【摘要】：目的:在我國(guó)所有惡性腫瘤中對(duì)于胃癌的發(fā)生及發(fā)展仍是當(dāng)前的研究重點(diǎn),本文通過(guò)免疫組化方法檢測(cè)E-cadherin及Ki-67在胃癌組織中的表達(dá)來(lái)研究其與胃癌發(fā)生進(jìn)展的相關(guān)性,以期進(jìn)一步討論胃癌的發(fā)展及轉(zhuǎn)移的分子機(jī)制以及為胃癌的基因靶向治療提供理論依據(jù)。方法:收集南昌大學(xué)第四附屬醫(yī)院自2011年1月至2013年12月行手術(shù)切除并行病理檢查確診胃癌標(biāo)本62例,并按胃癌標(biāo)本分為以下幾組,性別組、年齡組、腫瘤直徑組、腫瘤部位組、分化程度組、浸潤(rùn)深度組、淋巴結(jié)轉(zhuǎn)移組、遠(yuǎn)處轉(zhuǎn)移組、TNM分期組,利用免疫組化法(MaxVision)檢測(cè)胃癌組織標(biāo)本中的E-cadherin及Ki-67表達(dá)的陽(yáng)性率及陰性率以及20例正常胃組織,并用SPSS22.0分析他們與各組的相關(guān)性并對(duì)實(shí)驗(yàn)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,以及E-cadherin和Ki-67的關(guān)系。結(jié)果:1、在62例胃癌組織中E-cadherin及Ki-67的陽(yáng)性率分別為48.4%(30/62),61.3%(38/62)。2、E-cadherin在正常胃組織中的陽(yáng)性率100%,高于胃癌組中的表達(dá),差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。E-cadherin在高分化及中分化胃癌組中的陽(yáng)性率81.3%(13/16)高于低分化胃癌組中的陽(yáng)性率37%(17/46),差異顯著具有統(tǒng)計(jì)學(xué)意義(P0.05),此外E-cadherin在胃癌浸潤(rùn)深度中T1+T2組中的陽(yáng)性率66.7%(16/24)高于T3+T4組中的陽(yáng)性率36.8%(14/38),在淋巴結(jié)轉(zhuǎn)移中無(wú)淋巴結(jié)轉(zhuǎn)移組中的陽(yáng)性率65.2%(15/23)高于有淋巴結(jié)轉(zhuǎn)移組中的陽(yáng)性率38.5%(15/39),在遠(yuǎn)處轉(zhuǎn)移中無(wú)遠(yuǎn)處轉(zhuǎn)移組中的陽(yáng)性率59.6%(28/47)高于有遠(yuǎn)處轉(zhuǎn)移組中的陽(yáng)性率13.3%(2/15),在TNM分期中Ⅰ+Ⅱ期組中的陽(yáng)性率65.6%(21/32)高于Ⅲ+Ⅳ組中的陽(yáng)性率30.0%(9/30),以上P值均0.05;而E-cadherin與胃癌患者的性別組、年齡組及腫瘤直徑組和腫瘤部位組無(wú)關(guān)(P0.05)。3、Ki-67在正常胃組織中的陽(yáng)性率為5%,低于胃癌組,差異具有顯著性(P0.05)。Ki-67在胃癌中的表達(dá)顯示其在胃癌浸潤(rùn)深度中T1+T2組中的陽(yáng)性率41.7%(10/24)低于T3+T4組中的陽(yáng)性率73.7%(28/38),在淋巴結(jié)轉(zhuǎn)移中有淋巴結(jié)轉(zhuǎn)移組中的陽(yáng)性率74.4%(29/39)高于無(wú)淋巴結(jié)轉(zhuǎn)移組中的陽(yáng)性率39.1%(9/23),在TNM分期中Ⅲ+Ⅳ期組中的陽(yáng)性率80.0%(24/30)高于Ⅰ+Ⅱ期組中的陽(yáng)性率43.8%(14/32),以上P值0.05;而Ki-67與胃癌患者的性別組、年齡組、腫瘤部位組、腫瘤直徑組、分化程度組、遠(yuǎn)處轉(zhuǎn)移組并未發(fā)現(xiàn)相關(guān)性(P0.05)。4、在胃癌組織標(biāo)本中E-cadherin以及Ki-67的表達(dá)的陽(yáng)性率,經(jīng)過(guò)SPSS22.0的Spearman秩相關(guān)進(jìn)行相關(guān)性分析,結(jié)果提示:E-cadherin與Ki-67的表達(dá)呈負(fù)相關(guān),其中rs值為-0.291,而P值0.05。結(jié)論:1、E-cadherin在胃癌中的表達(dá)與胃癌的浸潤(rùn)深度、淋巴轉(zhuǎn)移、遠(yuǎn)處轉(zhuǎn)移、分化程度及TNM分期呈負(fù)相關(guān),提示其在胃癌中表達(dá)下降容易致使胃癌細(xì)胞具有侵襲力,發(fā)生更多的淋巴轉(zhuǎn)移及遠(yuǎn)處轉(zhuǎn)移。2、Ki-67在胃癌中的陽(yáng)性率表達(dá)提示Ki-67與胃癌浸潤(rùn)深度、淋巴結(jié)轉(zhuǎn)移、TNM分期呈正相關(guān),也表明了Ki-67在胃癌中的陽(yáng)性表達(dá)與胃癌的惡性程度及轉(zhuǎn)移密切相關(guān)。3、在胃癌中E-cadherin與Ki-67的表達(dá)呈負(fù)相關(guān)。
[Abstract]:Objective: in order to study the relationship between the expression of E-cadherin and Ki-67 in gastric cancer tissues by immunohistochemical method, the occurrence and development of gastric cancer are still the focus of the present study in all malignant tumors in China. In order to further discuss the development and metastasis of gastric cancer molecular mechanism and provide a theoretical basis for gene targeted therapy of gastric cancer. Methods: from January 2011 to December 2013, 62 cases of gastric cancer were collected from the fourth affiliated Hospital of Nanchang University. The specimens were divided into the following groups: sex group, age group, tumor diameter group. The positive and negative rates of E-cadherin and Ki-67 expression in gastric cancer tissues were detected by immunohistochemical (MaxVision) method, including tumor location group, differentiation group, depth of invasion group, lymph node metastasis group, distant metastasis group, and 20 normal gastric tissues. SPSS22.0 was used to analyze their correlation with each group, and the experimental data were statistically analyzed, as well as the relationship between E-cadherin and Ki-67. Results the positive rates of E-cadherin and Ki-67 in 62 cases of gastric cancer were 48.4% (30 / 62), 61.3% (38 / 62) .2the positive rate of E-cadherin in normal gastric tissue was 100%, which was higher than that in gastric cancer group. The positive rate of E-cadherin in well-differentiated and moderately differentiated gastric cancer group was 81.3% (13 / 16), which was significantly higher than that in poorly differentiated gastric carcinoma group (17 / 46) (P0.05). In addition, the positive rate of E-cadherin in T1 / T2 group was significantly higher than that in poorly differentiated gastric carcinoma group (P0.05). The positive rate of sex was 66.7% (16 / 24) higher than that of T3 T4 group (36.8%, 14 / 38), the positive rate of lymph node metastasis group was 65.2% (15 / 23) higher than that of lymph node metastasis group (38.5%, 1539%), the positive rate of distant metastasis group was 59.6% (28 / 47) higher than that of non-distant metastasis group (28 / 47).
