Ras相關(guān)區(qū)域家族10在胃癌中的表達及與胃癌患者預后相關(guān)的臨床病理學研究

【摘要】:背景:Ras相關(guān)區(qū)域家族10(RASSF10)已經(jīng)被發(fā)現(xiàn)在多種惡性腫瘤中作為抑癌基因而異常表達,并且表達異常的RASSF10與腫瘤的惡性程度及其患者的臨床轉(zhuǎn)歸及預后密切相關(guān)。在胃癌中,RASSF10的表達情況和作用機制尚不清楚。目的:本研究旨在探討RASSF10基因在人類胃癌中的表達及與胃癌患者預后相關(guān)的臨床病理學參數(shù)的關(guān)系,為胃癌的臨床診治提供客觀依據(jù)。方法:選取2008年1月至2010年12月在第二軍醫(yī)大學附屬長征醫(yī)院胃癌手術(shù)標本共105例。所有患者臨床隨訪時間至少五年或隨訪至死亡。另取我院2015年3月至2015年6月胃癌組織標本及相對應癌旁組織標本各15例,于手術(shù)切除后切取一定大小立刻在液氮中進行速凍,并保持在-80℃冰箱中行Western blot和q RT-PCR分析。應用Western blot、q RT-PCR、免疫組織化學等方法檢測RASSF10在胃癌組織和相應癌旁正常組織中的表達情況,并分析其在胃癌不同TNM分期中的表達特點,及其與臨床病理學參數(shù)和預后的關(guān)系。在此基礎上,應用Western blot、q RT-PCR的方法檢測多株胃癌細胞株和永生化胃上皮細胞GES1細胞中RASSF10的表達情況。利用去甲基化試劑5-雜氮-2'-脫氧胞苷(5-aza-2'-deoxycytidine,5-Aza-d C)處理所選取的與GES1細胞相比表達差異最明顯的BGC-823細胞系,通過細胞劃痕實驗來檢測細胞的遷移能力,通過平板克隆形成實驗來檢測胃癌細胞的增殖能力和活力。最后,運用SPSS 21.0統(tǒng)計軟件進行統(tǒng)計學分析,計量資料采用t檢驗,計數(shù)資料采用χ~2檢驗,以P0.05為差異有統(tǒng)計學意義。結(jié)果:在15例冰凍胃癌組織和相對應癌旁組織中,qRT-PCR結(jié)果顯示RASSF10在胃癌組織中的表達與相匹配的癌旁正常組織相比顯著下降。Western blot實驗結(jié)果進一步證實了qRT-PCR的結(jié)果,RASSF10在胃癌組織中的表達水平的確低于相配對的癌旁正常組織。并且隨著臨床TNM分期的進展,Ⅲ、Ⅳ期胃癌患者中該基因表達水平與Ⅰ、Ⅱ期胃癌患者相比,呈現(xiàn)逐漸下降趨勢。免疫組織化學染色結(jié)果提示在所選取的105例胃癌組織中RASSF10陽性表達率為38.1%(40/105),陰性表達率為61.9%(65/105),在所選取的癌旁正常組織中RASSF10的陽性表達率為61.9%(65/105),陰性表達率為38.1%(40/105),兩者差別具有統(tǒng)計學意義(p0.05)。進一步統(tǒng)計學分析顯示,RASSF10表達水平與腫瘤大小、腫瘤分型、分化程度、浸潤深度、淋巴結(jié)轉(zhuǎn)移程度有關(guān),差異均有統(tǒng)計學意義(所有p0.05);RASSF10的表達水平與患者年齡、性別、腫瘤部位等差別無明顯統(tǒng)計學意義(所有p0.05)。通過Kaplan-Meier生存分析顯示RASSF10表達陰性患者有著更差的預后(p=0.008)。Cox比例風險回歸模型分析顯示:RASSF10表達(HR=0.528,95%CI:0.286-0.975,p=0.041)與AJCC臨床分期(HR=4.994,95%CI:1.998-12.486,p=0.001)可作為判斷胃癌預后的獨立的預后因子。體外細胞實驗結(jié)果顯示,RASSF10在多株胃癌細胞系中的表達水平均低于正常胃上皮細胞GES1細胞。5-Aza-d C處理BGC-823細胞后,RASSF10的表達水平升高。細胞平板克隆實驗結(jié)果提示,去甲基化試劑處理組細胞的克隆數(shù)較未處理組克隆數(shù)明顯減少。細胞劃痕實驗結(jié)果提示處理組的細胞從周邊遷移到中央的細胞數(shù)明顯減少,遷移距離明顯縮短。結(jié)論:RASSF10蛋白在胃癌組織中較相應癌旁組織表達明顯下調(diào),并且隨著臨床分期及病理分期的進展,RASSF10的表達水平下調(diào)更加明顯。統(tǒng)計學分析顯示,其表達水平的高低與胃癌臨床病理學參數(shù)密切相關(guān),且其在胃癌中表達下降可預測不良預后。這提示我們RASSF10或許可以作為胃癌診斷與預后的潛在生物標記物。在體外機制研究中,我們發(fā)現(xiàn)RASSF10在胃癌細胞系中的表達水平較GES1細胞表達下降,BGC-823細胞系中下降更加明顯。去甲基化試劑處理后,RASSF10表達增多,并可明顯減少胃癌細胞的克隆形成數(shù)以及向劃痕中央遷移的距離,降低胃癌細胞的增殖能力和活力,并能夠顯著降低胃癌細胞的遷移能力。
