【摘要】：目的:本研究旨在探討與垂體腺瘤術(shù)后復(fù)發(fā)相關(guān)影響因素,從而對垂體腺瘤術(shù)后復(fù)發(fā)的防治提供的參考,以減少術(shù)后復(fù)發(fā)。研究方法:回顧性分析自2010年1月至2016年1月寧夏醫(yī)科大學(xué)總醫(yī)院神經(jīng)外科收治的99例經(jīng)鼻蝶竇入路手術(shù)治療的垂體腺瘤患者臨床資料,收集其性別、年齡、臨床癥狀、腫瘤的大小、侵襲性、垂體卒中、手術(shù)全切情況、術(shù)后殘余、Ki-67等臨床資料。結(jié)果:本組研究中復(fù)發(fā)26例,復(fù)發(fā)率為26.26%。對影響復(fù)發(fā)的11項指標(biāo)進(jìn)行單因素分析得出腫瘤的侵襲性、大小、全切情況及術(shù)后殘留這四項指標(biāo)是影響垂體腺瘤術(shù)后復(fù)發(fā)的相關(guān)因素(P0.05)。再將這四項指標(biāo)進(jìn)行Logistic回歸分析得出腫瘤侵襲性、術(shù)后殘留為垂體腺瘤術(shù)后復(fù)發(fā)的獨立危險因素。同時,垂體腺瘤的侵襲性以及術(shù)后殘留β值0,說明這兩種是促進(jìn)垂體腺瘤術(shù)后復(fù)發(fā)的因素,而腫瘤存在侵襲性則是經(jīng)影響患者術(shù)后復(fù)發(fā)的主要風(fēng)險因素。結(jié)論:多種因素的綜合作用導(dǎo)致了垂體腺瘤的術(shù)后復(fù)發(fā),腫瘤侵襲性、術(shù)后殘留與術(shù)后復(fù)發(fā)明確相關(guān)。了解腫瘤性質(zhì),術(shù)中盡可能提高腫瘤切除率、減少術(shù)后殘余,可以降低垂腺瘤術(shù)后復(fù)發(fā)率。
[Abstract]:Objective: to investigate the factors related to postoperative recurrence of pituitary adenomas, and to provide references for the prevention and treatment of postoperative recurrence of pituitary adenomas. Methods: the clinical data of 99 patients with pituitary adenoma treated by transsphenoidal approach from January 2010 to January 2016 in Neurosurgery Department of General Hospital of Ningxia Medical University were retrospectively analyzed, and their sex, age, clinical symptoms were collected. Tumor size, invasion, pituitary apoplexy, total resection, postoperative residual Ki-67 and other clinical data. Results: there were 26 cases of recurrence in this study, and the recurrence rate was 26.26%. The single factor analysis showed that the invasiveness, size, total resection and postoperative residue of the tumor were the related factors of postoperative recurrence of pituitary adenoma (P0.05). The Logistic regression analysis showed that the tumor invasiveness was the independent risk factor of postoperative recurrence of pituitary adenoma. At the same time, the invasiveness of pituitary adenoma and the residual 尾 value of pituitary adenoma were 0, which indicated that these two kinds of factors could promote the recurrence of pituitary adenoma, and the invasiveness of tumor was the main risk factor of postoperative recurrence of pituitary adenoma. Conclusion: the combined effects of various factors lead to postoperative recurrence of pituitary adenoma, tumor invasiveness, postoperative residual and postoperative recurrence. To understand the nature of the tumor, to improve the tumor resection rate and reduce the postoperative residual, can reduce the recurrence rate of adenoma.
