【摘要】：[目的]本研究通過檢測(cè)宣威肺癌、非宣威肺癌、宣威肺良性病、健康對(duì)照四組人群血漿中FENDRR、H19基因的表達(dá)水平,研究FENDRR、H19在宣威肺癌患者血漿中的變化趨勢(shì)及臨床意義,評(píng)價(jià)FENDRR、H19在宣威肺癌患者中的潛在診斷價(jià)值。[方法]收集2015年6月至2016年8月昆明醫(yī)科大學(xué)第一附屬醫(yī)院患者術(shù)前以及性別年齡匹配的體檢健康者血漿,共163例樣本。樣本分宣威肺癌、非宣威肺癌、宣威良性肺疾病、健康對(duì)照4組,其中宣威肺癌43例,其它3組每組各40例。采用實(shí)時(shí)熒光定量PCR方法檢測(cè)研究對(duì)象血漿中FENDRR、H19基因的表達(dá)水平。并對(duì)宣威肺癌患者臨床病例資料進(jìn)行統(tǒng)計(jì)分析,分析FENDRR、H19表達(dá)水平與患者性別、年齡、吸煙情況、TNM分期、淋巴結(jié)轉(zhuǎn)移情況、腫塊大小和組織學(xué)類型的相關(guān)性,評(píng)價(jià)血漿FENDRR和H19作為宣威肺癌篩查標(biāo)志物的診斷價(jià)值。[結(jié)果]與對(duì)照組相比,FENDRR、H19在宣威肺癌組、非宣威肺癌組中表達(dá)水平整體呈下調(diào)趨勢(shì)(P0.05)。FENDRR、H19的表達(dá)水平與患者性別、年齡、吸煙史、TNM分期、組織學(xué)類型、腫塊大小均無相關(guān)性(P0.05);H19的表達(dá)水平與患者淋巴結(jié)轉(zhuǎn)移情況有相關(guān)性(P0.05)。NSE的AUC值為0.648,敏感性和特異性分別為52.6%和78.9%;CEA的AUC值為0.759,敏感性和特異性分別為68.4%和 73.7%;聯(lián)合檢測(cè) NSE 和 CEA 時(shí),AUC 值為 0.778。FENDRR的AUC值為 0.812,敏感性和特異性分別為90.7%和75%;H19的AUC值為0.833,敏感性和特異性分別為74.4%和85%;而當(dāng)聯(lián)合檢測(cè)FENDRR和H19時(shí),AUC值為0.894。[結(jié)論]FENDRR、H19在宣威肺癌的發(fā)生、發(fā)展中發(fā)揮著重要作用,它們可以作為宣威肺癌潛在的診斷標(biāo)志物,而且診斷價(jià)值優(yōu)于傳統(tǒng)標(biāo)志物。
[Abstract]:[objective] to investigate the expression of FENDRR,H19 gene in the plasma of Xuanwei lung cancer, non-Xuanwei lung cancer, Xuanwei lung benign venereal disease and healthy control group, and to study the change trend and clinical significance of FENDRR,H19 in the plasma of Xuanwei lung cancer patients. To evaluate the potential diagnostic value of FENDRR,H19 in Xuanwei lung cancer patients. Methods from June 2015 to August 2016, a total of 163 plasma samples were collected from patients in the first affiliated Hospital of Kunming Medical University. The samples were divided into 4 groups: Xuanwei lung cancer, non-Xuanwei lung cancer, Xuanwei benign lung disease, healthy control group, 43 cases of Xuanwei lung cancer, 40 cases each of the other three groups. Real-time fluorescence quantitative PCR was used to detect the expression of FENDRR,H19 gene in plasma. The correlation between FENDRR,H19 expression and sex, age, smoking status, TNM stage, lymph node metastasis, tumor size and histological type was analyzed. To evaluate the diagnostic value of plasma FENDRR and H 19 as screening markers of Xuan Wei lung cancer. [results] compared with the control group, the expression of FENDRR,H19 in Xuanwei lung cancer group and non-Xuanwei lung cancer group showed a downward trend (P0.05). The expression level of FENDRR,H19 was correlated with sex, age, smoking history, TNM stage and histological type. There was no correlation between tumor size and tumor size (P0.05). The expression of H19 was correlated with lymph node metastasis (P0.05 AUC value of). NSE was 0.648, sensitivity and specificity were 52.6% and 78.9%, respectively); The AUC value of CEA was 0.759, the sensitivity and specificity were 68.4% and 73.7%, the AUC value of 0.778.FENDRR was 0.812, the sensitivity and specificity were 90.7% and 75%, respectively. The AUC value of H19 was 0.833, the sensitivity and specificity of H19 were 74.4% and 85.When combined detection of FENDRR and H19, the AUC value was 0.894. [conclusion] FENDRR,H19 plays an important role in the occurrence and development of Xuanwei lung cancer. They can be used as potential diagnostic markers of Xuanwei lung cancer, and the diagnostic value is superior to that of traditional markers.
