【摘要】：目的:以疏肝和胃為切入點,觀察柴胡芩連湯治療前庭性偏頭痛的臨床療效,為中醫(yī)藥治療前庭性偏頭痛提供新的思路和方法。方法:選取80例前庭性偏頭痛患者,隨機分為治療組與對照組各40例,開展隨機對照臨床研究。治療組給予口服柴胡芩連湯,對照組給予口服甲磺酸倍他司汀片,療程均為4周。觀察兩組治療前后的臨床總療效、眩暈障礙量表(DHI)、健康狀況量表(SF-36)及眩暈發(fā)作次數(shù)、眩暈發(fā)作持續(xù)時間的變化。治療后隨訪3個月,觀察兩組眩暈復(fù)發(fā)情況。結(jié)果:治療4周后,最終完成臨床觀察患者共76例(治療組脫落1例,對照組脫落3例),對該76例患者臨床觀察數(shù)據(jù)進行統(tǒng)計分析。1.治療組的臨床總療效為82.05%,對照組的臨床總療效為72.68%,治療組優(yōu)于對照組,兩組比較差異有統(tǒng)計學(xué)意義(P㩳0.05);治療組及對照組的眩暈障礙量表(DHI)積分、健康狀況量表(SF-36)積分及眩暈發(fā)作次數(shù)、眩暈發(fā)作持續(xù)時間均有所改善,且兩組組內(nèi)比較差異具有統(tǒng)計學(xué)意義(P0.05),說明用藥后患者各項療效指標(biāo)均優(yōu)于用藥前;組間比較,治療組較對照組健康狀況量表積分、眩暈發(fā)作次數(shù)、眩暈發(fā)作持續(xù)時間改善明顯,治療組優(yōu)于對照組,且差異有統(tǒng)計學(xué)意義(P0.05)。眩暈障礙量表積分較對照組改善無明顯變化,差異無統(tǒng)計學(xué)意義(P0.05)。2.隨訪3個月后,兩組患者的眩暈發(fā)作次數(shù)、眩暈發(fā)作持續(xù)時間組間比較差異有統(tǒng)計學(xué)意義(P0.05),治療組的眩暈復(fù)發(fā)狀況明顯優(yōu)于對照組。結(jié)論:柴胡芩連湯可以有效緩解前庭性偏頭痛患者的中醫(yī)癥狀,減輕眩暈發(fā)作程度,提高患者生活質(zhì)量,且能夠減少眩暈發(fā)作次數(shù)、縮短眩暈發(fā)作持續(xù)時間,有效控制眩暈癥狀的復(fù)發(fā),遠期療效較好,值得臨床推廣。
[Abstract]:Objective: to observe the clinical effect of Chaihu Qinlian decoction in treating vestibular migraine and provide new ideas and methods for treating vestibular migraine with traditional Chinese medicine.Methods: 80 patients with vestibular migraine were randomly divided into treatment group (n = 40) and control group (n = 40).The treatment group was treated with Chaihu Qinlian decoction and the control group with betastatin mesylate for 4 weeks.The changes of total clinical efficacy, dizziness scale, SF-36), the number of vertigo attacks and the duration of vertigo attack were observed before and after treatment in the two groups.The patients were followed up for 3 months to observe the recurrence of vertigo in the two groups.Results: after 4 weeks of treatment, 76 patients (1 case in the treatment group and 3 cases in the control group) finished the clinical observation. The clinical observation data of the 76 cases were statistically analyzed.The total clinical efficacy of the treatment group was 82.05 and that of the control group was 72.68. The treatment group was superior to the control group, and the difference between the two groups was statistically significant.The scores of SF-36), the frequency of vertigo attack and the duration of vertigo attack were improved, and the difference between the two groups was statistically significant (P 0.05).Compared with the control group, the scores of health status scale, the frequency of vertigo attack and the duration of vertigo attack in the treatment group were significantly improved, and the treatment group was better than the control group, and the difference was statistically significant (P 0.05).Compared with the control group, the score of vertigo disorder scale had no significant change, and the difference was not statistically significant (P 0.05).After 3 months follow-up, there were significant differences in the number of vertigo attacks and the duration of vertigo attacks between the two groups. The relapse status of vertigo in the treatment group was significantly better than that in the control group (P 0.05).Conclusion: Chaihu Qinlian decoction can effectively relieve the symptoms of vestibular migraine, reduce the degree of vertigo attack, improve the quality of life of the patients, reduce the frequency of vertigo attack and shorten the duration of vertigo attack.Effective control of the recurrence of vertigo symptoms, long-term results are better, worthy of clinical promotion.
