【摘要】：目的探討中國女性乳腺癌患者術(shù)后復(fù)發(fā)的影響因素,為改善乳腺癌患者預(yù)后提供參考依據(jù)。方法檢索中國學(xué)術(shù)文獻(xiàn)總庫(CNKI)、萬方數(shù)據(jù)庫、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(CBM)、Web Of Science數(shù)據(jù)庫、Pubmed數(shù)據(jù)庫、Science Direct數(shù)據(jù)庫和BIOSIS數(shù)據(jù)庫,并輔以手工檢索和文獻(xiàn)追溯法收集2000年1月1日—2013年12月31日公開發(fā)表的關(guān)于中國女性乳腺癌患者術(shù)后復(fù)發(fā)影響因素的相關(guān)文獻(xiàn);應(yīng)用Rev Man 5.2和SAS 9.2軟件對納入的文獻(xiàn)進(jìn)行meta分析。結(jié)果最終納入25篇中文文獻(xiàn),共累計女性乳腺癌患者16 471例,其中術(shù)后復(fù)發(fā)患者1 048例,復(fù)發(fā)率為6.36%;meta分析結(jié)果顯示,腫瘤直徑2 cm(OR=2.89,95%CI=1.76~4.74)、腫瘤分期Ⅱ期(OR=3.79,95%CI=3.08~4.66)、原發(fā)灶T分期T2期(OR=3.78,95%CI=2.47~5.78)、有淋巴結(jié)轉(zhuǎn)移(OR=2.68,95%CI=1.67~4.31)、淋巴結(jié)轉(zhuǎn)移數(shù)目3個(OR=3.17,95%CI=2.49~4.03)和人表皮生長因子2陽性(OR=2.85,95%CI=2.21~3.67)為中國女性乳腺癌患者術(shù)后復(fù)發(fā)的危險因素,雌激素受體/孕激素受體陽性(OR=0.47,95%CI=0.38~0.59)和術(shù)后放療(OR=0.21,95%CI=0.12~0.36)為中國女性乳腺癌患者術(shù)后復(fù)發(fā)的保護(hù)因素;敏感性分析和發(fā)表偏倚評估結(jié)果顯示,納入的文獻(xiàn)不存在發(fā)表偏倚,結(jié)果較為可靠。結(jié)論腫瘤直徑、腫瘤分期、淋巴結(jié)轉(zhuǎn)移狀態(tài)、淋巴結(jié)轉(zhuǎn)移數(shù)目、原發(fā)灶T分期、人表皮生長因子2狀態(tài)、雌激素受體/孕激素受體狀態(tài)和術(shù)后放療情況是中國女性乳腺癌患者術(shù)后復(fù)發(fā)的主要影響因素。
[Abstract]:Objective to investigate the influencing factors of postoperative recurrence of female breast cancer patients in China, and to provide reference for improving the prognosis of breast cancer patients. Methods Chinese academic literature database (CNKI), Wanfang database, Chinese biomedical literature database (CBM) web Of Science database, (CNKI), Direct database and BIOSIS database were searched. The related literatures on the factors influencing the recurrence of female breast cancer patients in China from January 1, 2000 to December 31, 2013 were collected by manual retrieval and literature tracing.
