【摘要】：目的:探討擴散加權(quán)成像和表觀系數(shù)擴散(ADC)值在鑒別間變型管膜瘤與非間變型室管膜瘤中的價值。方法:回顧性分析我院經(jīng)手術(shù)病理證實的52例室管膜瘤患者的病例資料,其中非間變型室管膜瘤27例,間變型室管膜瘤25例。測量腫瘤實質(zhì)部分的最小、平均及最大ADC值,并采用ROC曲線分析其診斷效能。結(jié)果:間變型室管膜瘤的最小ADC值[(0.808±0.190)×10~(-3)mm~2/s]低于非間變型室管膜瘤[(1.093±0.365)×10~(-3)mm~2/s],差異有統(tǒng)計學意義(P0.05);鑒別兩種腫瘤的最佳最小ADC值為0.887×10~(-3)mm~2/s,診斷敏感度為78.5%、特異度為73.2%。兩種類型腫瘤的平均ADC值和最大ADC值的差異無統(tǒng)計學意義(P0.05)。結(jié)論:最小ADC值有助于間變型室管膜瘤與非間變型室管膜瘤的鑒別診斷。
[Abstract]:Objective: to evaluate the value of diffusion weighted imaging (DWI) and apparent coefficient diffusion (ADC) in differential diagnosis of intervariant and non-intervariant ependymoma. Methods: the data of 52 cases of ependymoma confirmed by operation and pathology in our hospital were retrospectively analyzed. Among them, 27 cases were non-anaplastic ependymoma and 25 cases were anaplastic ependymoma. The minimum, average and maximum ADC values of the tumor parenchyma were measured, and the diagnostic efficacy was analyzed by ROC curve. Results: the minimum ADC value of stromal ependymoma [(0.808 鹵0.190) 脳 10 ~ (-3) mm~2/s] was significantly lower than that of non-intervariant ependymoma [(1.093 鹵0.365) 脳 10 ~ (-3) mm~2/s] (P0.05). The best minimum ADC value for differentiating two kinds of tumors was 0.887 脳 10 ~ (-3) mm~2/s,. The diagnostic sensitivity was 78.5% and the specificity was 73.2%. There was no significant difference between the average ADC value and the maximum ADC value of the two types of tumors (P0.05). Conclusion: the minimum ADC value is helpful to the differential diagnosis of intermetamorphic ependymoma and non-anaplastic ependymoma.
