【摘要】：目的觀察張晉峰教授經(jīng)驗(yàn)方益坤抗衰湯聯(lián)合克齡蒙治療腎虛型卵巢早衰的臨床療效并初步探討其作用機(jī)理。方法嚴(yán)格按照納入標(biāo)準(zhǔn)及排除標(biāo)準(zhǔn),門診收集60例符合入選標(biāo)準(zhǔn)的腎虛型卵巢早衰患者,隨機(jī)分為三組,治療組為益坤抗衰湯聯(lián)合克齡蒙組,簡稱益克聯(lián)合組;中藥對照組為益坤抗衰組;西藥對照組為克齡組。益克聯(lián)合組服用益坤抗衰湯聯(lián)合克齡蒙;益坤抗衰組服用益坤抗衰湯;克齡組服用克齡蒙。治療周期均為3個月,治療結(jié)束后均再觀察3個月。觀察治療前后的疾病綜合療效、中醫(yī)證候療效、中醫(yī)證候總積分變化情況、月經(jīng)不調(diào)積分改善情況、中醫(yī)主癥腰膝酸軟積分變化情況、FSH值、LH值、E2值變化情況、子宮內(nèi)膜厚度及左右卵巢大小變化情況。并注意血常規(guī)、尿常規(guī)、凝血常規(guī)、肝腎功能、心電圖、子宮及雙側(cè)附件彩超、雙側(cè)乳腺彩超等安全性等觀測。所有數(shù)據(jù)均采用SPSS16.0軟件進(jìn)行統(tǒng)計學(xué)分析,并依據(jù)統(tǒng)計結(jié)果,做出結(jié)論。結(jié)果益克聯(lián)合組治療腎虛型卵巢早衰總有效率為89.47%,中醫(yī)證候療效有效率94.73%。治療后患者FSH值、LH值較治療前均顯著下降(P0.01;P0.01)、E2值較治療前顯著升高(P0.01);子宮內(nèi)膜厚度較治療前也有顯著增厚(P0.01);月經(jīng)不調(diào)積分、腰膝酸軟積分及中醫(yī)證候總積分均有顯著下降(P0.01)。結(jié)論益坤抗衰湯聯(lián)合克齡蒙治療腎虛型卵巢早衰有確切的臨床療效,可以有效地改善腎虛型卵巢早衰患者的月經(jīng)不調(diào)情況、改善中醫(yī)證候積分、減輕腰膝酸軟的癥狀、降低FSH值、LH值,升高E2值、增加子宮內(nèi)膜厚度。益克聯(lián)合組療效明顯優(yōu)于克齡組及益坤抗衰組。益坤抗衰湯聯(lián)合克齡蒙治療腎虛型卵巢早衰療效確切,安全性好,值得臨床進(jìn)一步研究與推廣。
[Abstract]:Objective to observe the clinical effect of Professor Zhang Jinfeng's experience prescription Yikun Kangshuai decoction combined with Kelingmeng in the treatment of premature ovarian failure of kidney deficiency type and to explore its mechanism. Methods according to the inclusive standard and exclusion standard strictly. Sixty patients with premature ovarian failure of kidney deficiency type were randomly divided into three groups. The treatment group was treated with Yikun Kangshuai decoction combined with Ke Ling Meng group. The traditional Chinese medicine control group: Yikun anti-aging group; The western medicine control group was Keling group, Yike combination group was treated with Yikun Kangshuai decoction combined with Kelingmen; Yikun Kangshuai group took Yikun Kangshuai decoction; The treatment period of Keling group was 3 months and the treatment period was 3 months. The comprehensive curative effect of the disease before and after treatment the curative effect of TCM syndromes and the change of total integral of TCM syndrome were observed before and after the treatment. Improvement of menstrual irregular score, changes of Lumbar and knee acid and soft score in TCM, changes of FSH value and LH value and E2 value, endometrial thickness and size of left and right ovaries, and pay attention to blood routine. Urine routine, coagulation routine, liver and kidney function, electrocardiogram, uterus and bilateral accessory color ultrasound, bilateral breast color ultrasound and other safety observation. All data were statistically analyzed by SPSS16.0 software. Results the total effective rate of treatment of kidney deficiency type premature ovarian failure was 89.47, and the effective rate of TCM syndromes was 94.73. The FSH value of patients after treatment was obtained. The value of LH was significantly lower than that before treatment (P 0.01). The E _ 2 value of P _ (0.01) was significantly higher than that before treatment. The thickness of endometrium was significantly thicker than that before treatment. Menstrual disorder score, waist and knee acid-soft score and total integral of TCM syndromes were significantly decreased P0.01.Conclusion Yikun Kangshuai decoction combined with Kelingmeng has definite clinical curative effect on premature ovarian failure of kidney deficiency type. Can effectively improve the kidney deficiency type of premature ovarian failure patients menstrual disorder, improve the TCM syndrome score, reduce the waist and knee pain and soft symptoms, reduce the FSH value of LH, increase E2 value. Increase the thickness of endometrium. Yike combination group is significantly better than the Ke Ling group and Yikun anti-aging group. Yikun Kangshuai decoction combined with Ke Ling Meng treatment of kidney deficiency type of premature ovarian failure is effective and safe. It is worthy of further clinical research and promotion.
