絕經(jīng)前后中國(guó)女性BMI與乳腺癌發(fā)病關(guān)系的劑量-反應(yīng)meta分析

【摘要】:目的探討不同絕經(jīng)狀態(tài)下中國(guó)女性BMI與乳腺癌發(fā)病的關(guān)系。方法計(jì)算機(jī)檢索Pub Med、EMBASE、萬(wàn)方數(shù)據(jù)庫(kù)、維普數(shù)據(jù)庫(kù)、相關(guān)期刊論文(CNKI),收集國(guó)內(nèi)外2000年1月至2017年2月公開(kāi)發(fā)表的關(guān)于中國(guó)女性不同體質(zhì)指數(shù)(BMI)分組與乳腺癌發(fā)病關(guān)系的研究文獻(xiàn)。應(yīng)用Stata12.0進(jìn)行Meta分析,在總體效應(yīng)關(guān)系存在的基礎(chǔ)上,進(jìn)行線性和非線性模型擬合,若未發(fā)現(xiàn)劑量反應(yīng)關(guān)系,再進(jìn)一步探討不同BMI對(duì)乳腺癌發(fā)病的截?cái)嘈?yīng),采用Begg's檢驗(yàn)評(píng)估發(fā)表偏倚。結(jié)果共納入8篇文獻(xiàn),共103 141人。絕經(jīng)前組合并后的效應(yīng)量RR值為1.10(95%CI:0.95~1.27),絕經(jīng)后組合并后的效應(yīng)量RR值為1.52(95%CI:1.31~1.75),絕經(jīng)前、后均未發(fā)現(xiàn)劑量-反應(yīng)關(guān)系(X~2=1.95,PLinearity=0.16;=2.02,PNonlinearity=0.57)。相對(duì)于參照組(18.5 kg/m~2BMI≤24kg/m~2),絕經(jīng)后超重人群(24kg/m~2BMI≤28kg/m~2)其乳腺癌發(fā)病風(fēng)險(xiǎn)RR值為1.33(95%CI:1.10~1.60),肥胖人群(BMI28 kg/m~2)為1.83(95%CI:1.46~2.29)。Begg's檢驗(yàn)的Z=0.82,P=0.41,所納入分析的文獻(xiàn)不存在發(fā)表偏倚。結(jié)論在中國(guó),BMI升高可增加絕經(jīng)后女性乳腺癌的發(fā)病風(fēng)險(xiǎn),而在絕經(jīng)前女性人群中尚未發(fā)現(xiàn)此關(guān)聯(lián)。
[Abstract]:Objective to investigate the relationship between BMI and breast cancer in Chinese women with different menopausal status. Methods Pub Med,EMBASE, Wanfang database, Weipu database and (CNKI), database were searched by computer. From January 2000 to February 2017, the literature on the relationship between (BMI) grouping and breast cancer incidence in Chinese women was collected from January 2000 to February 2017.
