【摘要】：目的觀察加味大承氣湯聯(lián)合常規(guī)治療對急性重癥胰腺炎患者凝血功能方面的影響。方法 2015年12月-2016年12月期間收治住院的急性重癥胰腺炎60例患者,采取隨機數(shù)字表法分為對照組(30例)和研究組(30例),兩組患者均采用常規(guī)對癥支持治療,在對照組的基礎上,研究組加用加味大承氣湯,觀察中醫(yī)證候療效、生化及凝血指標等,評價加味承氣湯對急性重癥胰腺炎凝血功能的作用。結(jié)果治療后研究組有效治愈率(96.67%)明顯高于對照組(80.00%)(P0.05);治療后兩組APACHEⅡ評分、血清AMY均較治療前下降,且研究組降低水平大于對照組(P0.05,P0.01)。血清鈣濃度逐漸增加,且治療組的增加幅度大于對照組(P0.05)。兩組血凝功能示研究組PT、APTT下降較對照組明顯(P0.05),FIB下降不明顯。結(jié)論加味承氣湯對急性重癥胰腺炎患者有一定的臨床療效,可改善患者凝血功能,血液高凝狀態(tài),故中醫(yī)藥在臨床治療胰腺炎凝血異常中值得推廣運用。
[Abstract]:Objective to observe the effect of modified Dachengqi decoction combined with routine therapy on coagulation function in patients with severe acute pancreatitis. Methods 60 patients with severe acute pancreatitis were admitted from December 2015 to December 2016. The study group was divided into control group (n = 30) and study group (n = 30). The patients in both groups were treated with routine symptomatic support. On the basis of the control group, the study group was treated with modified Dachengqi decoction to observe the curative effect of TCM syndromes. To evaluate the effect of Jiawei Chengqi decoction on coagulation function of severe acute pancreatitis, the effective cure rate of the study group was 96.677.Results the effective cure rate of the study group was significantly higher than that of the control group (P 0.05).
