酶聯(lián)免疫吸附法測定腫瘤性貧血患者的血清細胞因子水平及其發(fā)病機制研究

【摘要】:目的研究腫瘤壞死因子-α(TNF-α)、紅細胞生成素(EPO)、干擾素-γ(IFN-γ)對腫瘤性貧血(CRA)的作用機制。方法將惡性腫瘤患者分為試驗組(腫瘤伴貧血,76例)和對照組(腫瘤無貧血,21例);再將試驗組患者照鐵代謝參數(shù)分為試驗Ⅰ組(29例)和試驗Ⅱ組(47例),另選擇50例健康志愿者作為正常組。用放射免疫法檢測各組血清EPO、鐵蛋白(SF)水平,以酶聯(lián)免疫吸附試驗檢測血清TNF-α、IFN-γ水平。抽取正常骨髓,常規(guī)培養(yǎng),分別加入不同濃度TNF-α、IFN-γ,對骨髓紅細胞集落生成單位(CFU-E)計數(shù);培養(yǎng)體系中再加入不同濃度的重組人促紅素(rhEPO),對各濃度時CFU-E進行計數(shù)。結(jié)果試驗Ⅰ組、試驗Ⅱ組及對照組的血清TNF-α分別為(137.8±13.2),(241.4±19.8),(113.7±12.6)ng·L~(-1);這3組的EPO分別為(513.7±37.1),(268.8±21.9),(94.6±11.4)ng·L~(-1);這3組的IFN-γ分別為(191.5±16.5),(289.7±20.3),(157.9±14.7)ng·L~(-1)。這3組的這3種指標均顯著高于正常組的(51.3±8.4),(56.8±7.1),(57.4±7.8)ng·L~(-1),差異有統(tǒng)計學意義(均P0.05);同時,試驗Ⅱ組血清EPO顯著高于對照組,但顯著低于試驗Ⅰ組,差異有統(tǒng)計學意義(P0.05)。體外細胞培養(yǎng)結(jié)果顯示,TNF-α、IFN-γ均對正常骨髓CFU-E具有抑制作用,與濃度有依賴性且呈正相關(guān);在培養(yǎng)體系中加入rhEPO,能夠改善TNF-α、IFN-γ的抑制作用;在2×10~(-10)~32×10~(-10)mol·mL~(-1)內(nèi),rhEPO濃度越高,這種改善作用越明顯,但依然無法恢復至正常水平。結(jié)論在TNF-α、INF-γ等細胞因子作用下,骨髓紅系對EPO的反應性不足,這可能是腫瘤性貧血的發(fā)病機制之一。
