血漿長鏈非編碼RNA FENDRR、H19在宣威肺癌中的潛在診斷價值研究

【摘要】:[目的]本研究通過檢測宣威肺癌、非宣威肺癌、宣威肺良性病、健康對照四組人群血漿中FENDRR、H19基因的表達水平,研究FENDRR、H19在宣威肺癌患者血漿中的變化趨勢及臨床意義,評價FENDRR、H19在宣威肺癌患者中的潛在診斷價值。[方法]收集2015年6月至2016年8月昆明醫(yī)科大學第一附屬醫(yī)院患者術前以及性別年齡匹配的體檢健康者血漿,共163例樣本。樣本分宣威肺癌、非宣威肺癌、宣威良性肺疾病、健康對照4組,其中宣威肺癌43例,其它3組每組各40例。采用實時熒光定量PCR方法檢測研究對象血漿中FENDRR、H19基因的表達水平。并對宣威肺癌患者臨床病例資料進行統(tǒng)計分析,分析FENDRR、H19表達水平與患者性別、年齡、吸煙情況、TNM分期、淋巴結轉移情況、腫塊大小和組織學類型的相關性,評價血漿FENDRR和H19作為宣威肺癌篩查標志物的診斷價值。[結果]與對照組相比,FENDRR、H19在宣威肺癌組、非宣威肺癌組中表達水平整體呈下調趨勢(P0.05)。FENDRR、H19的表達水平與患者性別、年齡、吸煙史、TNM分期、組織學類型、腫塊大小均無相關性(P0.05);H19的表達水平與患者淋巴結轉移情況有相關性(P0.05)。NSE的AUC值為0.648,敏感性和特異性分別為52.6%和78.9%;CEA的AUC值為0.759,敏感性和特異性分別為68.4%和 73.7%;聯(lián)合檢測 NSE 和 CEA 時,AUC 值為 0.778。FENDRR的AUC值為 0.812,敏感性和特異性分別為90.7%和75%;H19的AUC值為0.833,敏感性和特異性分別為74.4%和85%;而當聯(lián)合檢測FENDRR和H19時,AUC值為0.894。[結論]FENDRR、H19在宣威肺癌的發(fā)生、發(fā)展中發(fā)揮著重要作用,它們可以作為宣威肺癌潛在的診斷標志物,而且診斷價值優(yōu)于傳統(tǒng)標志物。
[Abstract]:[objective] to investigate the expression of FENDRR,H19 gene in the plasma of Xuanwei lung cancer, non-Xuanwei lung cancer, Xuanwei lung benign venereal disease and healthy control group, and to study the change trend and clinical significance of FENDRR,H19 in the plasma of Xuanwei lung cancer patients.
