【摘要】：目的:探討腎臟惡性孤立性纖維腫瘤(Malignant solitary fibroustumor,MSFT)的臨床表現(xiàn)、影像學(xué)特點、診斷與鑒別診斷、治療及預(yù)后。方法:回顧性分析1例罕見的腎惡性SFT患者的臨床資料,結(jié)合相關(guān)文獻(xiàn)分析該病的組織來源、病理學(xué)特征、鑒別診斷及治療隨訪情況。結(jié)果:29歲男性患者1例,術(shù)前診斷為右腎透明細(xì)胞癌,行后腹腔鏡下根治性腎切除術(shù),術(shù)后病理為右腎低度惡性SFT,免疫組化提示CD34(+)、BCL-2(+)、CD68(+)、CD99(+)、vimentin(+)、Ki 67 5%(+)、SMA局灶弱陽性。隨訪4月,未見腫瘤復(fù)發(fā)或轉(zhuǎn)移。結(jié)論:腎臟惡性SFT非常罕見,診斷與鑒別診斷依據(jù)術(shù)后病理學(xué)及免疫組化學(xué),主要治療方式為根治性腎切除術(shù),預(yù)后較好。
[Abstract]:Objective: to investigate the clinical manifestations, imaging features, diagnosis and differential diagnosis, treatment and prognosis of renal malignant solitary fibrous tumor (Malignant solitary fibroustumor,MSFT). Methods: the clinical data of a rare case of renal malignant SFT were retrospectively analyzed. The histological origin, pathological features, differential diagnosis and therapeutic follow-up of the disease were analyzed. Results: a 29-year-old male patient was diagnosed as right renal clear cell carcinoma before operation. Retroperitoneal laparoscopic radical nephrectomy was performed. Postoperative pathology was low grade malignant SFT, immunohistochemical diagnosis of right kidney. CD34 (), BCL-2 (), CD68 (), was suggested by immunohistochemistry. CD99 (), vimentin (), Ki 67.5% (), SMA was weakly positive. Follow-up for 4 months showed no recurrence or metastasis of the tumor. Conclusion: renal malignant SFT is very rare. The diagnosis and differential diagnosis are based on postoperative pathology and immuno-histochemistry. The main treatment is radical nephrectomy with a good prognosis.
