【摘要】：目的:在中醫(yī)理論的指導(dǎo)下,運(yùn)用益氣養(yǎng)陰行氣潤(rùn)腸法治療老年人功能性便秘,科學(xué)系統(tǒng)地對(duì)受試者療效進(jìn)行評(píng)分、記錄、數(shù)據(jù)分析,對(duì)此方法的臨床療效進(jìn)行評(píng)價(jià)。方法:本研究采取隨機(jī)、對(duì)照的方法,將2015年9月至2016年11月間,河北省中醫(yī)院肛腸二科門(mén)診就診的老年功能性便秘患者中屬于中醫(yī)診斷為氣陰虧虛證型的病歷,共60例,分為治療組和對(duì)照組,每組30例。治療組予益氣養(yǎng)陰行氣潤(rùn)腸功效的中藥顆粒劑口服,對(duì)照組予麻仁軟膠囊口服,兩組均以10天為1療程,共治療和觀察記錄3個(gè)療程,將兩組患者治療前后的臨床癥狀,按評(píng)分標(biāo)準(zhǔn)打分。治療結(jié)束4周后對(duì)兩組患者進(jìn)行隨訪(fǎng),按評(píng)分標(biāo)準(zhǔn)打分。對(duì)兩組最終收集的數(shù)據(jù),使用SPSS19.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析,從多角度評(píng)價(jià)本方法的臨床療效。治療期間,囑全部患者停止使用影響腸動(dòng)力的藥物,合理膳食,規(guī)律作息,積極配合。結(jié)果:共完成臨床觀察病例52例,包括治療組27例,對(duì)照組25例。1治療組與對(duì)照組綜合療效比較,總有效率治療組為85.2%,對(duì)照組為72.0%,兩組總有效率比較P0.05,無(wú)統(tǒng)計(jì)學(xué)意義,說(shuō)明兩組治療方法均有效;總顯效率治療組33.3%,對(duì)照組8.0%,P0.05,說(shuō)明有統(tǒng)計(jì)學(xué)意義,治療組療效優(yōu)于對(duì)照組;2性別與療效的相關(guān)性比較,男性患者兩種治療的療效無(wú)差異,女性患者兩種治療的療效無(wú)差異。治療組男性與女性患者療效無(wú)差異,對(duì)照組男性與女性患者療效無(wú)差異;3病程與療效的相關(guān)性比較,病程在5年內(nèi)及10年以上的患者中治療組療效優(yōu)于對(duì)照組;4兩組受試者3個(gè)療程治療結(jié)束,4周后隨訪(fǎng),治療組復(fù)發(fā)率為17.4%,對(duì)照組復(fù)發(fā)率為50%,兩組復(fù)發(fā)率經(jīng)卡方檢驗(yàn):χ2=4.96,P=0.03,P0.05,有統(tǒng)計(jì)學(xué)差異,說(shuō)明遠(yuǎn)期療效治療組優(yōu)于對(duì)照組。結(jié)論:益氣養(yǎng)陰行氣潤(rùn)腸法對(duì)老年人功能性便秘的療效確切,在綜合療效、較長(zhǎng)病程的老年人功能性便秘的治療方面效果顯著,且復(fù)發(fā)率低,無(wú)明顯副作用,值得推廣。
[Abstract]:Objective: under the guidance of traditional Chinese medicine theory, the treatment of functional constipation in the elderly with the method of nourishing qi and nourishing yin and nourishing qi and nourishing intestines was used to scientifically and systematically evaluate, record and analyze the curative effect of the subjects. Methods: a randomized, controlled method was used to evaluate the clinical efficacy of this method from September 2015 to November 2016. Among the elderly patients with functional constipation diagnosed as deficiency of qi and yin, 60 cases were divided into treatment group and control group. 30 cases in each group. The treatment group was treated with traditional Chinese medicine granules of invigorating qi, nourishing yin and nourishing qi and nourishing intestines. The control group was given Maren soft capsule orally. The two groups were treated with 10 days as a course of treatment, and three courses of treatment were observed and recorded. The clinical symptoms of the two groups before and after treatment were scored according to the scoring standard. The patients in the two groups were followed up 4 weeks after the treatment and scored according to the scoring standard. The final data collected in the two groups were statistically analyzed by SPSS19.0 statistical software. The clinical effect of this method was evaluated from various angles. During the treatment, all patients were told to stop using drugs affecting intestinal motility, reasonable diet, regular rest and active cooperation. Results: a total of 52 cases of clinical observation were completed, including 27 cases in the treatment group. The total effective rate was 85.2in the treatment group and 72.0 in the control group. There was no statistical significance in the total effective rate between the two groups (P 0.05), which indicated that the two treatment methods were effective. The total effective rate of treatment group was 33. 3 and that of control group was 8. 0% P 0. 05, which indicated that there was statistical significance. The curative effect of treatment group was better than that of control group. There was no difference in curative effect between male and female patients in treatment group, and there was no difference between male and female patients in control group. The curative effect of the treatment group was better than that of the control group in the course of 5 years or more than 10 years. The patients in the two groups were followed up for 4 weeks after 3 courses of treatment. The recurrence rate of the treatment group was 17.4 and that of the control group was 50. The recurrence rate of the two groups was tested by chi-square test. There was statistical difference between the two groups, indicating that the long-term curative effect of the treatment group was better than that of the control group. Conclusion: the therapy of nourishing qi, nourishing yin and nourishing qi and nourishing the intestines is effective in treating functional constipation in the elderly. In the treatment of functional constipation in the elderly with long course of disease, the curative effect is significant, and the recurrence rate is low, no obvious side effects, so it is worth popularizing.
