CD147表達與結(jié)直腸癌臨床病理因素及預(yù)后研究

【摘要】：目的本研究旨在探討CD147與結(jié)直腸癌患者術(shù)后臨床病理因素及預(yù)后的相關(guān)性,說明CD147是否能夠成為指導(dǎo)臨床有效指標(biāo)。方法回顧性分析蚌埠醫(yī)學(xué)院附屬泰興醫(yī)院普通外科2012年1月至2013年12月收治入院手術(shù)的124例結(jié)直腸癌患者的臨床資料,采用免疫組化的方法分析CD147表達與患者臨床病理資料、術(shù)后應(yīng)激及恢復(fù)情況以及預(yù)后之間關(guān)系。結(jié)果根據(jù)CD147表達情況將結(jié)果分為陰性和陽性,陽性中分為弱陽性、中度陽性、強陽性。免疫組化結(jié)果顯示CD147表達陰性46例,表達陽性78例,其中弱陽性35例,中度陽性26例,強陽性17例,在結(jié)直腸癌患者中CD147陽性率62.9%,高于正常組織中的23.4%,差異有統(tǒng)計學(xué)意義(P0.01)。CD147在結(jié)直腸癌中表達與患者性別、年齡、腫瘤位置、腫瘤大小、分化程度間差異均無統(tǒng)計學(xué)意義(P0.05),而與腫瘤的淋巴結(jié)轉(zhuǎn)移、遠處轉(zhuǎn)移、浸潤深度、TMN分期之間差異有統(tǒng)計學(xué)意義(P0.05)。單因素方差分析結(jié)果顯示CD147表達與術(shù)后白細胞計數(shù)、中性粒細胞計數(shù)、血漿白蛋白、通氣時間、住院天數(shù)之間差異無統(tǒng)計學(xué)意義(P0.05)。Kaplan-Meier分析顯示CD147表達與患者的生存率成負相關(guān),患者腫瘤細胞中CD147表達越高生存期越短,CD147表達與患者術(shù)后短期生存率差異有顯著統(tǒng)計學(xué)意義(P0.01)。結(jié)論結(jié)直腸癌組織中CD147表達增高,與結(jié)直腸癌的轉(zhuǎn)移有關(guān),表達越高的患者術(shù)后預(yù)后越差,可以成為結(jié)直腸癌預(yù)后的監(jiān)測指標(biāo)。
[Abstract]:Objective to investigate the correlation between CD147 and postoperative clinicopathological factors and prognosis in colorectal cancer patients, and to see whether CD147 can be used as an effective index for clinical application. Methods the clinical data of 124 patients with colorectal cancer treated in general surgery department of Taixing Hospital affiliated to Bengbu Medical College from January 2012 to December 2013 were analyzed retrospectively. The expression of CD147 and the clinicopathological data of the patients were analyzed by immunohistochemical method. The relationship between stress, recovery and prognosis after operation. Results according to the expression of CD147, the results were classified as negative and positive. The positive was divided into weak positive, moderate positive and strong positive. Immunohistochemical results showed that CD147 was negative in 46 cases and positive in 78 cases, including weak positive 35 cases, moderate positive 26 cases and strong positive 17 cases. The positive rate of CD147 in colorectal cancer was 62.9%, which was higher than that in normal tissues (23.4%). There was no significant difference between CD147 expression and sex, age, tumor location, tumor size and differentiation degree in colorectal cancer (P0.05), but there was no significant difference between CD147 expression and lymph node metastasis. There were significant differences in distant metastasis, depth of invasion and TMN stage (P0.05). Univariate analysis of variance showed that CD147 expression was associated with postoperative leukocyte count, neutrophil count, plasma albumin, and ventilation time. Kaplan-Meier analysis showed that the expression of CD147 was negatively correlated with the survival rate of the patients, and the higher the expression of CD147 was, the shorter the survival time was. The expression of CD147 was significantly different from the short-term survival rate (P 0. 01). Conclusion the increased expression of CD147 in colorectal carcinoma is related to the metastasis of colorectal cancer. The higher the expression of CD147 is, the worse the postoperative prognosis is, and the higher the expression is, the better the prognosis of colorectal cancer is.
