【摘要】：背景:異基因造血干細(xì)胞移植的本質(zhì)是免疫細(xì)胞的移植,其治療作用主要依靠移植物抗白血病效應(yīng),但其伴隨的移植物抗宿主病增加了患者的死亡率,影響療效。目前研究者提出了多種減輕移植物抗宿主病同時增強移植物抗白血病效應(yīng)的方法,但各有優(yōu)缺點及爭議點,故對兩者之間的關(guān)系及分離方法尚需進(jìn)一步深入的研究。目的:綜述異基因造血干細(xì)胞移植后減輕移植物抗宿主病而加強或不降低移植物抗白血病效應(yīng)的研究現(xiàn)狀。方法:由第一作者檢索萬方、CNKI、Pub Med數(shù)據(jù)庫,檢索時間為2000至2016年,英文檢索詞為"hematopoietic stem cell transplantation,graft versus leukemia,graft-versus-host disease,donor lymphocyte infusion,regulatory T cell,natural killer cell,mesenchymal stem cell",查閱近年增強移植物抗白血病效應(yīng)及減低移植物抗宿主病的相關(guān)文獻(xiàn)。結(jié)果與結(jié)論:最終納入42篇文獻(xiàn)。目前有關(guān)移植物抗白血病效應(yīng)和移植物抗宿主病的關(guān)系及各自的發(fā)生機制仍不十分明確,有關(guān)兩者分離方法雖多,但各有優(yōu)缺點及爭議點,尚未有統(tǒng)一的最優(yōu)方案,需要進(jìn)一步研究。
[Abstract]:Background: the essence of allogeneic hematopoietic stem cell transplantation (HSCT) is immune cell transplantation. The therapeutic effect of allogeneic hematopoietic stem cell transplantation mainly depends on the graft versus leukemia effect, but the associated graft versus host disease increases the mortality of patients and affects the curative effect. At present, many methods have been proposed to reduce graft-versus-host disease and enhance graft-versus-leukemia effect, but each has its own advantages and disadvantages and controversial points, so the relationship between the two and the separation methods need to be further studied. Objective: to review the current research status of allogeneic hematopoietic stem cell transplantation (HSCT) to alleviate graft-versus-host disease (GVHD) and to enhance or not reduce GVL effect. Methods: the Med database was searched by the first author from 2000 to 2016. The key word was "hematopoietic stem cell transplantation/ graft versus leukemiagraft-versus-host disease donor lymphocyte infusionated T cell natural killer cell mesenchymal stem cell". The literature on enhancing graft versus leukemia effect and reducing graft versus host disease in recent years was reviewed. Results & conclusion: 42 articles were included. At present, the relationship between graft versus leukemia and graft-versus-host disease and their respective mechanisms are not very clear. Although there are many methods for separating the two, they have their own advantages, disadvantages and controversial points, and there is not yet a unified optimal scheme. Further study is needed.
