食管癌患者放療期間營養(yǎng)不良預(yù)測因素分析

【摘要】:目的通過分析食管癌患者放療期間營養(yǎng)狀況,篩選營養(yǎng)不良的早期預(yù)測因子。方法采集120名初診的食管癌患者放療前后的體質(zhì)量及就診時的基本資料,分析影響其營養(yǎng)狀況的相關(guān)因素。結(jié)果就診時已有18例(15%)患者丟失≥10%的體質(zhì)量,69例(57.5%)患者丟失≥5%的體質(zhì)量;放療期間,共有81.7%(98/120)的患者出現(xiàn)體質(zhì)量減輕,其中體質(zhì)量丟失≥5%的占70.0%(84/120),體質(zhì)量減輕的范圍為0.8~8.7 kg,平均值為5.4 kg。逐步回歸發(fā)現(xiàn),整體健康狀況、體質(zhì)量指數(shù)(BMI)、吞咽困難、疼痛、梗阻、卡氏評分(KPS)、病程、疲倦與放療期間體質(zhì)量減輕密切相關(guān)。結(jié)論食管癌患者普遍存在營養(yǎng)不良。整體健康狀況、BMI、吞咽困難、疼痛、梗阻、病程、KPS及疲倦8個因素可以預(yù)測食管癌放療期間營養(yǎng)不良。
