【摘要】：自2012年我榮幸成為全國(guó)老中醫(yī)藥專家王莒生的學(xué)術(shù)經(jīng)驗(yàn)繼承人,在三年的跟師學(xué)習(xí)過(guò)程中,結(jié)合王老師的耐心講解及自身感悟,我深刻體會(huì)到老師在臨床中的治病理念與診療技巧,較全面的總結(jié)出王老師的學(xué)術(shù)思想和臨床經(jīng)驗(yàn)。王莒生老師是首都醫(yī)科大學(xué)附屬北京中醫(yī)醫(yī)院皮科主任醫(yī)師,國(guó)家級(jí)名老中醫(yī),全國(guó)第四、五批老中醫(yī)藥專家學(xué)術(shù)經(jīng)驗(yàn)繼承指導(dǎo)老師,享受國(guó)務(wù)院政府特殊津貼。在幾十年的行醫(yī)生涯中,累積了豐富的臨床經(jīng)驗(yàn),擅長(zhǎng)治療白癜風(fēng)、濕疹、銀屑病、蕁麻疹、痤瘡等皮膚科常見(jiàn)病。對(duì)于內(nèi)科呼吸道疾病、老年性便秘等疾病的防治也有著自己獨(dú)特的觀點(diǎn)；對(duì)于兒科皮膚病、呼吸道疾病等也頗有研究。王莒生老師的學(xué)術(shù)思想主要受趙炳南和關(guān)幼波等中醫(yī)專家影響,一是“藥療”與“話療"相結(jié)合的中醫(yī)治療整體觀,講究診病時(shí)藥物治療與心理疏導(dǎo)相結(jié)合,真正做到整體治療；二是“肝肺同治”治療皮膚病的治療觀；三是內(nèi)科雜病及疑難雜癥“從肝論治"的思想。臨床經(jīng)驗(yàn)總結(jié)方面,我系統(tǒng)總結(jié)了王老師治療濕疹、白癜風(fēng)、銀屑病、蕁麻疹、痤瘡、慢性咳嗽、老年性便秘等疾病的經(jīng)驗(yàn),歸納整理了老師特色的治療方法和學(xué)術(shù)思想,總結(jié)了老師在藥物應(yīng)用及方劑使用方面的特色,著重介紹了能體現(xiàn)老師學(xué)術(shù)價(jià)值的代表方劑和常用藥物的使用經(jīng)驗(yàn)。本文的主要內(nèi)容分為三大部分：一、王莒生老中醫(yī)藥專家學(xué)術(shù)淵源王莒生老師提倡尊崇經(jīng)典,融會(huì)新知。推崇《黃帝內(nèi)經(jīng)》的養(yǎng)生理念；善用《傷寒論》、《金匱要略》的經(jīng)典方劑治療疾病,主張“古方今用”；在學(xué)習(xí)經(jīng)典的基礎(chǔ)上,積極吸取現(xiàn)代醫(yī)學(xué)成果,從各種疾病的最新治療指南到中藥藥理學(xué)新進(jìn)展,靡不涉獵。王莒生老師曾師從我院多名中醫(yī)大師,其中醫(yī)學(xué)術(shù)特點(diǎn)的形成與其工作經(jīng)歷息息相關(guān)。王老師從科研及門(mén)診兩方面與各位名老中醫(yī)進(jìn)行學(xué)習(xí),繼承并發(fā)揚(yáng)了他們的學(xué)術(shù)思想及臨床經(jīng)驗(yàn),內(nèi)科治療尊崇關(guān)幼波老師氣血辨證理論,與八綱辨證相結(jié)合,形成“十綱辨證”法,重視”從肝論治”,應(yīng)用于多種疾病的辨證治療中。皮膚病診治思想深受皮科泰斗趙炳南先生影響,效法趙老外病內(nèi)治思想,善于外病內(nèi)治,整體治療,并提出“肝肺同治”,在皮膚病治療中發(fā)揮了重要的作用。呼吸病深受許公巖、滕宣光、林杰豪、曹希平等人的學(xué)術(shù)思想影響,強(qiáng)調(diào)痰濕是咳喘病久治不愈的重要內(nèi)因,主張治咳喘重在明確咳喘與五臟的關(guān)系,從臟腑失衡方面入手進(jìn)行治療。兒科受溫振英老中醫(yī)影響,推崇補(bǔ)益氣陰,并將這一治療理念運(yùn)用到許多兒科疾病的治療當(dāng)中。二、學(xué)術(shù)思想主要特點(diǎn)1.“藥療”與“話療”相結(jié)合的中醫(yī)治療整體觀王莒生老師從整體觀念出發(fā),辨證時(shí)既重視當(dāng)前病證,又重視全面觀察分析與現(xiàn)病證相關(guān)的其他病證及隱患。論治時(shí)緊抓根本,在考慮解決主要癥狀的同時(shí),對(duì)其他病證適當(dāng)兼顧。她認(rèn)為真正的醫(yī)學(xué)模式應(yīng)該是與社會(huì)、環(huán)境、身心相關(guān)的模式,講究診病時(shí)藥物治療與心理疏導(dǎo)相結(jié)合,真正做到整體治療,并用于指導(dǎo)臨床治療。2.皮膚類疾病應(yīng)“肝肺同治”的思想王莒生老師認(rèn)為皮膚病雖發(fā)于外,但常常與內(nèi)臟有著密切的關(guān)系。尤其是與“肺”、“肝”二臟的關(guān)系密切,一方面“肺主皮毛”,空氣污染等外在因素直接影響到皮膚病變；另一方面社會(huì)壓力增大,導(dǎo)致患者肝氣不舒,加之皮膚病常常反復(fù)發(fā)作,干擾患者的日常生活,常致肝郁,因此“疏肝理氣”、“開(kāi)肺竅”是王莒生老師治療皮膚科疾病的重要的學(xué)術(shù)思想。3.“從肝論治”治療內(nèi)科雜病及疑難雜癥的思想臨床診治內(nèi)科雜病時(shí),王莒生老師尤其強(qiáng)調(diào)“疏肝”的重要性,診病施治時(shí),時(shí)時(shí)注重對(duì)肝臟的調(diào)理,其重視“疏肝”的思想不僅要體現(xiàn)在皮膚科病證的診治方面,在內(nèi)科其它各種疾病的診治過(guò)程中,也需貫徹這一思想,故在臨床中立足“疏肝”,論治多種疾病,往往取得意想不到的效果。4.臨床經(jīng)驗(yàn)主要特點(diǎn)系統(tǒng)總結(jié)王老師治療濕疹、白癜風(fēng)、銀屑病、蕁麻疹、痤瘡、慢性咳嗽、便秘等疾病的經(jīng)驗(yàn),歸納整理了老師特色的治療方法和學(xué)術(shù)思想,總結(jié)了老師在藥物應(yīng)用及方劑使用方面的特色,著重介紹了能體現(xiàn)老師學(xué)術(shù)價(jià)值的代表方劑和常用藥物的使用經(jīng)驗(yàn)。三、專病研究通過(guò)臨床試驗(yàn)反映王莒生老師治療濕疹的學(xué)術(shù)思想及經(jīng)驗(yàn)特色為開(kāi)肺竅；王老師運(yùn)用浮楮清熱除濕湯治療急性濕疹的機(jī)理在于該方藥可以通過(guò)細(xì)胞因子白介素的調(diào)節(jié)對(duì)急性濕疹進(jìn)行治療,臨床試驗(yàn)發(fā)現(xiàn)該方劑對(duì)細(xì)胞因子IL-2、IL-4、INF-γ有一定的干預(yù)作用,有望找到中藥治療該疾病的靶點(diǎn)所在,有待進(jìn)一步研究證實(shí)。
[Abstract]:Since 2012, I have the honor to become the national old TCM expert Wang Jusheng's academic experience inheritance, learning with the teacher in the course of three years, according to Wang's own feeling and patience to explain, I deeply appreciate the teacher in the clinical treatment and treatment concept of comprehensive skills, and the academic thought and clinical experience of the king the teacher. Mr. Wang Jusheng is the chief physician of the Department of Dermatology of Beijing Chinese Medicine Hospital affiliated to Capital Medical University. He is a