【摘要】：[目的]運(yùn)用“象思維”探討弱精子癥的病因病機(jī),為弱精子癥辨證治療提供新的角度和思路。歸納總結(jié)導(dǎo)師治療弱精子癥經(jīng)驗(yàn),提出治療弱精子癥遣方用藥之法,以更好的為臨床服務(wù)。[方法]從認(rèn)識(shí)論角度出發(fā),采用文獻(xiàn)整理的研究方法,查閱古今關(guān)于象思維的文獻(xiàn)資料。[結(jié)果]“象”分為物象、具象、意象三個(gè)類別。動(dòng)物之大象是象內(nèi)涵的起點(diǎn),古人通過(guò)對(duì)大象動(dòng)物含義的衍生,逐漸豐富著象的內(nèi)涵;衍生出天象的含義,天象對(duì)人類健康產(chǎn)生影響;衍生出相貌之象的含義,相貌之象是中醫(yī)辨證的重要依據(jù);此三者皆屬于物象的范疇,是象思維路徑觀物取象的體現(xiàn),而形象邏輯思維是其主要表現(xiàn)形式。隨著思維活動(dòng)的增強(qiáng)逐漸衍生出象征的含義,并在具象思維的應(yīng)用下,通過(guò)將脈象具象化,用以推導(dǎo)臟腑之象,是中醫(yī)辨證的另一重要來(lái)源,而中醫(yī)取象比類的思維方法則是象思維應(yīng)用的另一重要成果。此三者皆屬于具象的范疇,是象思維路徑立象以盡意的體現(xiàn)。意象是象思維路徑中得意忘象的體現(xiàn),通過(guò)對(duì)物象、具象的歸納、總結(jié)、推衍生成意向。象形文字的演化過(guò)程便體現(xiàn)了象思維得意忘象的過(guò)程。而卦象模型則是對(duì)自然之理的高度總結(jié),并對(duì)中醫(yī)認(rèn)識(shí)疾病產(chǎn)生深遠(yuǎn)影響。象思維模型對(duì)精子的認(rèn)識(shí):陰陽(yáng)模型是象思維模型中最基本的思維模型,將精液與女性生殖之精對(duì)比精液屬陽(yáng),將精液分陰陽(yáng),精子屬陽(yáng),精漿屬陰。藏象模型是對(duì)臟腑功能的總結(jié),通過(guò)對(duì)精子“諸生宗室”之象的歸納總結(jié),將其納入腎藏象模型,是象思維“立象以盡意”的體現(xiàn)。五行模型是連接臟腑與自然界關(guān)系的紐帶,通過(guò)所立腎之藏象闡述五行模型下對(duì)弱精子癥的認(rèn)識(shí)。部分弱精子癥患者,舌脈無(wú)明顯異常,僅以精液參數(shù)異常為其主要臨床表現(xiàn),辨證困難。而基于象思維,可以通過(guò)對(duì)精液“所視之象”與“所聞之象”的觀察對(duì)弱精子癥進(jìn)行辨證。所視之象包括精液量、性狀、顏色、微觀之象,所聞之象包括精液的氣味、患者所述病史。通過(guò)對(duì)其把握,從而進(jìn)行辨證治療是象思維路徑的體現(xiàn)�？偨Y(jié)導(dǎo)師理論及臨床經(jīng)驗(yàn)。導(dǎo)師運(yùn)用“清金資水”“調(diào)火治水”法治療弱精子癥的理論及其臨床經(jīng)驗(yàn),并在臨床上取得了較好的治療效果。[結(jié)論]象思維是中醫(yī)的原創(chuàng)思維,其通過(guò)觀物取象,立象盡意構(gòu)建象模型,并通過(guò)象模型對(duì)未知事物進(jìn)行聯(lián)系,比類,以達(dá)到認(rèn)識(shí)事物的目的。弱精子癥病因病機(jī)尚未明確,因此基于象思維探討弱精子癥的病因病機(jī)符合這一階段對(duì)疾病認(rèn)識(shí)的要求。通過(guò)象思維辨證論治弱精子癥取得了良好的臨床效果。
