【摘要】：背景與目的由于缺乏特異的臨床癥狀,70%的肺癌患者確診時為局部晚期或晚期,多數(shù)患者伴有實性器官轉(zhuǎn)移,不同部位轉(zhuǎn)移患者的臨床表現(xiàn)及預后不同。隨著診斷技術(shù)的發(fā)展,肺癌伴胰腺轉(zhuǎn)移似有逐漸增多趨勢。本研究針對肺癌胰腺轉(zhuǎn)移的臨床特點、診治、預后及生存情況進行統(tǒng)計分析,探討肺癌胰腺轉(zhuǎn)移的相關預后因素。方法回顧性分析1996年7月-2017年6月于北京腫瘤醫(yī)院胸部腫瘤內(nèi)一科就診的35例經(jīng)病理確診的肺癌并胰腺轉(zhuǎn)移或在治療過程中出現(xiàn)胰腺轉(zhuǎn)移的患者,其中33例有完整隨訪資料。結(jié)果 35例患者中,小細胞肺癌28例(80%),腺癌3例(8.6%),鱗癌4例(11.4%)。初治時即存在胰腺轉(zhuǎn)移者21例(60%),14例治療過程中出現(xiàn)胰腺轉(zhuǎn)移(40%)。在胰腺轉(zhuǎn)移灶中,胰頭轉(zhuǎn)移者15例(42.9%),胰腺體尾部轉(zhuǎn)移者20例(57.1%),單發(fā)轉(zhuǎn)移23例(65.7%),多發(fā)轉(zhuǎn)移12例(34.3%)。肺癌胰腺轉(zhuǎn)移患者多無明顯臨床癥狀,本組病例中,僅4例患者在病程中出現(xiàn)胰腺炎癥狀。病理類型是影響肺癌胰腺轉(zhuǎn)移患者生存的獨立預后因素。結(jié)論部分晚期肺癌患者可以出現(xiàn)胰腺轉(zhuǎn)移,多見于小細胞肺癌。肺癌患者出現(xiàn)胰腺轉(zhuǎn)移,治療原則以全身化療為主的綜合治療。病理類型是影響肺癌胰腺轉(zhuǎn)移患者生存的獨立預后因素。
[Abstract]:Background and objective due to the absence of specific clinical symptoms 70% of lung cancer patients were locally advanced or advanced when diagnosed and most of the patients were accompanied with solid organ metastasis. The clinical manifestations and prognosis of patients with different sites of metastasis were different. With the development of diagnostic techniques, lung cancer with pancreatic metastasis seems to increase gradually. In this study, the clinical features, diagnosis and treatment, prognosis and survival of pancreatic metastasis of lung cancer were statistically analyzed, and the prognostic factors of pancreatic metastasis of lung cancer were discussed. Methods from July 1996 to June 2017, 35 cases of lung cancer with pancreatic metastasis or pancreatic metastasis were retrospectively analyzed. 33 of them had complete follow-up data. Results among the 35 patients, 28 (80%) were small cell lung cancer, 3 (8.6%) were adenocarcinoma, 4 (11.4%) were squamous cell carcinoma. Pancreatic metastasis was found in 21 cases (60%) and 14 cases (40%). There were 15 cases of pancreatic head metastasis (42.9%), 20 cases of pancreatic body and tail metastasis (57.1%), 23 cases of single metastasis (65.7%) and 12 cases of multiple metastasis (34.3%). There were no obvious clinical symptoms in patients with pancreatic metastasis of lung cancer, only 4 cases had pancreatitis symptoms in the course of the disease. Pathological type is an independent prognostic factor affecting the survival of patients with pancreatic metastasis from lung cancer. Conclusion Pancreatic metastasis can occur in some patients with advanced lung cancer, and is more common in small cell lung cancer. Lung cancer patients have pancreatic metastasis, the principle of treatment is systemic chemotherapy. Pathological type is an independent prognostic factor affecting the survival of patients with pancreatic metastasis from lung cancer.
