【摘要】：目的:探討可切除肺癌伴不同肺葉結(jié)節(jié)的診斷及處理策略。方法:回顧性分析2014年1月—2016年11月我科62例手術(shù)切除肺癌原發(fā)灶伴肺內(nèi)結(jié)節(jié)患者的臨床資料,分別從結(jié)節(jié)大小、數(shù)量,肺門、縱隔淋巴結(jié)轉(zhuǎn)移情況,PET-CT檢查,原發(fā)癌分期及結(jié)節(jié)與原發(fā)灶的關(guān)系等方面進(jìn)行鑒別分析。結(jié)果:62例肺癌患者均行手術(shù)切除不同肺葉結(jié)節(jié),38例病理檢查提示良性結(jié)節(jié),占病例總數(shù)的61%。這些良性結(jié)節(jié)中纖維組織占有16例、炎性假瘤占有5例、錯(cuò)構(gòu)瘤占有3例、硬化性血管瘤占有1例、其他良性病變13例。24例病理檢查為惡性結(jié)節(jié),占病例總數(shù)的39%,其中為雙原發(fā)肺癌1例。結(jié)論:肺癌伴不同肺葉結(jié)節(jié)應(yīng)積極通過各種臨床方法及手段進(jìn)行鑒別診斷處理,避免將一部分肺癌伴良性結(jié)節(jié)的患者誤診為肺癌伴肺內(nèi)轉(zhuǎn)移,延誤了早期肺癌患者的外科治療,影響患者的預(yù)后并增加其心理負(fù)擔(dān),因此肺癌伴隨不同肺葉結(jié)節(jié)應(yīng)積極行外科治療。
[Abstract]:Objective: to investigate the diagnosis and management of resectable lung cancer with different lobar nodules. Methods: from January 2014 to November 2016, the clinical data of 62 patients with primary lung cancer with intrapulmonary nodules were analyzed retrospectively. The size, number, hilar, mediastinal lymph node metastasis and PET-CT examination were analyzed respectively. The stage of primary carcinoma and the relationship between nodule and primary tumor were analyzed. Results: 62 patients with lung cancer underwent surgical resection of different lobar nodules, 38 cases showed benign nodules, accounting for 61% of the total number of cases. Among these benign nodules, 16 cases were fibrous tissue, 5 cases were inflammatory pseudotumor, 3 cases were hamartoma, 1 case was sclerosing hemangioma, 13 cases were other benign lesions. There was one case of double primary lung cancer. Conclusion: the differential diagnosis and treatment of lung cancer with different pulmonary lobar nodules should be carried out actively through various clinical methods and means to avoid misdiagnosis of some patients with lung cancer with benign nodules as lung cancer with intrapulmonary metastasis, which delays the surgical treatment of early lung cancer patients. The prognosis is affected and psychological burden is increased, so lung cancer with different lobar nodules should be actively treated by surgery.
