【摘要】：研究目的通過臨床研究,系統(tǒng)評價根據(jù)“清火散瘀、培土祛濕”法擬定的“止癢潤膚湯”治療慢性濕瘡(脾蘊瘀熱證)的臨床療效、安全性、復發(fā)率,為臨床治療慢性濕瘡提供一種新的治療思路。研究方法本研究選取成都中醫(yī)藥大學附屬醫(yī)院皮膚科門診中符合納入標準的60例患者,將其隨機分為治療組和對照組。治療組30例,予中藥止癢潤膚湯口服；對照組30例,予地氯雷他定分散片口服,兩組均予冰黃膚樂軟膏外擦。每周復診1次,連續(xù)治療4周。用SPSS21.0軟件對數(shù)據(jù)進行統(tǒng)計分析,對比患者治療前后癥狀、體征改善情況及復發(fā)率等,做出相關臨床判定。研究結果本研究共完成臨床病例58例,治療組29例,治愈5例,顯效9例,有效9例,無效6例,有效率為79.31%；對照組29例,治愈2例,顯效5例,有效11例,無效11例,有效率為62.07%。經(jīng)統(tǒng)計學分析,兩組有效率差異具有統(tǒng)計學意義(P0.05),說明治療組臨床有效率優(yōu)于對照組；在改善瘙癢程度方面,兩組差異不具有統(tǒng)計學意義(P0.05),說明治療組與對照組療效相當；在改善皮疹情況及全身伴隨癥狀方面,兩組差異具有統(tǒng)計學意義(P0.05),說明治療組療效優(yōu)于對照組；對兩組痊愈及顯效的患者進行復發(fā)情況的比較,差異具有統(tǒng)計學意義(P0.05),說明治療組在防止慢性濕瘡復發(fā)方面明顯優(yōu)于對照組。結論由“清火散瘀、培土祛濕”法擬定的“止癢潤膚湯”能有效改善慢性濕瘡患者的局部及全身癥狀,療效優(yōu)于對照組,復發(fā)率低,無明顯不良反應,值得廣泛應用于臨床。
[Abstract]:Objective to systematically evaluate the clinical efficacy, safety and recurrence rate of "Zhitao Runfu decoction" in the treatment of chronic wet sore (spleen stasis heat syndrome) according to the method of "clearing fire and dissipating stasis and nourishing soil and dispelling dampness". In this study, 60 patients who met the inclusion criteria in dermatology outpatient department of Chengdu University of traditional Chinese Medicine were selected. It was randomly divided into treatment group and control group. 30 cases in treatment group were treated with Zhiyao Runfu decoction, 30 cases in control group were treated with desloratadine dispersible tablets, both groups were treated with binghuangfule ointment, and treated once a week, and 30 cases in control group were treated with desloratadine dispersible tablets. After 4 weeks of continuous treatment, the data were statistically analyzed with SPSS21.0 software, and the symptoms, signs and recurrence rate of the patients were compared before and after treatment. The results showed that there were 58 cases in this study and 29 cases in the treatment group. 5 cases were cured, 9 cases were effective, 9 cases were effective, 6 cases were ineffective, the effective rate was 79.31. In the control group, 29 cases were cured, 2 cases were cured, 5 cases were effective, 11 cases were effective, 11 cases were ineffective, and the effective rate was 62.07%. The difference of effective rate between the two groups was statistically significant (P 0.05), which indicated that the clinical effective rate of the treatment group was better than that of the control group, but the difference between the two groups was not statistically significant in improving the degree of pruritus, which indicated that the curative effect of the treatment group was similar to that of the control group. The difference between the two groups was statistically significant (P 0.05), indicating that the curative effect of the treatment group was better than that of the control group. The difference was statistically significant (P 0.05), which indicated that the treatment group was superior to the control group in preventing the recurrence of chronic wet sore. The "Zhitao Runfu decoction" developed by the method of "Peitu Qushi" can effectively improve the local and systemic symptoms of patients with chronic wet sores. The curative effect is superior to that of the control group, the recurrence rate is low and there is no obvious adverse reaction, so it is worthy of being widely used in clinical practice.