national famous veteran doctor of traditional Chinese medicine. He is the fourth, fifth batch of veteran doctors of traditional Chinese medicine who inherits the academic experience, and he enjoys the special allowance of the State Council. In the decades of medical career, accumulated rich clinical experience, good at the treatment of vitiligo, eczema, psoriasis, urticaria, acne, and other common diseases in the Department of dermatology. It also has its own unique views on the prevention and treatment of respiratory diseases, senile constipation and other diseases. For pediatric dermatology, respiratory diseases and so on, there are also some researches. Professor Wang Jusheng's academic thought mainly by Zhao Bingnan and Guan Youbo and other Chinese experts, one is "medication" and "talk therapy" treatment of traditional Chinese medicine combining whole concept, pay attention to diagnosis when drug therapy and psychological counseling combined with truly holistic treatment; two is "treatment of liver and lung with the treatment of skin diseases; three miscellaneous diseases and Difficult miscellaneous diseases" from the liver "thought. To summarize the clinical experience, I summarized the teacher Wang treatment of eczema, vitiligo, psoriasis, urticaria, acne, chronic cough, constipation and other diseases, induction treatment and finishing the teacher academic thought characteristic, summarizes the characteristics of the teacher use in drug application and prescription, emphatically introduces the experience to reflect the use of teacher academic value and representative prescription drugs. The main contents of this article are divided into three parts: 1. The academic origin of Wang Jusheng's old Chinese medicine expert, Wang Jusheng, advocates respecting the classics and integrating new knowledge. The idea of "the respected" Yellow Emperor; the use of "typhoid fever", "Golden Chamber" of the classic prescription for treating diseases, advocates of "ancient means"; on the basis of classical learning, and actively absorb the achievement of modern medicine from various diseases to the latest guidelines for the treatment of Chinese medicine pharmacology progress doesn't dabble. Mr. Wang Jusheng had been a master of traditional Chinese medicine from our hospital. The formation of the academic characteristics of traditional Chinese medicine was closely related to its work experience. Mr. Wang from the two aspects of scientific research and clinic with all the old Chinese medicine to learn, inherit and carry forward the academic thought and clinical experience of their medical treatment, respect the teacher acted off syndrome differentiation of Qi and blood theory, combined with the eight principal syndromes, the formation of "ten Gang syndrome", "attention from the liver, syndrome differentiation and treatment application in a variety of diseases in. Skin disease diagnosis thinking by the dermatology Dean Mr. Zhao Bingnan, according to Zhao foreigner within the disease, good within the disease, overall treatment, and put forward the "liver lung Tongzhi", played an important role in treating skin diseases. Influence of the academic thoughts of respiratory disease by Xu Gongyan, Teng Xuanguang, Lin Jiehao, Cao Xiping et al., stressed that phlegm is an important internal cause of asthma treatment, cure cough cough is clear that the relationship with the five internal organs, starting from the treatment of viscera imbalance.
