胰腺癌中低氧誘導因子-1α和軸突誘導因子4D的表達及其臨床意義

【摘要】:目的:研究低氧誘導因子-1α(hypoxia inducible factor-1α,HIF-1α)和軸突誘導因子4D(Semaphorin 4D,Sema4D)在胰腺癌組織中的表達,探討兩者之間的相關性及其臨床意義。方法:采用免疫組織化學(immunohistochemistry,SP)法檢測天津醫(yī)科大學附屬腫瘤醫(yī)院和邯鄲市中心醫(yī)院82例胰腺癌組織和相應癌旁組織中HIF-1α和Sema4D的表達,采用Chi-square檢驗分析HIF-1α和Sema4D在不同組織中的表達差異,采用Chi-square檢驗分析HIF-1α和Sema4D與患者的性別、年齡、分化程度、腫瘤大小、胰腺癌分期、淋巴結(jié)轉(zhuǎn)移、神經(jīng)受侵、脈管內(nèi)瘤栓等的相關性。HIF-1α和Sema4D在胰腺癌組織中的表達的相關性采用Mc Nemar檢驗分析其相關性。HIF-1α和Sema4D對胰腺癌患者生存率的影響分析采用Kaplan-meier法,生存曲線的比較采用Log-rank檢驗。Cox回歸模型多因素分析方法,分析HIF-1α和Sema4D是否胰腺癌獨立預后因素。結(jié)果:HIF-1α和Sema4D的陽性表達主要在細胞質(zhì)內(nèi),呈棕黃色顆粒。HIF-1α和Sema4D在胰腺癌組織中的陽性表達率分別為52.4%(43/82)和51.2%(42/82),顯著高于兩者在癌旁組織的表達,差異有統(tǒng)計學意義(P0.05)。HIF-1α的異常表達與胰腺癌分期(χ~2=11.398,P=0.000)、淋巴結(jié)轉(zhuǎn)移(χ~2=20.341,P=0.000)、神經(jīng)受侵(χ~2=23.478,P=0.000)、脈管內(nèi)瘤栓(χ~2=8.281,P=0.000)相關,Sema4D的異常表達與胰腺癌分期(χ~2=7.212,P=0.007)、淋巴結(jié)轉(zhuǎn)移(χ~2=60129,P=0.013)、神經(jīng)受侵(χ~2=25.794,P=0.000)、脈管內(nèi)瘤栓(χ~2=6.678,P=0.010)相關。HIF-1α和Sema4D的表達之間存在正相關性(κ=0.438,P=0.000)。單因素生存分析HIF-1α(χ~2=12.098,P=0.001)、Sema4D(χ~2=9.644,P=0.002)是影響胰腺癌的預后因素,Cox分析顯示2者均不是影響胰腺癌的獨立預后因素。結(jié)論:胰腺癌組織中存在HIF-1α和Sema4D表達的上調(diào),兩者具有明顯相關性,與胰腺癌淋巴結(jié)轉(zhuǎn)移、神經(jīng)受侵、脈管內(nèi)瘤栓有關,HIF-1α、Sema4D的表達可能提示胰腺癌患者分期較晚和預后不良。
