【摘要】：目的:觀察健脾益腎湯治療IgA腎病的臨床療效,為中醫(yī)藥治療IgA腎病提供臨床依據(jù)。方法:將符合納入標(biāo)準(zhǔn)的46例IgA腎病患者,給予健脾益腎湯進(jìn)行治療,療程為3個(gè)月,觀察患者治療前后的中醫(yī)癥狀積分、尿蛋白、尿沉渣鏡檢紅細(xì)胞計(jì)數(shù)、24小時(shí)尿蛋白定量、血漿白蛋白、血肌酐、血尿酸、血清胱抑素C等實(shí)驗(yàn)室指標(biāo)以及IgA腎病預(yù)后系統(tǒng)評(píng)分積分的變化,應(yīng)用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:1、本臨床觀察共46例IgA腎病患者,治療3個(gè)月后綜合療效總有效率為84.78%,中醫(yī)癥狀療效總有效率為95.65%,中醫(yī)癥狀積分較治療前減小明顯(P0.05)。2、治療后尿蛋白、尿沉渣鏡檢紅細(xì)胞計(jì)數(shù)、24小時(shí)尿蛋白定量、血清胱抑素C、收縮壓較治療前均明顯降低(P0.05),血漿白蛋白升高(P0.05)。治療后血肌酐、血尿酸較治療前變化無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3、治療后IgA腎病預(yù)后系統(tǒng)評(píng)分積分降低顯著(P0.05)。結(jié)論:1、健脾益腎湯可改善IgA腎病患者的臨床癥狀。2、健脾益腎湯可降低IgA腎病患者血尿、蛋白尿水平,提高血清白蛋白水平。3、健脾益腎湯可降低IgA腎病患者血清胱抑素C水平,對(duì)IgA腎病早期腎功能下降有改善作用。4、健脾益腎湯可減小IgA腎病患者預(yù)后評(píng)分,降低本病進(jìn)展為ESRD的風(fēng)險(xiǎn)。
[Abstract]:Objective: to observe the clinical effect of Jianpi Yishen decoction on IgA nephropathy and to provide clinical basis for TCM treatment of IgA nephropathy.Methods: 46 patients with IgA nephropathy were treated with Jianpi Yishen decoction for 3 months. The scores of TCM symptoms, urine protein and erythrocyte count were observed before and after treatment.The changes of plasma albumin, serum creatinine, serum uric acid, serum cystatin C and prognosis system score of IgA nephropathy were analyzed by SPSS17.0 software.Results: in this clinical study, 46 patients with IgA nephropathy were observed. After 3 months of treatment, the overall effective rate was 84.78, the total effective rate of TCM symptom was 95.655.The score of TCM symptom decreased significantly compared with that of before treatment.Urine sediment microscopic examination of erythrocyte count and 24 hours urine protein quantity, serum cystatin C, systolic blood pressure were significantly lower than before treatment, plasma albumin increased P0.05N.There was no significant change in serum creatinine and uric acid after treatment compared with that before treatment (P 0.05). After treatment, the score of prognosis system score of IgA nephropathy decreased significantly (P 0.05).Conclusion: 1, Jianpi Yishen decoction can improve the clinical symptoms of IgA nephropathy patients. Jianpi Yishen decoction can reduce the level of hematuria and proteinuria in IgA nephropathy patients, and increase the serum albumin level. The Jianpi Yishen decoction can reduce the level of serum cystatin C in IgA nephropathy patients.It can improve the decline of renal function in early stage of IgA nephropathy. 4. Jianpi Yishen decoction can reduce the prognosis score of patients with IgA nephropathy and reduce the risk of progression to ESRD.
