groups in FMA scores before and after treatment, but the effect in the treatment group was more obvious than that in the control group. The modified Ashworth score before and after treatment in the two groups had obvious effect. But the treatment group was superior to the control group. Conclusion: acupuncture combined with rehabilitation training can significantly improve upper limb spasm and reduce muscle tension after stroke. The combination of neuter needle and rehabilitation training and filiform needle combined with rehabilitation training can relieve muscle spasm and reduce muscle tension. After treatment, the FMA score of upper limb of the two groups was significantly higher than that of the MAS score (P 0.05), which indicated that the curative effect of acupuncture combined with rehabilitation training was better than that of acupuncture combined with rehabilitation training.
