【摘要】：目的:觀察自擬婦安湯中藥熏洗治療肝郁脾虛證外陰營養(yǎng)不良的臨床療效。闡述本病的病因病機(jī)以及發(fā)病機(jī)理,為中醫(yī)外治法治療外陰營養(yǎng)不良提供科學(xué)的依據(jù)。方法:本研究在規(guī)定時(shí)間(2014年12月至2015年12月)選取72例外陰營養(yǎng)不良(肝郁脾虛證)的患者,皆就診于長(zhǎng)春中醫(yī)藥大學(xué)附屬醫(yī)院婦科門診。并隨機(jī)將此72例患者分為治療組36例和對(duì)照組36例進(jìn)行規(guī)范化臨床觀察。治療組用自擬婦安湯每日1劑加水2000ml左右,煎煮至水沸后20分鐘,用無菌紗布過濾后將藥湯倒入浴盆,趁熱熏洗陰部,待溫度適中后浸洗患處,每次15~30分鐘。對(duì)照組36例采用天津藥業(yè)集團(tuán)有限公司,國藥準(zhǔn)字H12020638,醋酸曲安奈德軟膏,外陰涂擦用藥。兩組均于月經(jīng)凈三天后用藥,早晚各一次,15天為1療程,連續(xù)用藥3個(gè)療程停藥觀察療效,并隨訪3個(gè)月。觀察治療組和對(duì)照組的患者的癥狀和體征的變化并運(yùn)用統(tǒng)計(jì)學(xué)方法判斷其臨床療效,對(duì)各項(xiàng)參數(shù)指標(biāo)的變化和療效的相關(guān)性進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:治療組:痊愈10例,痊愈率36.1%;顯效12例,顯效率33.3%;有效11例,有效率33.3%;無效1例,無效率2.8%,總有效率達(dá)97.2%。對(duì)照組:痊愈3例,痊愈率8.3%;顯效10例,顯效率27.8%;有效13例,有效率36.1%;無效10例,無效率27.8%,總有效率達(dá)72.2%,兩組總療程比較,差異有顯著意義(P0.05),說明治療組治療外陰營養(yǎng)不良療效明顯優(yōu)于對(duì)照組。結(jié)論:采用中藥熏洗治療外陰營養(yǎng)不良的方法綠色有效、安全可靠。實(shí)驗(yàn)證明,治療組治療外陰營養(yǎng)不良(肝郁脾虛證)的效果要明顯優(yōu)于對(duì)照組。
[Abstract]:Objective: to observe the clinical effect of fumigation and washing of Fuan decoction on vulvar dystrophy due to liver stagnation and spleen deficiency. The etiology, pathogenesis and pathogenesis of this disease are expounded, which provides a scientific basis for the treatment of vulvar dystrophy by external treatment of traditional Chinese medicine. Methods: from December 2014 to December 2015, 72 patients with yin malnutrition (liver stagnation and spleen deficiency syndrome) were enrolled in the gynecological clinic of Changchun University of traditional Chinese Medicine (Changchun University of traditional Chinese Medicine). The 72 patients were randomly divided into treatment group (36 cases) and control group (36 cases) for standardized clinical observation. In the treatment group, 1 dose of Fuan decoction was added with 2000ml or so daily, boiled until boiling for 20 minutes, filtered with sterile gauze, then poured into the bath tub, fumigated and washed the pubic part while hot, soaking the affected place at moderate temperature, 1530 minutes each time. 36 cases of control group were treated with Tianjin Pharmaceutical Group Co., Ltd., H12020638, triamcinolone acetonide acetate ointment, vulva smear. The two groups were treated after 3 days of menstruation, 15 days a day in the morning and evening for a course of treatment. The curative effect was observed after 3 consecutive courses of treatment and followed up for 3 months. The changes of symptoms and signs of the patients in the treatment group and the control group were observed and the clinical efficacy was evaluated by statistical method. The change of the parameters and the correlation of the curative effect were analyzed statistically. Results: in the treatment group, 10 cases were cured, the cure rate was 36.1%, 12 cases were markedly effective, the effective rate was 33.3%, 11 cases were effective, the effective rate was 33.3%; one case was ineffective, the ineffective rate was 2.8%, and the total effective rate was 97.2%. In the control group, 3 cases were cured, the cure rate was 8.3%; the remarkable effect was 10 cases, the effective rate was 27.881; the effective rate was 13 cases, the effective rate was 36.1%; the ineffective 10 cases, the inefficiency was 27.828%, the total effective rate was 72.2%; the total effective rate was 72.2%; the total effective rate was 72.2%, and the total effective rate was 72.2%. The difference was significant (P 0.05), which indicated that the curative effect of the treatment group was better than that of the control group. Conclusion: the traditional Chinese medicine fumigation is effective, safe and reliable in the treatment of vulvar dystrophy. Experimental results show that the treatment group is superior to the control group in the treatment of vulvar dystrophy (liver stagnation and spleen deficiency syndrome).
