【摘要】：目的:本研究旨在觀察PNF技術配合陰陽平衡針法治療腦卒中后上肢痙攣狀態(tài)的臨床療效,對比常規(guī)康復療法配合陰陽平衡針法治療腦卒中后上肢痙攣性癱瘓患者的痙攣程度、上肢運動功能和日常生活能力等方面的變化,對結果進行綜合分析,來探討有效的治療方法,為其應用于臨床提供實踐依據。方法:采用隨機對照的試驗方法,將60例符合納入標準的腦卒中后上肢痙攣狀態(tài)患者隨機分為治療組和對照組,每組各30例。兩組均接受常規(guī)內科藥物治療的基礎上,治療組接受PNF技術結合陰陽平衡針法治療,對照組接受常規(guī)康復結合陰陽平衡針法治療,各連續(xù)治療6天,休息1天,共治療24天后,以痙攣程度評定表、日常生活能力指數積分表、簡試Fugl-Meyer上肢運動功能評分法為評定標準進行療效觀察,然后采用統計學SPSS19.0軟件進行結果分析。結果:治療組30例患者中顯效10例,有效17例,無效3例,總有效率為90%。對照組30例患者顯效5例,有效16例,無效9例,總有效率為70%。經秩和檢驗表明,其差異有統計學意義(P0.05),說明PNF配合陰陽平衡針法治療腦卒中后上肢痙攣總療效優(yōu)于常規(guī)康復配合陰陽平衡針法,且治療后兩組患者的上肢痙攣程度、上肢運動功能、日常生活能力等均有不同程度的改善,治療組均優(yōu)于對照組,差異有統計學意義(P0.05)。結論:與常規(guī)康復配合陰陽平衡針法相對比,PNF技術配合陰陽平衡針法能夠明顯改善腦卒中后上肢痙攣性癱瘓患者的上肢痙攣程度、降低上肢肌張力,擴大上肢關節(jié)活動度,該方法經臨床證實安全有效,具有科學性、可行性。
[Abstract]:Objective: to observe the clinical effect of PNF combined with Yin-Yang balanced acupuncture in the treatment of spasticity of upper limb after stroke, and to compare the degree of spasm in patients with spastic paralysis of upper limb after stroke treated by routine rehabilitation therapy and Yin-yang balanced acupuncture. The changes of upper limb motor function and daily life ability were analyzed synthetically to discuss the effective treatment method and to provide practical basis for its clinical application. Methods: a randomized controlled trial method was used. Sixty patients with spasticity of upper limb after stroke were randomly divided into treatment group and control group with 30 cases in each group. On the basis of routine medical treatment, the treatment group was treated with PNF technique combined with yin and yang balance acupuncture. The control group was treated with routine rehabilitation combined with Yin-Yang balanced acupuncture therapy for 6 consecutive days, rest for 1 day. After 24 days of treatment, the patients were assessed with spasticity scale and the activity of daily living index (ADL) score table. The results were analyzed by statistical SPSS19.0 software. Results: in the treatment group, 10 cases were effective, 17 cases were effective, 3 cases were ineffective. The total effective rate was 90. In the control group, there were 5 cases of remarkable effect, 16 cases of effective and 9 cases of ineffectiveness. The total effective rate was 70%. The difference was statistically significant (P 0.05), which indicated that the total curative effect of PNF combined with Yin-Yang balanced acupuncture in treating spasm of upper limb after stroke was better than that of routine rehabilitation combined with Yin-Yang balanced acupuncture, and the degree of spasticity and motor function of upper limb in the two groups after treatment. The ability of daily living was improved to some extent, and the treatment group was superior to the control group. Conclusion: compared with conventional rehabilitation combined with Yin-Yang balanced acupuncture, PNF combined with Yin-Yang balanced acupuncture can obviously improve the degree of upper limb spasm and reduce the tension of upper limb muscle in patients with spastic paralysis of upper limb after stroke. The method is proved to be safe, effective, scientific and feasible.
