【摘要】：目的探討嬰幼兒反復(fù)喘息的診斷思路。方法分析1例以反復(fù)喘息為主要癥狀的成熟縱隔畸胎瘤患兒的臨床診療過(guò)程,并復(fù)習(xí)相關(guān)文獻(xiàn)。結(jié)果患兒6個(gè)月,咳嗽、喘息病程反復(fù)發(fā)作1個(gè)月,均診斷"毛細(xì)支氣管炎"給予治療,長(zhǎng)期誤診、誤治,直至查胸部X線(xiàn)片提示左肺占位病變,住院后確診成熟縱隔畸胎瘤,手術(shù)切除后癥狀消失。結(jié)論常規(guī)治療嬰幼兒反復(fù)喘息效果不佳時(shí),醫(yī)生需調(diào)整診療思路,綜合考慮呼吸系統(tǒng)、心血管系統(tǒng)或者縱隔疾病等,并重視胸部X線(xiàn)片、過(guò)敏原檢測(cè)及支氣管鏡術(shù)在反復(fù)喘息診斷中發(fā)揮的作用。
[Abstract]:Objective to explore the diagnosis of recurrent wheezing in infants. Methods A case of mature mediastinal teratoma with recurrent wheezing was analyzed. Results after 6 months, the course of cough and wheezing occurred repeatedly for 1 month, all the children were treated with "bronchiolitis", misdiagnosed for a long time, mistreated, until the chest X-ray film showed the left lung occupying lesion, and the mature mediastinal teratoma was diagnosed after hospitalization. The symptoms disappeared after resection. Conclusion when the routine treatment of infantile recurrent wheezing is not good, the doctor should adjust the thinking of diagnosis and treatment, consider the respiratory system, cardiovascular system or mediastinal diseases, and attach importance to the chest radiography. The role of allergen detection and bronchoscopy in the diagnosis of recurrent wheezing.
