【摘要】：二甲雙胍是目前使用最為普遍的口服降糖藥物之一,其降糖以外的抗腫瘤作用逐漸被人們認知,二甲雙胍能抑制多種類型腫瘤進展,其中包括大腸癌。二甲雙胍對大腸癌的防治作用主要通過以下方面:激活A(yù)MPK途徑、抑制腫瘤血管生成、免疫調(diào)節(jié)、增強腫瘤細胞對化療藥物敏感性及抑制腫瘤干細胞等。長期大量實驗性研究證實二甲雙胍對大腸癌防治均有一定療效,因此二甲雙胍有望成為新一代抗腫瘤藥物。但由于目前多數(shù)二甲雙胍對大腸癌防治的臨床研究均存在明顯偏倚性、隨機對照試驗極少、循證醫(yī)學(xué)依據(jù)尚且不足,因此關(guān)于二甲雙胍是否應(yīng)繼續(xù)用于大腸癌防治的問題存在廣泛爭議。
[Abstract]:Metformin is one of the most commonly used oral hypoglycemic drugs. Its antitumor effects beyond hypoglycemia have been gradually recognized. Metformin can inhibit the progression of many types of tumors, including colorectal cancer. The preventive and therapeutic effects of metformin on colorectal cancer are as follows: activation of AMPK pathway, inhibition of tumor angiogenesis, immunomodulation, enhancement of tumor cell sensitivity to chemotherapeutic drugs and inhibition of tumor stem cells, etc. A large number of experimental studies have proved that metformin has a certain therapeutic effect on the prevention and treatment of colorectal cancer, so metformin is expected to become a new generation of antitumor drugs. However, most of the clinical studies on the prevention and treatment of colorectal cancer by metformin have obvious bias, and there are few randomized controlled trials and insufficient evidence based medicine. Therefore, whether metformin should continue to be used in the prevention and treatment of colorectal cancer is controversial.
