cIAP和Caspase-7在子宮內(nèi)膜腺癌中的表達(dá)及意義

【摘要】:目的:研究凋亡抑制蛋白cIAP和凋亡蛋白Caspase-7分別在不同子宮內(nèi)膜組織中的表達(dá),分析二者的表達(dá)與子宮內(nèi)膜腺癌組織臨床分期(FIGO 2009)、病理分級(jí)、淋巴結(jié)轉(zhuǎn)移等臨床生物學(xué)特征的關(guān)系,及二者表達(dá)的相關(guān)性,探討二者在子宮內(nèi)膜組織中表達(dá)的意義。方法:選取河南科技大學(xué)第一附屬醫(yī)院病理科2008~2010年存檔的子宮內(nèi)膜組織標(biāo)本84例,根據(jù)不同組織學(xué)類(lèi)型分為:正常增殖期子宮內(nèi)膜組(12例)、子宮內(nèi)膜不典型增生組(17例)與子宮內(nèi)膜腺癌組(55例)。應(yīng)用免疫組化SP技術(shù)分別分析cIAP蛋白和Caspase-7蛋白在各組中的表達(dá)情況。分別分析cIAP蛋白和Caspase-7蛋白與55例子宮內(nèi)膜腺癌患者的臨床分期、病理分級(jí)、肌層浸潤(rùn)等臨床生物學(xué)特性的關(guān)系。55例子宮內(nèi)膜腺癌組患者均行外科分期手術(shù),包括:I~II期37例,III~IV期18例;共有45例患者行淋巴清掃術(shù),發(fā)現(xiàn)有淋巴結(jié)轉(zhuǎn)移18例。分析cIAP蛋白和Caspase-7蛋白在55例子宮內(nèi)膜腺癌中表達(dá)的相關(guān)性。所選組織標(biāo)本患者均無(wú)其它系統(tǒng)惡性腫瘤,手術(shù)病人術(shù)前均未行放射和化學(xué)治療,所有組織標(biāo)本均由河南科技大學(xué)第一附屬醫(yī)院病理科固定(10%福爾馬林液)、包埋(石蠟),由兩位及以上資深病理專(zhuān)業(yè)醫(yī)師閱片。本實(shí)驗(yàn)數(shù)據(jù)采用SPSS17.0統(tǒng)計(jì)學(xué)軟件,運(yùn)用χ2檢驗(yàn)、Spearman相關(guān)分析進(jìn)行分析,以P0.05為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果:cIAP蛋白和Caspase-7蛋白陽(yáng)性信號(hào)在子宮內(nèi)膜上皮細(xì)胞中定位于細(xì)胞質(zhì),呈棕黃色顆粒。cIAP蛋白的陽(yáng)性表達(dá)分別為:子宮內(nèi)膜腺癌組(43/55,78.2%),子宮內(nèi)膜不典型生組(5/17,29.4%),正常增殖期子宮內(nèi)膜組(3/12,25.0%),三組比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。cIAP的表達(dá)與55例子宮內(nèi)膜腺癌患者的臨床分期、病理分級(jí)、淋巴轉(zhuǎn)移有關(guān)(P0.05),而與子宮肌層浸潤(rùn)及患者年齡無(wú)關(guān)(P0.05)。Caspase-7蛋白的陽(yáng)性表達(dá)分別為:子宮內(nèi)膜腺癌組(20/55,36.3%),子宮內(nèi)膜不典型增生組(13/17,76.4%),正常增殖期子宮內(nèi)膜組(10/12,83.3%),三組相比差異有統(tǒng)計(jì)學(xué)意義(P0.05),Caspase-7的表達(dá)與55例子宮內(nèi)膜腺癌患者的臨床分期、病理分級(jí)、淋巴轉(zhuǎn)移有關(guān)(P0.05),而與子宮肌層浸潤(rùn)及患者年齡無(wú)關(guān)(P0.05)。采用Spearman相關(guān)分析,cIAP蛋白和Caspase-7蛋白在55例子宮內(nèi)膜腺癌標(biāo)本中表達(dá)呈負(fù)相關(guān)(rs=-0.607,P0.05)。結(jié)論:1.與正常增殖期子宮內(nèi)膜組及子宮內(nèi)膜不典型增生組相比較,凋亡抑制蛋白cIAP在子宮內(nèi)膜腺癌組中高表達(dá),凋亡蛋白Caspase-7在子宮內(nèi)膜腺癌組中低表達(dá),提示細(xì)胞凋亡紊亂可能參與了子宮內(nèi)膜組織的惡性增殖轉(zhuǎn)化。2.cIAP和Caspase-7在子宮內(nèi)膜腺癌中的表達(dá)均與患者病理分級(jí)、臨床分期及有無(wú)淋巴結(jié)轉(zhuǎn)移相關(guān),而與患者年齡和腫瘤侵犯子宮肌層深度無(wú)關(guān),提示二者可能在子宮內(nèi)膜腺癌的侵襲進(jìn)展中發(fā)揮作用。3.cIAP和Caspase-7在子宮內(nèi)膜腺癌組織中的表達(dá)呈負(fù)相關(guān),提示二者在子宮內(nèi)膜腺癌的發(fā)展過(guò)程中相互制約,兩因子聯(lián)合檢測(cè)可能對(duì)評(píng)估子宮內(nèi)膜腺癌的臨床預(yù)后提供理論依據(jù)。
[Abstract]:Objective: To study the expression of apoptosis inhibitory protein cIAP and apoptotic protein Caspase-7 in different endometrium tissues, and analyze the relationship between the expression of two and the clinical biologic characteristics of endometrial adenocarcinoma clinical staging (FIGO 2009), pathological classification, lymph node metastasis, and the correlation between the two expressions, and