【摘要】：目的探討經(jīng)支氣管鏡氬氣刀(APC)聯(lián)合冷凍介入治療氣道罕見良性腫瘤的價(jià)值。方法回顧性分析2014年5月-2015年12月4例該科經(jīng)支氣管鏡下APC聯(lián)合冷凍治療的氣道罕少良性腫瘤臨床特征及文獻(xiàn)復(fù)習(xí)。結(jié)果男3例,女1例,發(fā)病年齡23~80歲,平均56歲。4例患者分別診斷為:左主支氣管平滑肌瘤、氣管多形性腺瘤、氣管嗜酸性細(xì)胞腺瘤、右肺上葉開口脂肪瘤。麻醉方式以局麻為主,氣管多形性腺瘤首次治療在全麻喉罩通氣下完成。介入方式:APC熱消融、止血,二氧化碳CO2冷凍凍取瘤組織及術(shù)后凍融治療,術(shù)后1周清除凍融后壞死組織。治療時(shí)間平均20 min。4例患者共進(jìn)行10次介入治療,術(shù)中未出現(xiàn)大出血、氣道著火及穿孔。4例患者隨訪至今局部無復(fù)發(fā)。結(jié)論 APC聯(lián)合冷凍治療氣道罕少良性腫瘤安全有效,值得推廣。
[Abstract]:Objective to evaluate the value of transbronchial argon knife (APC) combined with cryotherapy in the treatment of rare benign airway tumors. Methods from May 2014 to December 2015, the clinical features and literature review of 4 cases of airway rare benign tumors treated with APC combined with cryotherapy under bronchoscope were retrospectively analyzed. Results there were 3 males and 1 female with an average age of 56. 4 patients were diagnosed as left main bronchial leiomyoma, tracheal pleomorphic adenoma, eosinophil adenoma of trachea, lipoma of right superior lobe orifice. The first treatment of tracheal pleomorphic adenoma was performed under general anesthesia and laryngeal mask ventilation. Interventional methods: APC thermal ablation, hemostasis, carbon dioxide CO2 cryopreservation of tumor tissue and postoperative freeze-thaw treatment, 1 week after the removal of frozen and thawed necrotic tissue. The average time of treatment was 20 min.4. Ten times of interventional therapy were performed in the patients. There was no massive hemorrhage, airway fire and perforation in 4 cases. There was no local recurrence in 4 cases. Conclusion APC combined with cryotherapy is safe and effective in the treatment of rare and rare airway benign tumors, and it is worth popularizing.
