【摘要】：目的:通過檢測原發(fā)性肝癌患者血清AFP mRNA、h-TERT mRNA、CD44v6mRNA和MAGE mRNA表達(dá)情況,研究聯(lián)合檢測四種標(biāo)志物在預(yù)測腫瘤術(shù)后復(fù)發(fā)轉(zhuǎn)移中的早期預(yù)測價(jià)值。方法:采集40例PHC患者、20例活動(dòng)性肝炎(HBV)患者及20例健康成人外周血,通過RT-PCR技術(shù)檢測血清AFP mRNA、h TERT mRNA、CD44v6mRNA、MAGE-1 mRNA表達(dá)情況。結(jié)果:40例PHC患者血清AFP mRNA、h-TERT mRNA、CD44v6 mRNA、MAGE-1 mRNA陽性表達(dá)率分別為50%(20/40)、62.5%(25/40)、55%(22/40)、37.5%(15/40),90%的患者至少表達(dá)一種標(biāo)志物。ROC分析顯示AFP/h-TERT/CD44v6/MAGE-1 mRNA均陽性表達(dá)較AFP mRNA、h-TERT mRNA、CD44v6 mRNA、MAGE-1 mRNA中任一種標(biāo)志物具有更大的曲線下面積(AUC=0.766),特異性(96.2%),陽性預(yù)測值(88.9%),陰性預(yù)測值(71.4%)。生存分析表明臨床分期、h-TERT mRNA、CD44v6 mRNA、AFP/hTERT/CD44v6/MAGE-1 mRNA與患者術(shù)后復(fù)發(fā)轉(zhuǎn)移相關(guān),進(jìn)一步logistic回歸分析表明,臨床分期及AFP/h-TERT/CD44v6/MAGE-1均陽性表達(dá)是原發(fā)性肝癌患者術(shù)后復(fù)發(fā)的獨(dú)立預(yù)測因素(P0.05)。結(jié)論:聯(lián)合檢測AFP mRNA、h-TERT mRNA、CD44v6 mRNA和MAGE mRNA表達(dá)提高了術(shù)后早期預(yù)測PHC患者復(fù)發(fā)的準(zhǔn)確性,可能具有一定的臨床應(yīng)用價(jià)值。
[Abstract]:Objective: to study the early predictive value of combined detection of four markers in predicting recurrence and metastasis of primary liver cancer by detecting the expression of serum AFP mRNA,h-TERT mRNA,CD44v6mRNA and MAGE mRNA in patients with primary liver cancer. Methods: peripheral blood samples were collected from 40 patients with PHC and 20 patients with active hepatitis (HBV) and 20 healthy adults. The expression of serum AFP mRNA,h TERT mRNA,CD44v6mRNA,MAGE-1 mRNA was detected by RT-PCR technique. Results the positive expression rate of serum AFP mRNA,h-TERT mRNA,CD44v6 mRNA,MAGE-1 mRNA in 40 PHC patients was 50% (20 / 40), 62.5% (25 / 40) or 55% (22 / 40) or 90% (15 / 40), respectively. (AUC=0.766), specificity (96.2%), positive predictive value (88.9%), negative predictive value (71.4%). Survival analysis showed that the clinical staging of h-TERT mRNA,CD44v6 mRNA,AFP/hTERT/CD44v6/MAGE-1 mRNA was associated with postoperative recurrence and metastasis. Further logistic regression analysis showed that the positive expression of AFP/h-TERT/CD44v6/MAGE-1 and clinical staging were independent predictors of postoperative recurrence in patients with primary liver cancer (P0.05). Conclusion: combined detection of AFP mRNA,h-TERT mRNA,CD44v6 mRNA and MAGE mRNA can improve the accuracy of early prediction of recurrence of PHC patients, and may have some clinical value.
