胃腸道腫瘤術后輔助化療病人營養(yǎng)不良現(xiàn)狀調查與分析

【摘要】:目的 :通過對胃腸道腫瘤術后輔助化療病人進行營養(yǎng)風險篩查,調查營養(yǎng)不良的發(fā)生率并進行原因分析,為該類病人臨床營養(yǎng)干預治療提供依據(jù)。方法 :自2016年7月至12月對入住普通外科胃腸腫瘤術后并行輔助化療的181例病人,運用PG-SGA進行營養(yǎng)風險篩查,同時檢測血清白蛋白、血紅蛋白和白細胞,記錄病人化療期間的不良反應。根據(jù)性別、年齡、不同腫瘤部位、化療療程、癥狀等進行不同分值的亞組分析。結果 :所有181例病人均完成營養(yǎng)風險篩查,其中PG-SGA得分在0~3分的69例(占38.1%),4~8分即中度營養(yǎng)不良的71例(占39.2%);大于8分即存在嚴重營養(yǎng)不良的41例(占22.7%),4分以上即存在營養(yǎng)不良的共112例占61.9%。對112例營養(yǎng)不良病人進一步分析:胃癌病人PG-SGA得分明顯高于直腸癌病人(P0.05)和結腸癌病人(P0.01);男性發(fā)生率高于女性(P0.05);年齡大于65歲的營養(yǎng)不良發(fā)生率高于65歲以下病人(P0.01)。PG-SGA得分越高化療藥物引起的惡心、嘔吐等毒副反應越重,血紅蛋白、白細胞水平也越低(P0.01)。營養(yǎng)不良與病人化療療程無統(tǒng)計學差異。結論 :胃腸道腫瘤術后輔助化療病人營養(yǎng)不良發(fā)生率高,腫瘤部位、性別、年齡和化療不良反應等是高危因素。