explore the two in endometrium group. Methods: 84 cases of endometrial tissue in the pathology department of the First Affiliated Hospital of Henan University of Science and Technology were selected for 2008~2010 years. According to the different histological types, the endometrium group (12 cases), endometrial dysplasia (17 cases) and endometrial adenocarcinoma group (55 cases) were treated by different histological types. The experimental data were analyzed by SPSS17.0 statistics, using the chi chi 2 test and Spearman correlation analysis. The difference was statistically significant with P0.05. The clinical stage, pathological grade and lymphatic metastasis (P0.05) were associated with endometrial adenocarcinoma (P0.05), but the positive expression of.Caspase-7 protein in endometrial adenocarcinoma (20/55,36.3%), endometrial atypical hyperplasia (13/17,76.4%), normal proliferative endometrium group (10/12,83.3%), and three groups of phases were not related to the uterine myometrium infiltration and patients' age, respectively. The difference was statistically significant (P0.05). The expression of Caspase-7 was related to the clinical stage, pathological grade and lymphatic metastasis (P0.05) in 55 cases of endometrial adenocarcinoma, but it was not related to the infiltration of myometrium and the age of the patients (P0.05). The expression of cIAP protein and Caspase-7 protein was negative in the specimens of endometrial adenocarcinoma in 55 cases. Conclusion: (rs=-0.607, P0.05) conclusion: 1. compared with the normal proliferative endometrium group and the endometrial dysplasia group, the apoptosis inhibitory protein cIAP is highly expressed in the endometrial adenocarcinoma group, and the apoptotic protein Caspase-7 is low expression in the endometrial adenocarcinoma group, suggesting that the apoptosis disorder may be involved in the malignant proliferation of endometrium. The expression of.2.cIAP and Caspase-7 in endometrial adenocarcinoma is related to the pathological grading, clinical stage and lymph node metastasis, but not related to the age of the patients and the depth of the invasion of the uterine myometrium, suggesting that the two may play.3.cIAP and Caspase-7 in endometrial adenocarcinoma tissue in the progression of endometrial adenocarcinoma. The expression is negatively correlated, suggesting that the two are mutually restricted during the development of endometrial adenocarcinoma. The combined detection of two factors may provide a theoretical basis for the evaluation of the clinical prognosis of endometrial adenocarcinoma.
